When you stop drinking after prolonged heavy use, your brain and body go through a rebound period that can range from uncomfortable to life-threatening. Symptoms typically begin within 6 to 24 hours of your last drink, peak between 24 and 72 hours, and gradually ease over the following days. For most people, the worst is over within a week, but some psychological symptoms can linger for weeks or months.
Alcohol detox is one of the few substance withdrawals that can be fatal without treatment. Understanding what to expect, and what crosses the line from manageable to dangerous, can help you or someone you care about navigate it safely.
Why Your Brain Reacts So Strongly
Alcohol enhances the activity of your brain’s main calming chemical (GABA) and suppresses its main excitatory chemical (glutamate). Drink heavily and often enough, and your brain adapts. It dials down its own calming signals and ramps up excitatory ones to compensate for the constant sedation alcohol provides. Your brain essentially recalibrates to function with alcohol on board.
When you suddenly remove alcohol, that recalibration backfires. You’re left with suppressed calming activity, a reduced number of calming receptors, and an overactive excitatory system all at once. This imbalance creates a state of neuronal hyperarousal, which is responsible for nearly every withdrawal symptom you experience: the anxiety, the tremors, the racing heart, the insomnia, and in severe cases, seizures. Research confirms that glutamate levels are measurably elevated on the first day of detox, and it takes roughly two weeks for the balance between excitatory and calming brain chemicals to begin normalizing.
The First 72 Hours
6 to 12 Hours
The earliest symptoms tend to be mild and easy to dismiss. Headache, mild anxiety, nausea, insomnia, and slight hand tremors are common. Your heart rate and blood pressure may rise. Some people feel irritable or restless without being able to pinpoint why. These symptoms can start as early as two hours after your last drink if you’ve been drinking very heavily.
12 to 24 Hours
Symptoms intensify. Sweating becomes more noticeable. Nausea may worsen. Some people experience hallucinations during this window, typically seeing, hearing, or feeling things that aren’t there. Unlike hallucinations in delirium tremens (which come later), you’re usually aware that what you’re experiencing isn’t real. 24 to 48 Hours
This is the highest-risk window for seizures. Alcohol withdrawal seizures are generalized tonic-clonic seizures, meaning they involve the whole body, with loss of consciousness, muscle stiffening, and convulsions. They can occur without any prior warning symptoms. Not everyone who goes through withdrawal will seize, but the risk is real for people with a history of heavy, prolonged drinking or previous withdrawal episodes.
48 to 72 Hours
For most people with mild to moderate withdrawal, symptoms peak somewhere in this range and then start improving. But for a smaller group, this is when the most dangerous complication can appear: delirium tremens.
Delirium Tremens
Delirium tremens (DTs) is the most severe form of alcohol withdrawal. It involves profound confusion, agitation, fever, rapid heartbeat, and hallucinations that feel completely real. People experiencing DTs often don’t know where they are, what day it is, or what’s happening to them. It typically appears 48 to 72 hours after the last drink, though onset can sometimes be delayed.
DTs are relatively uncommon in the general population, affecting roughly 0.2% to 0.7% of people. But among heavy, long-term drinkers going through withdrawal, the rate is significantly higher. The critical number to know: untreated delirium tremens carries a mortality rate of 15% to 20%. With proper medical treatment, that drops to approximately 1%. This is the main reason alcohol detox should not be treated casually.
What Medical Detox Looks Like
If you go through a supervised detox program, medical staff will monitor your symptoms using a standardized scoring system that tracks 10 specific signs: nausea, tremor, sweating, anxiety, agitation, tactile disturbances (like itching or burning sensations), auditory disturbances, visual disturbances, headache, and mental clarity. Your score determines whether you need medication and how much.
Scores below 8 to 10 indicate mild withdrawal that often doesn’t require medication. Scores between 8 and 15 suggest moderate withdrawal with noticeable physical symptoms like elevated heart rate and blood pressure. Scores above 15 signal severe withdrawal and the possibility of delirium tremens approaching.
The primary medications used are sedatives that work on the same calming brain receptors that alcohol affects, essentially smoothing the transition by preventing the hyperarousal state from becoming dangerous. Doctors may use one of three approaches: symptom-triggered dosing (medication only when your score crosses a threshold), fixed dosing (a set schedule that gradually tapers down over several days), or front-loading (higher doses early on to get symptoms under control quickly). The approach depends on how severe your withdrawal is and the clinical setting.
Additional medications can be added to manage specific symptoms like elevated blood pressure or rapid heart rate, but these are always used alongside the primary sedative, never alone, because they don’t prevent seizures or delirium.
Inpatient vs. Outpatient Detox
Not everyone needs to detox in a hospital. The decision depends on how severe your withdrawal is likely to be, your medical history, and your living situation. Mild withdrawal can often be managed with outpatient visits, where you check in daily or less frequently for monitoring. Moderate withdrawal may work in an outpatient setting with extended monitoring, provided you have a stable, supportive home environment and someone who can watch for worsening symptoms.
Inpatient detox, with 24-hour nursing care and physician oversight, is appropriate for severe withdrawal or when there’s a significant risk of complications. After the Acute Phase
Most physical withdrawal symptoms resolve within a week. But for many people, a second, subtler phase follows. Known as post-acute withdrawal syndrome (PAWS), it involves primarily psychological and mood-related symptoms: anxiety, irritability, difficulty concentrating, sleep disturbances, low energy, and emotional flatness. These symptoms tend to fluctuate rather than remain constant. You might feel fine for a few days, then have a stretch where cravings, restlessness, or low mood return without obvious triggers.

Withdrawal doesn’t hit everyone the same way. Several factors influence how severe your experience will be:
