Because Orgovyx is a GnRH antagonist with a relatively short half-life of around 60 hours, the drug clears your system faster than older injectable alternatives, and the hormonal recovery that follows is correspondingly quicker.
That recovery process brings real, noticeable changes to how you feel, but it also requires careful monitoring to make sure your cancer stays under control. Here’s what to expect.

Orgovyx has a mean half-life of about 61.5 hours, meaning roughly half the drug is eliminated every two and a half days. Within a week or so of your last pill, the active drug is largely cleared. This is a meaningful difference from injectable therapies like leuprolide, which use slow-release depot formulations that can linger for weeks or months after the final injection.
Because Orgovyx works by directly blocking the hormone signal that tells your body to produce testosterone, once the drug is gone, that signal resumes almost immediately. There’s no “testosterone flare” when you stop. Unlike older GnRH agonists, which can cause a temporary surge in testosterone when started, GnRH antagonists like Orgovyx allow a smooth, gradual rise back toward your normal levels.
The Testosterone Recovery Timeline
In the HERO phase 3 trial, which compared Orgovyx to leuprolide in men with advanced prostate cancer, the median time to testosterone recovery after stopping Orgovyx was 86 days. For comparison, only 3% of men who stopped leuprolide injections hit the same mark at 90 days, with a median recovery time of 112 days. That gap matters if you’re doing intermittent therapy or if quality of life during the off-treatment period is a priority for you. The faster your testosterone comes back, the sooner the side effects of hormone suppression start to fade.
Some men recover faster and some slower. Your age, how long you were on treatment, and your baseline testosterone levels before starting all play a role. Men who were on Orgovyx for shorter durations generally recover more quickly.
What Recovery Feels Like
The side effects of Orgovyx are really the side effects of having very low testosterone: hot flashes, fatigue, reduced sex drive, loss of muscle mass, mood changes, and sometimes cognitive fog. As your testosterone climbs back toward normal after stopping the drug, these effects gradually reverse.
The timeline for symptom relief isn’t identical to the testosterone recovery timeline. Some changes, like energy and mood improvements, may begin within weeks as testosterone starts rising, even before it reaches normal levels. Others take longer. Muscle mass and bone density, for example, can take months to rebuild. Hot flashes typically decrease in frequency and intensity as hormone levels stabilize, though exactly when varies from person to person.
Libido and sexual function often return as testosterone recovers, but this can be one of the slower changes, particularly in older men or those who were on treatment for a year or more. The key point is that recovery is a gradual process, not a switch that flips the day you stop taking the pill.
PSA Monitoring After Stopping
When testosterone rises, so can PSA, because prostate cancer cells (if any remain) become active again in the presence of testosterone. This is expected and is the main reason your medical team will want to watch your PSA closely after you stop.
Monthly PSA testing is a common recommendation during the off-treatment period. The goal is to catch any significant rise early. In intermittent androgen deprivation therapy protocols using Orgovyx, treatment is typically paused when PSA drops to 4 ng/mL or below and restarted if it climbs back to 10 ng/mL or above. Your specific thresholds may differ depending on your cancer stage and your doctor’s judgment, but those numbers give you a general sense of the framework.
A rising PSA after stopping doesn’t automatically mean the cancer is progressing in a dangerous way. Some rise is expected simply because testosterone is returning. What matters is the pattern: how fast PSA rises, how high it goes, and whether it stabilizes. This is why consistent, regular testing is so important during this window.
Intermittent Therapy: Planned Breaks
Many men don’t stop Orgovyx permanently. Instead, they cycle on and off the drug in a strategy called intermittent androgen deprivation therapy. The idea is to give your body a break from the side effects of testosterone suppression while still keeping cancer in check.
Orgovyx is particularly well suited for this approach. Its oral formulation means you simply stop taking pills rather than waiting for an injection to wear off, and its faster testosterone recovery means you spend less of your off-treatment time still feeling the effects of suppression. During off-treatment periods, you regain quality of life while your medical team monitors PSA to decide when to restart.
Not everyone is a candidate for intermittent therapy. It’s generally used in men with biochemical recurrence or certain stages of advanced disease rather than in the most aggressive cases. Your oncologist will help determine whether cycling on and off is appropriate for your situation.
No Testosterone Flare Risk
One concern men sometimes have is whether stopping Orgovyx could cause a dangerous spike in testosterone that feeds their cancer. This doesn’t happen. GnRH antagonists like Orgovyx block hormone receptors directly, so when you stop, testosterone simply resumes its normal production pathway. There’s no overshoot or flare effect.
This is different from what can happen when you start (not stop) older GnRH agonist injections, which initially overstimulate the hormonal pathway before suppressing it. That flare risk is specific to treatment initiation with agonists and isn’t a concern with Orgovyx at any point, whether starting or stopping.
What to Watch For
During the recovery period, pay attention to how your body responds. Most changes will be positive: more energy, fewer hot flashes, improved mood and mental clarity. But if you notice new bone pain, urinary symptoms, or unexplained weight loss, these warrant prompt discussion with your care team regardless of your PSA numbers.
The recovery period is also a time when cardiovascular risk deserves attention. Prolonged testosterone suppression can affect cholesterol levels, blood sugar, and heart health. As your hormones normalize, these metabolic markers typically improve, but staying active and maintaining a healthy diet during recovery supports that process. If you were on Orgovyx for a year or longer, a check of your metabolic health markers during the months after stopping is reasonable.
