Stopping Lexapro can trigger a set of physical and emotional symptoms known as discontinuation syndrome, especially if you’ve taken it for six weeks or longer. About 31% of people experience at least one symptom after stopping an antidepressant, though when you account for placebo effects, roughly 15% of people (about one in six or seven) have symptoms specifically caused by the discontinuation itself. Most symptoms start within one to three days after your last dose and resolve within one to two weeks, but stopping abruptly tends to make things significantly worse than tapering gradually.
Common Discontinuation Symptoms
The symptoms of Lexapro withdrawal are a mix of physical and emotional. On the physical side, many people describe flu-like feelings: fatigue, headaches, body aches, and sweating. Nausea is common, sometimes with vomiting. Dizziness and lightheadedness are among the most frequently reported complaints. You may also notice vivid dreams or nightmares that feel unusually intense and real.
Emotionally, the picture can be confusing. Anxiety, irritability, agitation, and even aggression can surface during the withdrawal window. These mood shifts don’t necessarily mean your original condition is coming back. They’re often a direct result of your brain adjusting to the absence of the medication.
One of the more distinctive and unsettling symptoms is what patients call “brain zaps.” These feel like a brief electrical shock inside your head, lasting about one second each. Despite feeling like they originate deep in the brain, they actually occur on the surface, around the nerves lining the brain. They’re often accompanied by a strange sensation when moving your eyes side to side, sometimes described as a faint “whoosh” sound. Some people say it feels like their brain briefly pauses and reboots, like a computer restarting. Brain zaps are more common when blood levels of the medication drop quickly, which is why abrupt stopping tends to produce them more than gradual tapering.
Why Your Brain Reacts This Way
Lexapro works by increasing serotonin availability in the brain. When you take it for weeks or months, your brain adapts. It dials down certain serotonin receptors and transporters to compensate for the extra serotonin the drug provides. This recalibration is why the medication takes several weeks to start working in the first place, and it’s also why stopping creates problems.
When you remove the drug, serotonin levels drop, but your brain’s receptors and transporters are still in their adjusted state. It takes time, sometimes several weeks, for them to return to their pre-medication configuration. During that gap, your nervous system is essentially running with mismatched settings. The disruption doesn’t just affect serotonin. Stopping SSRIs can also temporarily alter signaling in other brain systems, including those involved in stress hormones and excitatory neurotransmission. This cascade of imbalances is what produces the wide range of symptoms people experience.
This also explains why people who’ve only taken Lexapro for a week or two rarely experience discontinuation symptoms. The brain hasn’t had enough time to significantly reorganize its receptor landscape.
The Typical Timeline
Symptoms most commonly appear within one to three days of your last dose, though in rare cases they can start within hours or take more than a week to show up. For most people, the worst of it passes within one to two weeks. A small number of people experience symptoms that linger longer.
Lexapro carries a moderate risk of discontinuation syndrome compared to other antidepressants. It’s not in the highest-risk category (that distinction goes to drugs like venlafaxine and paroxetine), but a large meta-analysis covering over 21,000 patients found that escitalopram was associated with higher-than-average frequencies of discontinuation symptoms among SSRIs. About one in 35 patients will experience symptoms severe enough to significantly disrupt daily life.
Withdrawal Symptoms vs. Returning Depression
One of the hardest parts of stopping Lexapro is figuring out whether what you’re feeling is withdrawal or your depression or anxiety coming back. The two can look similar on the surface, but there are key differences.
Timing is the biggest clue. Discontinuation symptoms show up within days of stopping the medication. A relapse of depression develops more gradually, typically emerging weeks later. Withdrawal also tends to include physical symptoms that depression doesn’t usually cause, like dizziness, brain zaps, flu-like feelings, and tingling or burning sensations.
There’s a quick test that can help clarify things: if you take a dose of the antidepressant and symptoms improve within a day or two, that strongly suggests discontinuation syndrome. Treating a true depressive relapse with medication takes weeks to show results, not hours. If your symptoms last longer than a month and are getting worse rather than better, that points more toward a return of the underlying condition.
How Tapering Reduces Symptoms
The FDA’s prescribing information for escitalopram is clear: gradually reduce the dosage rather than stopping abruptly whenever possible. The goal of tapering is to give your brain time to readjust its receptor balance incrementally instead of all at once.
Traditional tapering typically involves stepping down through available tablet sizes over several weeks. But a newer approach called hyperbolic tapering is gaining attention for being more aligned with how the drug actually affects the brain. Because the relationship between dose and brain effect isn’t linear (cutting your dose from 10mg to 5mg has a much bigger neurological impact than cutting from 5mg to 2.5mg), hyperbolic tapering uses progressively smaller reductions.
In one documented case, a patient on 10mg of escitalopram reduced their dose weekly through steps of 5mg, 3mg, 1.5mg, 1mg, 0.5mg, and 0.25mg before stopping entirely. Each step represented roughly a 10% reduction in the drug’s effect on serotonin transporters. The result was minimal to mild withdrawal, which the patient described as dramatically better than previous attempts at stopping. Three months after the final dose, they remained symptom-free, the first time in five years they’d successfully discontinued an antidepressant.
Getting to very small doses like 0.25mg or 0.5mg often requires a liquid formulation or a compounding pharmacy, since standard tablets don’t come in those sizes. This is worth discussing if you’ve had difficulty stopping in the past.
Factors That Affect Your Risk
Several things influence how likely you are to experience discontinuation symptoms and how severe they’ll be. The longer you’ve been on Lexapro, the more time your brain has had to adapt, and the bigger the adjustment when you stop. Higher doses generally carry more risk than lower ones for the same reason. Stopping abruptly rather than tapering is the single most controllable risk factor.
Individual biology matters too. Some people stop with barely a blip; others have a rough few weeks even with careful tapering. If you’ve experienced discontinuation symptoms from antidepressants before, you’re more likely to experience them again.
