When your gums recede, the tissue that normally hugs the base of your teeth pulls back or wears away, exposing parts of the tooth root that are supposed to stay hidden. This is extremely common: about 50% of adults under 65 have at least one site of recession, and that number jumps to 88% for people 65 and older. What starts as a subtle cosmetic change can quietly create real problems, from sharp tooth sensitivity to a higher risk of cavities in places you’ve never had them before.
What You’ll Notice First
The earliest sign is often visual. Your teeth start to look longer than they used to because more of the tooth structure is visible above the gum line. You might also notice that your gums look uneven, sitting higher on some teeth than others. In some cases, you can feel a small notch or ledge where the gum meets the tooth.
As recession progresses, dark triangular gaps can appear between your teeth near the gum line. These “black triangles” form because the pointed tips of gum tissue (called papillae) that normally fill the spaces between teeth shrink or disappear. The triangles tend to be most noticeable between the front teeth, especially if your teeth are naturally narrower at the base than at the biting edge. Beyond appearance, these gaps trap food and can make flossing feel different than it used to.
Why Receded Gums Cause Sensitivity
The crown of your tooth, the part you can normally see, is covered in enamel. But below the gum line, the root is covered only by a very thin layer of cementum and then dentin, a softer tissue riddled with microscopic tubes. When gums recede and that root surface becomes exposed, those tiny tubes create a direct communication line to the nerve inside your tooth.
Hot coffee, cold water, sweet foods, or even a burst of cold air can cause fluid inside those tubes to shift. That fluid movement triggers nerve fibers at the base of each tube, producing a sharp, sudden jolt of pain. This is why sensitivity from recession feels different from a regular cavity. It’s fast and intense, hitting the moment a trigger touches the exposed area and fading quickly once the stimulus is gone. For some people it’s mildly annoying; for others it makes eating and drinking genuinely uncomfortable.
Higher Risk of Root Cavities
Exposed roots don’t just hurt. They decay more easily than the rest of your tooth. Enamel is the hardest substance in the human body, heavily mineralized and built to resist acid. Root surfaces are a different story. Dentin and cementum contain far more organic material and far less mineral than enamel, which means acid can break them down at a higher pH, one that wouldn’t even threaten enamel. The decay process on roots also involves the breakdown of collagen, a protein fiber that enamel doesn’t contain, so the damage can spread faster once it starts.
Root cavities are particularly tricky because they often form right at or just below the gum line, making them harder to spot during routine brushing and sometimes harder for a dentist to treat. People with significant recession need to be especially attentive to cleaning those newly exposed surfaces.
What Happens if It Keeps Getting Worse
Recession that continues unchecked eventually becomes a structural problem, not just a surface one. Your teeth are anchored in bone and held in place by a web of connective tissue fibers called the periodontal ligament. As gum disease drives recession deeper, it destroys both the ligament attachment and the underlying bone. The less bone and connective tissue supporting a tooth, the more that tooth moves.
Research consistently shows a direct relationship between the severity of this tissue destruction and tooth mobility. As the root loses surface area where connective tissue attaches, the tooth becomes progressively looser. Early mobility might feel like a slight wiggle you notice with your tongue. Advanced mobility can make biting into food unreliable and, in the worst cases, teeth can be lost entirely. This progression typically takes years, not weeks, but once bone is lost around a tooth, it doesn’t come back on its own.
Why Gums Don’t Grow Back
One of the most frustrating realities of gum recession is that your body is very poor at reversing it. Unlike a cut on your skin, which heals and regenerates relatively well, the periodontal complex involves multiple tissue types (gum, ligament, cementum, bone) that need to work together in a precise architecture. Under most circumstances, the regeneration ability of diseased periodontal tissue is weak, and the lost structures can’t repair themselves.
Even professional deep cleanings, which remove bacteria and diseased tissue to halt the progression, only result in a small amount of tissue regeneration at treated sites. Aging and systemic health conditions further reduce whatever limited regenerative capacity exists. This is why prevention matters so much: once the gum tissue is gone, you’re looking at surgical options rather than natural healing.
Common Causes
Gum disease is the leading driver of recession. Bacterial buildup along and beneath the gum line triggers chronic inflammation, which gradually destroys the attachment between gum and tooth. But it’s not the only cause.
Brushing too hard is a surprisingly common contributor. Aggressive technique, excessive pressure, stiff bristles, or abrasive toothpastes can physically wear away gum tissue over time. People often do the most damage on the side opposite their dominant hand, where they tend to press harder to compensate for less dexterity. Frequent, forceful flossing can also injure the tissue between teeth.
Other factors include teeth grinding (which places chronic lateral force on the gums and bone), tobacco use, hormonal changes, misaligned teeth that concentrate bite forces unevenly, and genetic predisposition. Some people simply have thinner gum tissue that’s more vulnerable to recession regardless of how well they care for it.
Treatment Options
For mild recession, the priority is stopping further loss. Switching to a soft-bristled toothbrush, using gentler technique, treating any underlying gum disease, and addressing grinding with a night guard can stabilize things. Desensitizing toothpastes or professional fluoride treatments can help manage sensitivity on exposed roots.
When recession is more significant, surgical repair becomes an option. The traditional approach is connective tissue grafting, where a small piece of tissue is taken from the roof of your mouth (or from a donor source) and stitched over the exposed root to rebuild the gum line. Recovery takes about two weeks, during which eating and brushing around the area are restricted.
A newer alternative is the pinhole surgical technique, which is less invasive. Instead of cutting and stitching a graft, the dentist makes a tiny hole in the gum tissue above the recession site and gently repositions the existing tissue downward to cover the exposed root. There’s no tissue harvesting from the palate, which typically means less post-operative discomfort. Both approaches have good track records for restoring gum coverage, though the best option depends on how severe the recession is, how many teeth are affected, and the thickness of your remaining tissue.
Protecting Your Gum Line
Prevention comes down to two things: keeping inflammation low and avoiding mechanical damage. Brush twice a day with a soft-bristled brush using light, circular motions rather than a hard back-and-forth scrub. If you’re unsure how much pressure is too much, many electric toothbrushes now have pressure sensors that alert you when you’re pushing too hard. Floss daily but gently, guiding the floss rather than snapping it into the gums.
Regular dental cleanings catch early signs of recession before they become advanced. Recession that’s identified at 1 to 2 millimeters is far easier and less expensive to manage than recession that’s reached 4 or 5 millimeters with bone involvement. If you notice your teeth looking longer, new gaps forming between teeth, or a sharp zing when you drink cold water, those are worth mentioning at your next visit rather than waiting to see if they get worse.
