In medical terms, HCC stands for hepatocellular carcinoma, the most common type of liver cancer. It begins in hepatocytes, the main working cells of the liver. HCC accounts for the majority of primary liver cancers and develops most often in people who already have chronic liver disease. There is also a second, unrelated use of the abbreviation in medical billing, where HCC stands for Hierarchical Condition Category, a coding system used to predict healthcare costs.
HCC as Liver Cancer
Hepatocellular carcinoma starts when liver cells begin growing out of control. Unlike cancers that spread to the liver from somewhere else (called secondary or metastatic liver cancers), HCC originates in the liver itself. It is closely tied to underlying liver damage: roughly 80% to 90% of people diagnosed with HCC already have cirrhosis, the advanced scarring that results from years of liver injury.
Major Risk Factors
Chronic infection with hepatitis B or hepatitis C is the single biggest driver of HCC worldwide. In people who already have cirrhosis from hepatitis C, the five-year cumulative risk of developing HCC reaches about 17% in Western countries and as high as 30% in Japan. For hepatitis B-related cirrhosis, the five-year risk is around 10% in the West and 15% in high-endemic regions.
Heavy alcohol use also raises the risk substantially. Among people with alcoholic cirrhosis who don’t carry hepatitis B or C, the five-year cumulative risk is about 8%. Carrying both a viral hepatitis infection and a heavy drinking habit multiplies the danger: alcohol abuse on top of viral cirrhosis increases HCC risk two- to fourfold compared with abstinence, and being co-infected with both hepatitis B and C raises it two- to sixfold.
Other independent predictors include older age, male sex, and how severely the liver is already damaged at the time of diagnosis. Hereditary hemochromatosis, a genetic condition that causes iron overload, carries a notably high five-year HCC incidence of about 21% in people with cirrhosis.
Symptoms to Recognize
Early-stage HCC often causes no symptoms at all, which is one reason it can be difficult to catch. As the tumor grows, common signs include a feeling of fullness or a hard lump under the ribs on the right side (from an enlarged liver), yellowing of the skin and eyes (jaundice), a swollen abdomen that feels like it’s filling with fluid, loss of appetite or feeling full after very small meals, and unexplained weight loss. Some people also notice fullness under the left ribs, which can signal an enlarged spleen.
Because these symptoms overlap with many other liver conditions, they don’t point to cancer on their own. People with known cirrhosis or chronic hepatitis are typically monitored with regular screening so that HCC can be caught before symptoms appear.
How HCC Is Diagnosed
Doctors use a combination of imaging and blood work to identify HCC. Ultrasound is usually the first screening tool, followed by CT or MRI scans that can reveal the characteristic blood-flow pattern of a liver tumor. A blood test measuring alpha-fetoprotein (AFP), a protein produced by liver cells, serves as a tumor marker. Most people with HCC have significantly elevated AFP levels.
Not everyone needs a biopsy. When imaging and blood tests together paint a clear picture, doctors can make the diagnosis without taking a tissue sample. When the picture is less certain, a needle biopsy through the skin can confirm whether cells are cancerous.
Staging and Treatment Options
HCC is commonly staged using the Barcelona Clinic Liver Cancer (BCLC) system, which groups patients into five stages based on tumor size, liver function, and overall health. Treatment decisions flow directly from this staging.
For early-stage disease, the goal is often a cure. Surgical removal of the tumor is preferred when the liver is functioning well enough and there is no significant portal hypertension (high blood pressure in the liver’s main vein). For very small tumors (3 cm or less), a heat-based destruction technique called ablation can offer survival rates competitive with surgery. Liver transplantation is another potentially curative option for patients who meet specific criteria, because it removes both the cancer and the diseased liver.
When the cancer is more advanced or surgery isn’t feasible, treatment shifts to systemic therapy. First-line options now include combinations of immunotherapy and targeted drugs that block blood vessel growth to the tumor. If the cancer progresses on first-line treatment, patients may move to oral medications that target the blood supply feeding the tumor. The specific path depends on how the liver is functioning, whether the cancer has invaded blood vessels or spread beyond the liver, and the patient’s general condition.
Survival by Stage
Prognosis varies enormously depending on how far the cancer has spread at diagnosis. Based on data from the American Cancer Society covering patients diagnosed between 2014 and 2020:
- Localized (cancer confined to the liver): 37% five-year relative survival rate
- Regional (spread to nearby structures or lymph nodes): 13%
- Distant (spread to lungs, bones, or other distant sites): 3%
Across all stages combined, the five-year survival rate is 22%. These numbers underscore why screening in high-risk groups matters so much. Catching HCC while it’s still localized dramatically improves the odds.
The Other HCC: Hierarchical Condition Categories
Outside of oncology, you may encounter “HCC” in the context of medical billing and insurance. Here it stands for Hierarchical Condition Category, a classification system the Centers for Medicare and Medicaid Services (CMS) uses to estimate how much it will cost to care for a given patient.
Every patient receives a Risk Adjustment Factor (RAF) score calculated from their age, sex, and documented medical conditions. Each qualifying condition is assigned an HCC code, and the more serious or numerous the conditions, the higher the RAF score. Insurance plans that enroll sicker, higher-cost patients receive larger payments, while plans with healthier enrollees receive less. The system is designed to prevent insurers from being penalized for covering people with chronic or complex illnesses.
Not every diagnosis qualifies for an HCC code. Conditions that are vague, minor, or subject to inconsistent documentation are excluded. The codes that do count tend to represent well-defined, costly conditions like diabetes, heart failure, and chronic kidney disease. For patients, HCC coding is invisible. It operates entirely behind the scenes in how insurers and government programs calculate reimbursement.
