What Happens If You Only Take One Dose of Antibiotics?

Taking just one dose of antibiotics and stopping won’t complete treatment for most infections. That single dose may kill some bacteria, but it typically won’t eliminate enough of them to resolve the infection. What happens next depends on the type of infection, which antibiotic was prescribed, and how your immune system responds.

What One Dose Does Inside Your Body

When you take an antibiotic, it enters your bloodstream and begins killing bacteria quickly. Research from the American Society for Microbiology shows that antibiotics produce a rapid initial killing phase, wiping out a large portion of actively growing bacteria in the first hours. But this fast kill is followed by a sharp decline in effectiveness, because not all bacteria are in the same state. Some are in a resting phase, growing slowly or not at all, which makes them much harder for the antibiotic to reach.

A single dose creates a temporary spike of the drug in your blood. Within hours, your body metabolizes and eliminates it. The bacteria that survived, whether because they were in that resting state or because the drug level dropped too quickly, are now free to start multiplying again. For most infections, one dose simply isn’t enough time at a high enough concentration to clear the bacterial population.

The Infection Can Come Back, Sometimes Worse

The most straightforward risk of stopping after one dose is that your infection doesn’t go away. You might feel slightly better for a day as the initial bacterial load drops, but the surviving bacteria rebound. In some cases, the returning infection can be harder to treat than the original one.

This is especially true for serious or deep-seated infections. Conditions like bone infections, heart valve infections, and tuberculosis can harbor small numbers of bacteria that persist even when symptoms improve dramatically. Stopping treatment early in these cases significantly raises the chance of relapse. Strep throat in people at risk of rheumatic fever is another example where full eradication of the bacteria matters more than symptom relief, because the real danger is the immune reaction that can follow, not just the sore throat itself.

For milder infections, your immune system may be able to pick up where the single dose left off and finish the job on its own. A mild skin infection or a urinary tract infection that was just getting started might resolve even with incomplete treatment, but you’re essentially gambling on your immune system’s ability to handle what the antibiotic didn’t finish.

The Resistance Problem

One of the more concerning consequences of partial antibiotic exposure is the potential for resistance. When you take a single dose, you create what scientists call selective pressure. The antibiotic kills off the most vulnerable bacteria, but any that have even slight natural resistance survive. Those survivors then multiply without competition from the bacteria that were just eliminated.

Research published in the American Society for Microbiology’s journals has shown that resistance genes sitting on mobile genetic elements can actually transfer between bacteria. When susceptible bacteria die, they release fragments of their DNA, including resistance genes. Surviving bacteria can pick up those genes and incorporate them, becoming resistant to antibiotics they’ve never even been directly exposed to. In one documented case, resistance genes for an unrelated antibiotic amplified in a patient’s gut bacteria within just three days of starting treatment. A single incomplete exposure adds to this cycle.

This doesn’t mean one missed course will create a superbug. But across millions of people making the same choice, partial antibiotic use is one of the forces driving the broader resistance crisis.

When One Dose Is Actually the Full Course

There are genuine exceptions where a single dose is the entire prescribed treatment. Some antibiotics are specifically designed to work this way. Fosfomycin, commonly prescribed for uncomplicated urinary tract infections, is a single-dose powder mixed in water. A single large dose of azithromycin is the standard treatment for chlamydia. These drugs either concentrate in the target tissue and stay active for days, or they reach such high local levels that one dose is genuinely sufficient.

For more serious infections, single-dose options also exist. The FDA approved oritavancin, an intravenous antibiotic, as a one-time infusion for acute bacterial skin infections caused by bacteria like Staphylococcus aureus. The drug’s long half-life keeps it working in the body for days after a single administration. If your doctor prescribed a single-dose antibiotic, taking just that one dose is exactly what you should do.

Shorter Courses May Work for Some Infections

The traditional advice to “always finish the full course” is more nuanced than it used to be. A widely discussed analysis in the BMJ reviewed data on treatment length for several common infections, including strep throat, skin infections, and pneumonia. For most of these conditions, shorter courses produced cure rates identical to longer ones. The notable exception was middle ear infections in children, where ten days of antibiotics clearly outperformed five days.

But “shorter course” still means multiple days of treatment, not one dose. The takeaway from this research is that some prescribed courses may be longer than strictly necessary, not that a single dose is sufficient. The difference between a five-day course and a seven-day course is very different from the difference between one dose and a seven-day course.

What to Do If You’ve Only Taken One Dose

If you took one dose and then stopped, whether because of side effects, forgetfulness, or a decision to quit, the best step is to resume the medication as soon as possible. If very little time has passed, simply take your next scheduled dose and continue the course normally. Don’t double up to compensate for the gap, as that increases the risk of side effects without meaningfully improving effectiveness.

If several days have gone by since that single dose, the situation is different. Your doctor may need to reassess whether the original prescription is still appropriate, restart the course from the beginning, or extend treatment. This is especially important if your symptoms have worsened or returned.

Side effects are one of the most common reasons people stop after one dose. If the antibiotic caused nausea, a rash, or diarrhea, that’s worth reporting rather than quietly abandoning treatment. There are usually alternative antibiotics that treat the same infection with a different side-effect profile. Stopping and saying nothing leaves the infection untreated and the side-effect problem unsolved for next time.
