When your blood sugar is high, your body starts sending signals that something is off. You might feel unusually thirsty, need to urinate frequently, and notice your energy dropping. A normal fasting blood sugar falls between 70 and 99 mg/dL. Once it climbs above 126 mg/dL on repeated tests, that typically indicates diabetes. But even levels in the 100 to 125 mg/dL range (prediabetes) mean your body is already struggling to manage glucose properly.
What happens next depends on how high your sugar goes, how long it stays elevated, and whether the spike is a one-time event or a chronic pattern. Here’s what’s actually going on inside your body at each stage.
The First Signs You’ll Notice
The earliest symptoms of high blood sugar tend to be frequent urination, increased thirst, blurred vision, and feeling weak or unusually tired. These four signs are connected. When glucose builds up in your bloodstream faster than your cells can use it, the excess spills into your urine. Glucose molecules pull water along with them, a process called osmotic diuresis, which is why you’re suddenly running to the bathroom. That fluid loss triggers thirst as your body tries to replace what it’s losing.
The fatigue comes from a cruel irony: your blood is flooded with fuel your cells can’t access efficiently. Without enough insulin to shuttle glucose inside, your cells are essentially starving while surrounded by energy. Your muscles and brain don’t get what they need, and you feel drained.
Why Your Vision Gets Blurry
Blurred vision during a blood sugar spike isn’t permanent damage. It’s a temporary shift in how your eye’s lens bends light. High glucose changes the osmotic pressure inside the lens, causing it to swell slightly. That swelling alters its shape and thickness, which throws your focus off. You might suddenly need to hold your phone farther away or find that your glasses don’t seem right.
The good news is that this reverses once blood sugar comes back down, though it’s not instant. In one documented case, it took about two months of improved blood sugar control for a patient’s vision to fully return to baseline. If you’re newly starting treatment for diabetes and your vision seems worse before it gets better, this is likely why.
Your Immune System Takes a Hit
High blood sugar quietly weakens your body’s defenses. Elevated glucose interferes with neutrophils, the white blood cells that serve as your immune system’s first responders. Specifically, it inhibits their ability to migrate toward infections, engulf bacteria, and kill pathogens. High glucose also disrupts the chemical signals that coordinate your inflammatory response, making the initial reaction to an invader slower and less effective.
On top of that, excess sugar in the bloodstream physically alters the shape of proteins your immune system uses to tag bacteria for destruction. This makes it harder for your body to recognize and neutralize infections. The practical result: people with poorly controlled blood sugar get more frequent infections, including urinary tract infections, skin infections, and fungal infections like yeast. Better blood sugar control has been shown to reduce infection rates in both surgical and critically ill patients.
What Happens to Your Brain
Repeated episodes of high blood sugar stress the brain over time, though the effects aren’t obvious right away. Chronically elevated glucose damages the small blood vessels that deliver oxygen-rich blood to brain tissue. When parts of the brain receive too little blood, cells can die, leading to problems with memory and thinking. Over time, this vascular damage can progress to vascular dementia.
Diabetes also raises the risk of stroke, memory loss, mood shifts, and eventually Alzheimer’s disease. In the short term, many people with high blood sugar report difficulty concentrating and mental fogginess, though these symptoms can be subtle enough to dismiss as stress or poor sleep.
When High Sugar Becomes an Emergency
If blood sugar keeps climbing without treatment, two dangerous conditions can develop.
Diabetic ketoacidosis (DKA) occurs when the body, unable to use glucose for fuel, starts breaking down fat at an accelerated rate. This produces toxic acids called ketones. DKA is diagnosed when blood sugar exceeds 250 mg/dL alongside acidic blood and the presence of ketones. Warning signs include fruity-smelling breath, nausea and vomiting, abdominal pain, shortness of breath, confusion, and eventually loss of consciousness. DKA is most common in people with type 1 diabetes but can happen in type 2 as well.
Hyperosmolar hyperglycemic state (HHS) is more common in type 2 diabetes and involves blood sugar climbing above 600 mg/dL. At these levels, the blood becomes dangerously thick and concentrated. The massive fluid loss from all that glucose-driven urination leads to severe dehydration and can cause seizures, coma, or death. HHS typically develops over days or weeks, often in older adults who may not recognize how dehydrated they’ve become.
Both conditions also strip the body of sodium and potassium through urine, creating dangerous electrolyte imbalances that can affect heart rhythm and muscle function.
Long-Term Damage From Chronic High Sugar
When blood sugar stays elevated for months or years, the damage shifts from uncomfortable symptoms to permanent complications. The core problem is vascular: excess glucose injures blood vessel walls through several overlapping mechanisms, including the buildup of harmful sugar-protein compounds, increased inflammation, and the formation of tiny blood clots. Both the smallest and largest blood vessels in the body are affected.
Small Blood Vessel Damage
The three hallmark complications of chronic high blood sugar all stem from damage to tiny blood vessels:
- Eye damage (retinopathy): Tiny bulges form in the blood vessels of the retina. Over time, abnormal new blood vessels can grow, leading to bleeding, retinal detachment, and partial or total vision loss. This is one of the leading causes of blindness in adults. There are no early symptoms, which is why regular eye exams matter.
- Kidney damage (nephropathy): The filtering units of the kidneys rely on microscopic blood vessels. Years of high sugar gradually destroys them, eventually leading to kidney failure that requires dialysis or transplant.
- Nerve damage (neuropathy): High glucose injures nerve fibers directly and starves them of blood supply. The most common form starts in the feet and hands, causing tingling, numbness, pain, or a total loss of sensation. Losing feeling in your feet is particularly dangerous because small cuts and blisters go unnoticed, leading to infections and ulcers that can become severe enough to require amputation.
Large Blood Vessel Damage
Chronically high blood sugar accelerates atherosclerosis, the buildup of fatty deposits in larger arteries. This raises the risk of heart attack and stroke significantly. The combination of high blood sugar with the high blood pressure and abnormal cholesterol levels that often accompany diabetes makes this damage progress faster than it would from any one of those factors alone.
High Blood Sugar Without Diabetes
You don’t need a diabetes diagnosis to experience high blood sugar. During serious illness, injury, surgery, or infection, your body mounts a stress response that floods the bloodstream with hormones like cortisol and adrenaline. These hormones ramp up glucose production in the liver while simultaneously making your cells more resistant to insulin. The result is a temporary spike in blood sugar called stress-induced hyperglycemia.
Certain medications can do the same thing. Corticosteroids (commonly prescribed for asthma, autoimmune conditions, and inflammation) are a well-known cause of elevated blood sugar, sometimes pushing levels high enough to require treatment even in people who’ve never had glucose problems before. Once the illness resolves or the medication is stopped, blood sugar typically returns to normal, but the spike itself can affect recovery, particularly in hospitalized patients where even temporary hyperglycemia is associated with worse outcomes.
