What Happens When You Donate Plasma, Step by Step

When you donate plasma, a machine draws your blood, spins out the liquid plasma portion, and returns your red blood cells back into your body. The whole process typically takes about 60 to 90 minutes, and it’s more involved than a standard blood donation. Here’s what happens at each stage.

Check-in and Health Screening

Every visit starts with a health screening to confirm you’re eligible and feeling well enough to donate. Staff will take a small blood sample and check your vital signs: blood pressure, pulse, and temperature. You’ll also answer questions about your recent health, medications, travel, and any new tattoos or piercings.

If it’s your first time, expect the visit to take longer. First-time donors receive a brief physical exam from a trained medical specialist, and you’ll go through a more extensive medical history review that includes testing for hepatitis and HIV. After that initial visit, you’ll receive a physical at least once a year to make sure you’re still in good health. To be eligible in the first place, you generally need to be at least 18, weigh 110 pounds or more, and not have gotten a tattoo or piercing in the last four months.

The Needle and the Machine

Once you’re cleared, you’ll sit in a reclining chair and a staff member will insert a needle into a vein in your arm, similar to a standard blood draw. The needle connects to a specialized machine that does the work of separating your blood components automatically.

The machine draws a small amount of blood at a time, then spins it in a centrifuge. That spinning force separates your blood into layers: red blood cells, white blood cells, platelets, and plasma. The machine collects the plasma (the pale yellow liquid that makes up about 55% of your blood) into a collection bag. Then it combines your remaining blood cells with a saline solution and sends them back through the same needle into your arm. This cycle of drawing, separating, and returning repeats several times throughout the donation.

What Citrate Does (and Why You Might Feel Tingling)

The machine adds a substance called citrate to your blood to prevent it from clotting inside the tubing. Some of that citrate enters your bloodstream when your blood cells are returned. Citrate temporarily lowers calcium levels in your body, which can cause tingling in your fingers or toes, chills, or a feeling of vibration around your lips. These sensations are usually mild and go away on their own. Staff at the donation center monitor for these reactions and can slow the machine down or offer calcium supplements if you’re uncomfortable.

After the Needle Comes Out

When the collection is complete, staff will remove the needle and bandage your arm. You’ll be asked to sit for a few minutes before leaving. Most people feel fine, though some experience mild fatigue or lightheadedness.

Hydration makes the biggest difference in how you feel afterward. Drinking plenty of water before and after your appointment helps your body rebuild its plasma volume quickly. Pairing that with iron-rich foods (dried apricots, fortified cereals, pumpkin seeds) and protein sources like eggs, Greek yogurt, or peanut butter on whole-grain crackers supports recovery. Combining iron-rich snacks with vitamin C, like orange slices or strawberries, helps your body absorb the iron more efficiently. A salty snack like pretzels can help maintain blood pressure if you’re feeling lightheaded.

How Often You Can Donate

Because your red blood cells are returned to you, plasma donation is easier on your body than whole blood donation, and you can do it more frequently. Most collection centers allow donations twice per week with at least one day between visits. Federal regulations set the specific limits, and individual centers may have their own policies on top of those.

Long-term studies have found that people who donate plasma regularly over extended periods generally don’t experience lasting health problems. That said, if you’re donating frequently, it’s worth periodically checking in with a healthcare provider to monitor iron levels and immunoglobulin levels (the antibody proteins in your blood that help fight infections).

Where Your Plasma Goes

Donated plasma gets manufactured into therapies for people with serious and often rare medical conditions. The scale of what’s needed is striking: treating one person with a primary immunodeficiency for a single year requires 130 plasma donations. Someone with hemophilia needs 1,200 donations per year. A person with a chronic nerve condition called CIDP requires 465 donations annually.

Plasma is used to create clotting factors for people with bleeding disorders, immune therapies for people whose bodies can’t produce enough antibodies on their own, and treatments for genetic lung conditions. It’s also given directly to trauma patients and burn victims to help with blood clotting and prevent shock. Beyond those uses, plasma-derived therapies treat tetanus, rabies, and a pregnancy complication called Rh sensitization where a mother’s immune system attacks her baby’s blood cells.

Unlike many medications that can be synthesized in a lab, these plasma-based therapies can only come from human donors. There is no artificial substitute, which is why collection centers are constantly recruiting.
