What Happens When Blood Glucose Is Too High?

When blood glucose climbs too high, your body starts showing signs in a predictable sequence. Symptoms typically don’t appear until levels exceed 180 to 200 mg/dL, but once they do, the effects range from frequent urination and fatigue to life-threatening emergencies. What happens in your body depends on how high the glucose goes, how quickly it rises, and how long it stays elevated.

The First Symptoms You’ll Notice

The earliest signs of high blood sugar are driven by your kidneys trying to flush the excess glucose out of your bloodstream. When glucose levels cross roughly 180 mg/dL, your kidneys can no longer reabsorb all of it, and the overflow spills into your urine. That excess sugar pulls water along with it through a process called osmotic diuresis, which is why frequent urination is almost always the first symptom. The water loss then triggers increased thirst as your body tries to compensate.

Other early symptoms include blurred vision, feeling unusually tired, and general weakness. These can develop gradually, which makes them easy to dismiss or blame on poor sleep, stress, or aging. Many people walk around with blood sugar well above normal for weeks or months without connecting the dots.

What’s Happening Inside Your Body

Glucose is fuel, but too much of it in the bloodstream acts more like a toxin. At the cellular level, excess glucose triggers a cascade of damaging reactions. It increases oxidative stress in the cells lining your blood vessels, making them stickier and more inflamed. Immune cells called macrophages ramp up their release of inflammatory signals. Smooth muscle cells in artery walls begin to multiply and migrate in abnormal ways. Proteins throughout the body get coated with sugar molecules in a process called glycation, which alters how they function.

These changes aren’t dramatic in the short term. You won’t feel your arteries becoming inflamed or your proteins getting damaged. But they accumulate, and over time they set the stage for serious complications in nearly every organ system.

When High Blood Sugar Becomes an Emergency

Severely elevated blood sugar can trigger two distinct medical emergencies, both of which require hospital treatment.

The first is diabetic ketoacidosis, or DKA. This happens most often in type 1 diabetes when the body has virtually no insulin. Without insulin, cells can’t use glucose for energy, so the body breaks down fat instead. That fat breakdown produces acids called ketones, which build up in the blood and make it dangerously acidic. Blood sugar in DKA is typically above 250 mg/dL and often between 350 and 500 mg/dL. Warning signs include fruity-smelling breath, nausea and vomiting, abdominal pain, shortness of breath, and confusion. If your blood sugar is 240 mg/dL or higher, checking your urine for ketones with an over-the-counter test kit can help you catch DKA before it progresses.

The second emergency, called hyperosmolar hyperglycemic state (HHS), is more common in type 2 diabetes and tends to develop more slowly. Blood sugar in HHS often exceeds 800 mg/dL. The extreme glucose concentration makes the blood thick and syrupy, pulling massive amounts of water out of tissues and causing severe dehydration. Unlike DKA, HHS doesn’t produce significant ketones, so the fruity breath and rapid breathing are usually absent. Instead, it presents with profound confusion, extreme thirst, and eventually loss of consciousness. HHS carries a higher mortality rate than DKA, partly because it tends to affect older adults with other health conditions.

How Your Brain Is Affected

High blood sugar affects your brain in ways that aren’t immediately obvious. The damage accumulates quietly over time, and most people don’t realize their thinking is being impacted. Chronically elevated glucose is associated with changes in cognitive function, including difficulties with memory and processing speed. The tricky part is that these effects develop gradually enough that they feel like normal aging rather than a consequence of blood sugar control.

In the short term, very high blood sugar can cause confusion and difficulty concentrating. These acute cognitive symptoms are more noticeable during hyperglycemic emergencies, but subtler versions can occur during everyday glucose spikes as well.

Long-Term Damage to Blood Vessels and Nerves

Chronic high blood sugar damages the body through two main pathways: injury to small blood vessels and injury to large blood vessels.

Small vessel damage affects the organs that depend on tiny, delicate capillaries. The eyes are particularly vulnerable. High glucose damages the small blood vessels in the retina, which can lead to vision problems and, if untreated, blindness. The kidneys rely on millions of microscopic filtering units, and sustained high blood sugar gradually destroys them, potentially leading to kidney failure. Nerve damage, especially in the feet and hands, causes numbness, tingling, and pain. This nerve damage also contributes to slow wound healing, which is why foot ulcers are a common and serious complication of poorly controlled diabetes.

Large vessel damage accelerates atherosclerosis, the buildup of plaques in major arteries. Research published in Circulation Research describes elevated blood glucose as a cardiovascular risk factor second only to age. The mechanisms are well documented: glucose-driven inflammation makes artery walls more prone to plaque formation, and glycation of proteins further destabilizes those plaques. The result is a significantly higher risk of heart attack and stroke.

Your Immune System Takes a Hit

High blood sugar forces your immune system to work harder while simultaneously making it less effective. White blood cells, the core of your immune defense, function poorly when glucose levels are elevated. At the same time, the body mounts an inflammatory response to the excess sugar, diverting immune resources toward managing that internal stress rather than fighting off infections.

The practical consequence is that people with persistently high blood sugar get sick more often and recover more slowly. Respiratory tract infections, flu, pneumonia, urinary tract infections, and skin infections all occur at higher rates. This increased vulnerability isn’t limited to people with diagnosed diabetes. Anyone with chronically elevated blood sugar faces some degree of immune impairment.

How Quickly the Damage Develops

The timeline varies enormously depending on the severity and duration of the glucose elevation. A single spike to 250 mg/dL after a large meal, while not ideal, is unlikely to cause lasting harm in someone who otherwise has good blood sugar control. Sustained levels above 180 to 200 mg/dL over weeks and months, however, begin the slow accumulation of vascular, nerve, and organ damage described above.

Emergency complications like DKA can develop in hours, especially in type 1 diabetes. HHS tends to build over days to weeks. The long-term complications, including eye disease, kidney disease, nerve damage, and cardiovascular disease, typically emerge over years of inadequately controlled blood sugar, though the process starts sooner than most people expect. By the time symptoms of these complications appear, significant damage has already occurred, which is why regular monitoring matters even when you feel fine.
