What Happens If You Drink Mercury: Risks by Type

Drinking liquid mercury from a broken thermometer is surprisingly unlikely to poison you. Elemental mercury, the shiny silver liquid most people picture, passes through your digestive tract almost entirely unabsorbed. Only about 0.04% of swallowed elemental mercury actually enters your bloodstream. The rest moves through your gut and comes out in your stool. But this doesn’t mean mercury is safe to drink. The form of mercury matters enormously, and some forms can kill you in tiny amounts.

Elemental Mercury: The Silvery Liquid

Elemental mercury is the metallic liquid found in old thermometers, some barometers, and certain industrial equipment. If you swallowed a small amount of this liquid, your body would treat it like an unwelcome guest and usher it toward the exit. Your intestinal lining doesn’t absorb it efficiently, so it passes through largely intact. In studies of people who ingested powdered mercury amalgam, roughly 80% of the mercury was recovered in their feces.

This doesn’t make it harmless. Liquid mercury is extraordinarily dense, about 13.5 times heavier than water. A small volume is surprisingly heavy, and a larger amount could theoretically become lodged in the folds of your intestines. In rare cases, surgery has been needed to remove mercury trapped in the gut. There’s also a secondary risk during vomiting: if mercury comes back up, you could accidentally inhale droplets into your lungs. Inhaled mercury vapor is far more dangerous than swallowed mercury because your lungs absorb it with devastating efficiency.

For most accidental exposures, like a child biting a mercury thermometer, the amount is small enough that the body clears it without treatment. Emergency teams may even prepare for the patient to vomit during transport, since the vomit itself can contain mercury that contaminates the vehicle.

Mercury Salts: A Different Threat Entirely

Mercury salts, particularly mercuric chloride, are a completely different story. These are inorganic compounds that were once used in industrial processes and, historically, as a poison. Swallowing mercury salts is a medical emergency. As little as 0.1 grams can damage body tissues, and 1 to 2 grams can be fatal. For a person weighing about 155 pounds, the lethal dose range documented in poisoning cases has been roughly 29 to 50 milligrams per kilogram of body weight.

Mercury salts are corrosive. They burn through the lining of the mouth, throat, esophagus, and stomach on contact. Symptoms appear rapidly and are severe: intense abdominal pain, bloody diarrhea, vomiting (sometimes with blood), drooling, and a metallic taste. The mucous membranes of the mouth and throat can turn an ashen gray as mercury compounds react with the tissue. Swelling in the throat can become severe enough to obstruct breathing. Blood pressure can drop dangerously low, leading to shock.

In a 1951 case series of 18 people who swallowed mercuric chloride, nine died. Even those who survive the initial poisoning face cascading organ damage, particularly to the kidneys. Kidney failure can occur even with relatively small doses, and some patients require dialysis if their kidneys don’t recover.

Organic Mercury: Slow and Insidious

Organic mercury compounds, like methylmercury found in contaminated fish, behave differently from both elemental mercury and mercury salts. They absorb readily through the gut and accumulate in the body over time. Rather than causing immediate burns or obvious symptoms, organic mercury poisoning typically has a delayed onset. Symptoms can take days to weeks to appear after exposure, and they primarily target the nervous system.

Chronic exposure to organic mercury causes tremors, difficulty with coordination, numbness in the hands and feet, vision and hearing changes, and cognitive impairment. These neurological effects can be permanent. This is the form of mercury most people encounter through dietary exposure, particularly from large predatory fish like swordfish and shark, though the concentrations in commercially available fish are generally low enough that moderate consumption doesn’t cause acute poisoning.

What Mercury Does to the Kidneys

The kidneys are one of mercury’s primary targets regardless of the form. In a study of 172 patients with mercury poisoning, about 27% developed kidney damage. Among those patients, nearly 90% developed nephrotic syndrome, a condition where the kidneys leak large amounts of protein into the urine because their filtering membranes are damaged. The most common underlying pattern of kidney injury was a specific type of inflammation in the kidney’s filtering units.

Kidney damage from mercury can range from temporary protein in the urine to complete kidney failure requiring dialysis. With mercury salt poisoning specifically, kidney failure can develop within hours to days and is one of the leading causes of death.

How Mercury Poisoning Is Treated

Treatment depends entirely on what form of mercury was swallowed and how much. For elemental mercury from a broken thermometer, most cases require no treatment at all. The mercury simply passes through.

For mercury salt ingestion, the situation is urgent. Doctors will not induce vomiting because the caustic compounds would burn the esophagus a second time on the way back up. Instead, they may use activated charcoal, which binds to mercury compounds in the gut and reduces absorption. They’ll often use a camera to look down the throat and assess how badly the esophagus and stomach have been burned.

For significant poisoning from any form, chelation therapy is the primary medical treatment. Chelating agents are drugs that bind to mercury in the bloodstream and pull it out through the urine. Newer chelating agents can be taken by mouth and are more effective with fewer side effects than older treatments. One of these is now the recommended treatment for organic mercury and lead poisoning. These drugs work by mobilizing mercury deposits from tissues and flushing them from the body.

Diagnosing mercury poisoning through lab tests can be tricky. Blood and urine mercury levels don’t always correlate well with symptoms. Levels can range from slightly above normal to astronomical (documented as high as 16,000 micrograms per liter in blood). Interpretation is straightforward when levels are extremely high but becomes difficult as they approach normal population ranges of under 10 to 20 micrograms per liter.

Why the Form of Mercury Matters So Much

The difference between swallowing a bead of liquid mercury from a thermometer and swallowing a mercury salt compound is the difference between a non-event and a life-threatening emergency. Elemental mercury sits in your gut like a ball bearing, too heavy and chemically stable to cross your intestinal wall in meaningful amounts. Mercury salts dissolve readily, burn tissue on contact, and absorb quickly enough to poison your organs. Organic mercury absorbs easily and accumulates quietly in your brain and nervous system over time.

If someone has swallowed any mercury compound other than a small amount of elemental mercury, or if there’s any uncertainty about what form was ingested, it should be treated as a poisoning emergency. Even with elemental mercury, inhaling the vapor from a spill in an enclosed space poses a serious risk that the simple act of swallowing does not.
