A panic attack is a sudden surge of intense fear that triggers a powerful physical reaction, even when there’s no actual danger present. Symptoms usually peak within 10 minutes of starting and can include a racing heart, chest pain, shortness of breath, and a terrifying sense that something is seriously wrong. Understanding what’s happening inside your body during those minutes can make the experience less frightening and easier to manage.

What Your Brain Does First

A panic attack begins in a small, almond-shaped structure deep in your brain that processes emotions. When this area perceives a threat, whether real or not, it fires off a distress signal to a nearby region that acts like a command center for your nervous system. That command center immediately activates your sympathetic nervous system, which is essentially the gas pedal for your body’s fight-or-flight response.

Within seconds, signals travel through your nerves to your adrenal glands, which sit on top of your kidneys. These glands respond by flooding your bloodstream with adrenaline. This is the hormone responsible for most of what you feel during a panic attack: your heart pounding, your muscles tensing, your breathing speeding up. Your body is preparing to fight or run from danger that isn’t actually there.

If the brain continues to register danger, a second stress system kicks in. This one is slower but longer-lasting, ultimately prompting your adrenal glands to release cortisol, a stress hormone that keeps your body in a heightened state of alertness. This two-wave hormonal response explains why a panic attack can feel like it comes in surges rather than hitting all at once and fading smoothly.

Why You Hyperventilate and What It Does

One of the most common features of a panic attack is hyperventilation, breathing faster and deeper than your body needs. This feels like you’re suffocating, but the opposite is happening: you’re expelling too much carbon dioxide. That creates a chemical imbalance in your blood called respiratory alkalosis, which narrows blood vessels, including the ones that supply your brain.

This is what produces the dizziness, the lightheadedness, the tingling in your fingers and lips. It can also make your heart beat faster and leave you feeling even more breathless, which makes you breathe harder, which drops your carbon dioxide further. It’s a feedback loop that can escalate a panic attack from uncomfortable to overwhelming in under a minute. Slow, deliberate breathing breaks the cycle by letting carbon dioxide levels normalize.

The Timeline of an Attack

Panic attacks begin suddenly. There’s no gradual buildup the way ordinary anxiety slowly intensifies over hours. Most attacks reach their peak intensity within 10 minutes or less. Some are remarkably brief, lasting only 1 to 5 minutes before easing.

That said, the experience isn’t always a single clean wave. Multiple attacks of different intensities can roll through over several hours, like ocean waves, making it feel like one continuous episode. Even after the worst of it passes, residual symptoms like muscle soreness, a jittery feeling, or a lingering sense of unease can stick around for much longer.

Why It Can Feel Like a Heart Attack

Chest pain, a racing heart, shortness of breath, sweating, nausea: the overlap between a panic attack and a heart attack is significant enough that emergency rooms see it regularly. If you’ve never had a panic attack before, it’s reasonable to get checked out, because the symptoms alone aren’t enough to tell the difference with certainty.

There are some general patterns that can help. Panic attack chest pain tends to be sharp or stabbing and often stays in one spot, while heart attack pain is more commonly described as a squeezing pressure that may radiate to the jaw, arm, or back. Panic attacks also tend to peak and then improve over 10 to 20 minutes, while cardiac symptoms typically persist or worsen. But these are generalizations. The only way to rule out a cardiac event is through medical evaluation, especially the first time it happens.

What Happens After the Attack Ends

Once your brain stops perceiving danger, your parasympathetic nervous system takes over. This is the counterpart to the fight-or-flight system, sometimes called the “rest and digest” response. It works to slow your heart rate, relax your muscles, and bring your breathing back to normal. But the recovery isn’t instant.

Your body just burned through a significant amount of energy. Adrenaline and cortisol are still circulating and take time to clear. These two treatments are considered first-line for panic disorder, and they work differently: therapy helps you change how you respond to panic sensations, while medication reduces the frequency and severity of attacks over time. When medication is used, the typical recommendation is to continue for 6 to 12 months once symptoms stabilize.

Current clinical guidance actually discourages the use of fast-acting anti-anxiety medications on an as-needed basis for panic attacks. The concern is that relying on a pill to stop an attack reinforces the idea that the attack itself is dangerous and that you can’t get through it on your own, which works against the goals of therapy. Non-medication strategies for acute episodes include slow breathing techniques, grounding exercises, and physical activity.

Beyond formal treatment, several practices have shown benefit for reducing overall anxiety levels: regular high-intensity exercise, yoga, tai chi, and mindfulness-based stress reduction. These aren’t substitutes for therapy or medication in someone with panic disorder, but they can meaningfully lower the baseline level of nervous system arousal that makes panic attacks more likely to fire in the first place.
