A mini stroke, known medically as a transient ischemic attack (TIA), happens when blood flow to part of your brain is temporarily blocked. Symptoms come on suddenly and usually disappear within minutes to an hour, though they can last up to 24 hours in rare cases. Despite the short duration, a TIA is a medical emergency. About 4% of people who have one will experience a full stroke within 90 days, with the highest risk concentrated in the first 48 hours.
What Happens Inside Your Brain
A TIA follows the same basic mechanism as a stroke. A blood clot or piece of debris (often from fatty buildup in an artery) temporarily blocks a vessel supplying your brain. Brain cells in that area stop getting the oxygen and nutrients they need. The difference between a TIA and a full stroke is that the blockage clears on its own before it kills brain tissue. Blood flow resumes, and symptoms fade.
This distinction is important because doctors now define a TIA based on whether brain tissue was permanently damaged, not just on how long symptoms lasted. If an MRI shows that tissue actually died, it’s classified as a stroke even if your symptoms resolved quickly. That’s why hospitals push for brain imaging within 24 hours of a suspected TIA.
Symptoms You’ll Notice
TIA symptoms appear without warning and mirror those of a full stroke. The most common include:
- Weakness, numbness, or paralysis in your face, arm, or leg, typically on one side of your body
- Slurred speech or difficulty understanding what others are saying
- Vision changes, including blindness in one or both eyes or double vision
- Dizziness, loss of balance, or coordination problems
Most people experience symptoms for only a few minutes. Because they resolve so quickly, it’s tempting to brush them off. That’s dangerous. The temporary nature of a TIA doesn’t mean the underlying problem has gone away. The same clot source or narrowed artery that caused the TIA can produce a full, permanent stroke at any time.
The Risk of a Full Stroke Afterward
A TIA is often called a warning stroke for good reason. Data from clinical trials shows that 1.3% of TIA patients have an ischemic stroke within 2 days, 2% within 7 days, and 4.1% within 90 days. The risk is front-loaded: the first two days are the most dangerous window. This is why emergency treatment matters even though TIA symptoms have already resolved by the time most people reach the hospital.
Getting treatment quickly makes a real difference. When doctors start blood-thinning medications within 24 hours, the benefit of preventing a recurrent stroke is at its peak. Waiting days or weeks to follow up dramatically reduces that protective window.
What Happens at the Hospital
Even though your symptoms may have resolved completely, expect a thorough workup. Doctors need to find out what caused the blockage and whether your brain sustained any permanent damage. A typical evaluation includes several components.
First, you’ll get a neurological exam testing your vision, speech, strength, reflexes, and sensation. Your doctor may listen to the carotid arteries in your neck with a stethoscope, checking for a whooshing sound that can signal fatty buildup narrowing the vessel. They may also look at the tiny blood vessels in the back of your eye for signs of debris.
Imaging comes next. An MRI is the gold standard because it can detect even small areas of brain damage that a CT scan might miss. You’ll likely also get imaging of the blood vessels in your neck and head, either through ultrasound of the carotid arteries or a CT or MRI angiogram that maps the vessels more completely. If doctors suspect a heart problem sent a clot to your brain, you may get an echocardiogram, an ultrasound of the heart that checks for clot-forming conditions.
Blood work rounds out the picture. Doctors check for stroke risk factors like high blood sugar, high cholesterol, and elevated blood pressure. These results help shape your prevention plan going forward.
Immediate Treatment
The primary goal after a TIA is preventing a full stroke. In most cases, doctors start a combination of two blood-thinning medications (typically aspirin plus a second agent) in the emergency room. This dual approach, maintained for about 21 days, has been shown to maximize the benefit of preventing another clot while keeping bleeding risk manageable. After that initial period, most patients continue on a single blood thinner long term.
If imaging reveals a significantly narrowed carotid artery, your doctor may recommend a procedure to open it up. If a heart rhythm problem like atrial fibrillation caused the clot, a different class of blood-thinning medication is used instead.
Long-Term Prevention
After the immediate crisis, the focus shifts to controlling the risk factors that led to the TIA in the first place. Blood pressure management is the single most important long-term change. Guidelines recommend keeping blood pressure below 130/80 for most people who’ve had a TIA. If you weren’t on blood pressure medication before, you likely will be now.
Cholesterol management, blood sugar control for people with diabetes, quitting smoking, regular physical activity, and dietary changes all play supporting roles. These aren’t vague lifestyle suggestions. Each one directly reduces the odds of a future stroke, and your doctor will set specific targets based on your test results. The combination of medication and lifestyle changes is substantially more effective than either one alone.
Cognitive Effects You Might Not Expect
Because a TIA doesn’t cause permanent brain damage visible on most scans, many people assume they’ll feel completely normal afterward. That’s not always the case. Research shows that more than a third of TIA patients have measurable cognitive difficulties within three months. Working memory (the ability to hold and manipulate information in your head) is the most commonly affected, impaired in about 25% of patients. Attention and processing speed are also frequently affected, at 22% and 16% respectively.
These aren’t dramatic impairments. You might notice you’re slower to process conversations, have more trouble multitasking, or feel mentally foggy in ways that are hard to describe. Interestingly, long-term memory tends to be spared. The cognitive effects likely reflect subtle disruptions to blood flow rather than dead tissue, but they’re real and can affect daily life. Depression and anxiety rates, somewhat surprisingly, don’t appear to be significantly elevated after a TIA compared to the general population. However, the psychological weight of knowing you’ve had a warning stroke can be considerable, and many people report heightened anxiety about their health in the months that follow.
