Sleeping on your left side positions your stomach, heart, and other organs in ways that can improve digestion, reduce acid reflux, and potentially help your brain clear waste more efficiently. It’s the most commonly recommended sleep position during pregnancy and for people with heartburn, though it does come with some trade-offs for your shoulders and joints.
How It Affects Digestion and Acid Reflux
Your stomach sits slightly to the left of your midline, and its natural curve means that when you lie on your left side, gravity keeps stomach acid pooled away from the opening to your esophagus. The American Gastroenterological Association recommends the left-side position specifically because it reduces nighttime acid exposure. When you flip to your right side, your stomach’s contents sit closer to that opening, making it easier for acid to creep upward.
This matters most if you have GERD or frequent heartburn. People who experience acid reflux at night often find that left-side sleeping reduces the burning sensation and coughing that can disrupt sleep. The effect is purely gravitational and anatomical, so it works whether or not you’re also taking medication for reflux.
Gallbladder and Liver Pressure
Your liver and gallbladder both sit on the right side of your body. Sleeping on your left side takes direct pressure off these organs, allowing your gallbladder to contract and expand more freely. If you’re dealing with gallstones or gallbladder pain, sleeping on the right side can compress the gallbladder and make it harder for a stone to pass. Left-side sleeping gives the gallbladder more room and may reduce discomfort while a bile duct blockage clears.
Brain Waste Clearance
Your brain has its own waste-removal system that’s most active during sleep. A study published in the Journal of Neuroscience found that this system, which flushes out metabolic waste including proteins linked to Alzheimer’s disease, works most efficiently when the body is in a lateral (side) position compared to sleeping face-down or on the back. In rodent models, side sleeping showed superior clearance of amyloid-beta, one of the proteins that accumulates in Alzheimer’s disease.
This research was conducted in rats and mice, so it’s not yet confirmed in humans. But it’s notable that most mammals naturally sleep on their sides, which the researchers suggested may have evolved partly because of this cleaning advantage.
Sleep Apnea and Breathing
Side sleeping in general helps keep your airway open, which is why it’s often recommended for people with obstructive sleep apnea. When you sleep on your back, gravity pulls the tongue and soft tissues toward the back of your throat, partially blocking airflow. Rolling to either side reduces this effect. In clinical sleep studies, the difference can be dramatic. One case documented by the American Thoracic Society showed a patient whose breathing disruptions dropped significantly in the left lateral position compared to the right side, though results vary from person to person.
If you snore or have been told you stop breathing during sleep, switching to side sleeping is one of the simplest interventions to try before more involved treatments.
Pregnancy: Less Critical Than You’ve Heard
For decades, pregnant women have been told they must sleep on their left side to avoid compressing the inferior vena cava, the large vein that returns blood from your lower body to your heart. The logic is that in later pregnancy, the weight of the uterus can press on this vein when you lie flat on your back, reducing blood flow to the placenta.
The reality is more nuanced. A review in the Journal of Obstetrics and Gynaecology Canada found that while some women lying on their backs develop symptoms like lightheadedness, only 2% to 4% experience significant compression. Even in that small group, there was no evidence of harm to the fetus. The authors concluded that the standard advice to always sleep on the left side “is therefore not relevant” for most pregnant women. If lying on your back makes you feel dizzy or uncomfortable, your body is already telling you to shift, and either side is fine.
The Lymphatic System Connection
The thoracic duct, your body’s largest lymphatic vessel, runs up the left side of your body and empties into veins at the base of your neck on the left. Each day it delivers about 3 liters of lymph, a fluid that carries waste, immune cells, and dietary fats, back into your bloodstream. Some practitioners suggest that left-side sleeping supports this drainage by aligning with the duct’s natural path, though there’s no direct clinical evidence showing that sleep position meaningfully changes lymphatic flow rates in healthy people.
The Downside: Shoulder and Hip Pressure
The main drawback of left-side sleeping is sustained pressure on your left shoulder and hip. Your shoulder joint bears your upper body’s weight all night, which can compress tendons and the fluid-filled cushions (bursae) between bones. Over time, this can contribute to shoulder impingement, where structures in the joint get pinched and inflamed. If you already have a rotator cuff issue or bursitis in your left shoulder, sleeping on that side will likely make it worse.
Your hip faces the same problem. The bony point of your hip pressing into the mattress for hours can irritate the bursa on the outside of the joint, especially on a firm mattress. A pillow between your knees helps keep your hips aligned and reduces strain on both the hip and lower back. For shoulder relief, hugging a pillow in front of your chest prevents your top arm from pulling the shoulder forward into an awkward position.
If you rotate between left and right side sleeping throughout the night, you distribute the pressure more evenly. Most people shift positions naturally during sleep, so committing to one side all night long isn’t necessary unless you’re managing a specific condition like reflux or gallbladder pain.
