A resting heart rate below 60 beats per minute is the standard medical threshold for bradycardia, the clinical term for a slow heart rate. But that number alone doesn’t tell the whole story. For some people, a heart rate in the 40s is perfectly healthy. For others, a rate of 55 can cause dizziness and fatigue. What matters most is whether your body is getting enough blood flow at that rate.
The 60 BPM Threshold
A normal adult resting heart rate falls between 60 and 100 beats per minute. Anything below 60 technically qualifies as bradycardia. That said, this number is a guideline, not a hard cutoff. Plenty of healthy people sit comfortably in the mid-to-upper 50s without any issues. The concern starts when a slow heart rate means your heart isn’t pumping enough oxygen-rich blood to your brain and organs.
Children and infants have naturally faster heart rates. A newborn’s resting rate ranges from 100 to 205 bpm, and a toddler’s from 98 to 140 bpm. School-age children (5 to 12) typically fall between 75 and 118 bpm. By adolescence, the adult range of 60 to 100 kicks in. So a heart rate of 70 that would be unremarkable in an adult could signal a problem in an infant.
When a Low Heart Rate Is Normal
Highly trained endurance athletes often have resting heart rates around 40 bpm. Their hearts are efficient enough to pump the same volume of blood with fewer beats. A lower resting heart rate in this context simply means the heart doesn’t have to work as hard. If you’re physically fit and feel fine, a rate in the 40s or 50s is generally nothing to worry about.
Sleep also brings your heart rate down significantly. Your sleeping heart rate typically runs 20% to 30% lower than your daytime resting rate, putting most healthy adults somewhere between 50 and 75 bpm overnight. Well-trained athletes can dip into the 30s during sleep without any concern, as long as they feel well during the day. Rates in the 20s during sleep, however, are worth discussing with a doctor to make sure the reading is accurate and nothing else is going on.
Symptoms That Signal a Problem
A slow heart rate becomes a medical issue when it causes symptoms. The key ones to watch for include:
- Dizziness or lightheadedness, especially when standing up
- Fainting or near-fainting spells
- Unusual fatigue that doesn’t match your activity level
- Shortness of breath with mild exertion
- Confusion or difficulty concentrating
- Chest discomfort
If you’re experiencing lightheadedness, fainting, or your symptoms last more than a minute or two, or keep coming back, that’s reason to get to an emergency room. Fainting is especially important to take seriously because it means your brain isn’t receiving adequate blood flow.
Common Causes of a Slow Heart Rate
Beyond fitness, several medical conditions and external factors can push your heart rate too low.
Thyroid problems are one of the more common culprits. An underactive thyroid slows your metabolism and, with it, your heart rate. This is something doctors routinely check with a simple blood test when evaluating bradycardia.
Sleep apnea, a condition where breathing repeatedly stops and starts during sleep, can also alter your heart rhythm. If you snore heavily, wake up feeling unrested, or a sleep tracker shows unusual heart rate patterns overnight, sleep apnea may be contributing.
Certain infections and imbalances in blood chemistry, particularly potassium levels, can interfere with the electrical signals that keep your heart beating at a steady pace.
Medications That Lower Heart Rate
A number of common medications slow the heart as either their intended effect or a side effect. Blood pressure drugs are the most frequent cause. Beta-blockers, one of the most widely prescribed classes of heart and blood pressure medication, slow the heart by reducing the effect of adrenaline. Calcium channel blockers like diltiazem and verapamil do the same through a different mechanism. Even beta-blocker eye drops used for glaucoma can lower heart rate enough to matter.
Other medications linked to slow heart rates include certain antidepressants (particularly some SSRIs), digoxin (used for heart failure), clonidine (a blood pressure drug that acts on the brain’s signaling), and donepezil (prescribed for Alzheimer’s disease, which can cause bradycardia in up to 48% of users in some studies, according to an American Heart Association scientific statement). If you’ve recently started a new medication and notice your heart rate dropping or new symptoms like fatigue and dizziness, the medication is a likely suspect.
How a Slow Heart Rate Is Evaluated
The primary tool is an electrocardiogram (ECG), which records your heart’s electrical activity through sensors placed on your chest. It’s quick, painless, and gives an immediate snapshot of your heart rhythm. The challenge is that bradycardia can come and go, so a normal ECG in the office doesn’t always catch the problem.
For intermittent symptoms, your doctor may have you wear a portable heart monitor. A Holter monitor records your heart continuously for a day or more during normal activities. An event recorder works differently: you wear it for up to 30 days and press a button when you feel symptoms, and it captures your heart rhythm at that moment. Both are designed to catch irregular patterns that a single office visit would miss.
A tilt table test is sometimes used if you’ve been fainting. You lie flat on a table while your heart rate and blood pressure are monitored, then the table tilts you upright. This reveals how your heart and nervous system respond to position changes, which can unmask certain causes of bradycardia. Blood tests to check thyroid function and potassium levels round out the standard workup, and a sleep study may be recommended if sleep apnea is suspected.
What “Too Low” Really Means
There’s no single number that’s universally dangerous. A heart rate of 45 in a 25-year-old marathon runner who feels great is normal. A heart rate of 55 in a 70-year-old who keeps nearly fainting is a problem. The combination of your heart rate, your symptoms, and your overall health determines whether your rate is too low for you.
As a general framework: if you’re not an athlete and your resting heart rate regularly sits below 60, it’s worth paying attention. If it drops below 50 and you’re feeling any of the symptoms listed above, that warrants a medical evaluation. And if your sleeping heart rate is consistently below 40, or you’re seeing readings in the 20s on a wearable device, that conversation with your doctor becomes more urgent.
