Drinking raw milk may cause nothing at all, or it may make you seriously ill. The difference depends on whether the specific batch you drink happens to carry dangerous bacteria, and whether your immune system can fight them off. Raw milk can harbor Salmonella, E. coli, Listeria, and Campylobacter, all of which cause foodborne illness ranging from a few days of misery to life-threatening complications. Despite its growing popularity, unpasteurized milk consumed by only about 3.2% of the U.S. population is responsible for 96% of all illnesses caused by contaminated dairy products.
The Bacteria You’re Gambling On
Pasteurization exists because raw milk is an ideal environment for bacterial growth. The pathogens most commonly found in unpasteurized milk include Salmonella, E. coli (including the dangerous O157 strain), Listeria, and Campylobacter. These aren’t rare contaminants that show up only in poorly managed farms. In a 2008 outbreak tied to a single Connecticut dairy that had met all state regulatory standards, 14 people fell ill from E. coli in raw milk. Five were hospitalized, and three developed hemolytic uremic syndrome, a serious kidney condition.
The number of raw milk outbreaks increased steadily from 2007 to 2012, tracking alongside the growing number of states that legalized its sale. About 81% of those outbreaks occurred in states where raw milk sales were legal.
What the Symptoms Feel Like and When They Start
If you drink contaminated raw milk, you won’t know right away. The timeline depends on which pathogen you’ve ingested. Salmonella symptoms typically appear within 6 hours to 6 days. Campylobacter takes longer, usually 2 to 5 days before you feel anything. That delay makes it easy to blame something else entirely.
The symptoms themselves are what you’d expect from food poisoning: vomiting, diarrhea, abdominal cramping, fever, headache, and body aches. For most healthy adults, this resolves on its own within several days. But “most” is doing heavy lifting in that sentence.
Who Faces the Worst Outcomes
Children under five are at significantly higher risk of severe complications. A quantitative risk assessment found that for every 100,000 servings of raw milk consumed, the expected number of hemolytic uremic syndrome cases per year was 52 for children five and under, compared to just 3.2 for people older than five. Hemolytic uremic syndrome damages red blood cells and can lead to kidney failure.
Pregnant women face a distinct threat from Listeria, which is often found in raw milk. Listeria infection during pregnancy can cause miscarriage, stillbirth, or severe illness in a newborn. Listeria is particularly dangerous because it can grow even at refrigerator temperatures, meaning properly stored raw milk isn’t necessarily safe milk.
People with weakened immune systems, older adults, and anyone with chronic illness also face disproportionate risk from these same pathogens.
The Bird Flu Factor
Since 2024, there’s a new concern. The FDA has found infectious H5N1 avian influenza virus in raw milk samples, with 39 out of 158 samples testing positive for live virus at concentrations of roughly 3,000 virus particles per milliliter. Pasteurized dairy products have consistently tested negative for viable H5N1, with all 464 samples the FDA tested showing no infectious virus.
Even aging raw milk cheese for 60 days or longer, the process traditionally thought to make raw cheese safer, does not eliminate H5N1. The virus survived through and beyond the standard 60-day aging period. Standard pasteurization temperatures easily inactivate it.
Do You Actually Get More Nutrients?
One of the main reasons people seek out raw milk is the belief that pasteurization destroys important nutrients. The reality is less dramatic. Vitamins present at high levels in milk, including riboflavin, B6, and B12, are heat-stable and survive pasteurization with minimal loss. The only vitamin significantly affected is vitamin C, which drops by roughly 15 to 17% during standard pasteurization. But milk is not a meaningful source of vitamin C to begin with. A glass of raw milk contains about 23 milligrams per liter, a fraction of what you’d get from a single orange.
Most naturally occurring enzymes in milk also survive pasteurization largely intact. The protein-digesting enzyme plasmin retains a large percentage of its activity even after pasteurization, and lysozyme loses only about 6% of its activity under conditions much more intense than standard processing.
The Allergy and Asthma Claims
Some research does suggest a connection between farm milk consumption and lower rates of childhood asthma. In the GABRIELA birth cohort study, children who consumed unprocessed farm milk had lower asthma rates by age six compared to children who drank store-bought milk. Higher-fat, unprocessed milk was associated with higher levels of omega-3 fatty acids, which correlated inversely with asthma and markers of inflammation.
There’s an important caveat here. These studies looked at children growing up on farms, drinking milk from cows they lived near, in a broader environment that includes exposure to barn dust, animal dander, and diverse microbes. Researchers have not been able to isolate raw milk itself as the protective factor versus the overall farm environment. Buying raw milk from a store and drinking it in a city apartment is a very different exposure profile than growing up on a dairy farm.
Does Raw Milk Help With Lactose Intolerance?
Another common claim is that lactose-intolerant people can drink raw milk without symptoms. A randomized controlled trial tested this directly, having participants drink 24 ounces of raw milk, pasteurized milk, and soy milk in a crossover design. Of 63 participants, 43% tested positive for lactose malabsorption. The study measured self-reported severity of gas, diarrhea, bowel sounds, and cramping. Raw milk did not produce meaningfully different symptom scores compared to pasteurized milk in people with confirmed lactose malabsorption. The enzymes naturally present in raw milk do not appear to pre-digest enough lactose to make a practical difference.
Where It’s Legal to Buy
Federal law prohibits selling raw milk across state lines. Within states, the rules vary widely. Some states allow raw milk in retail stores. Others restrict sales to the farm where the milk was produced. A growing number of states permit “herd-share” arrangements, where you buy partial ownership of a cow and receive a portion of its milk, sidestepping retail sale laws. Some states still prohibit all sales of unpasteurized milk to consumers. The patchwork of regulations means your access depends entirely on where you live.
