Rest, gentle compression, and early movement within your pain tolerance are the most effective combination for healing a pulled muscle. Most mild strains recover within a week, while moderate tears can take several weeks to a few months. What you do in the first 48 hours matters, but so does how you manage the days and weeks that follow.
What Happens Inside a Pulled Muscle
A pulled muscle, or muscle strain, means some or all of the fibers in the muscle have been stretched beyond their limit or torn. Doctors grade strains on a three-point scale. A Grade 1 strain involves only a few torn fibers. The muscle is sore and tender, but you can still use it at full strength. A Grade 2 strain means a larger number of fibers are damaged, producing noticeable swelling, bruising, and a clear drop in strength.
A Grade 3 strain is a complete tear. The muscle rips into two pieces or shears off its tendon, sometimes with an audible pop. This causes severe pain, significant discoloration, and total loss of function in that muscle. Grade 3 tears sometimes require surgery.
The First 48 Hours
The initial phase of a muscle strain is all about limiting further damage and letting the body’s inflammatory process get to work. Sports medicine has moved beyond the old “RICE” advice (rest, ice, compression, elevation). A framework published in the British Journal of Sports Medicine called PEACE and LOVE now guides treatment across both the acute and recovery phases.
In the first one to three days, the priorities are:
- Protect the muscle. Reduce movement and unload the injured area to minimize bleeding inside the tissue and prevent the torn fibers from separating further. Use pain as your guide for how much to restrict activity, but don’t immobilize completely. Prolonged rest weakens the healing tissue.
- Elevate. Keep the injured limb above your heart when you can. This helps fluid drain away from the injury site and reduces swelling.
- Compress. A bandage or compression wrap limits swelling and internal bleeding. Even light, consistent pressure helps.
One counterintuitive recommendation: avoid anti-inflammatory medications in these first couple of days if you can manage the pain without them. Inflammation is the body’s repair mechanism. The swelling and heat you feel are signs that your body is sending repair cells to the injury. Suppressing that process early on, especially at higher doses, may slow long-term healing. If the pain is truly unmanageable, over-the-counter options like ibuprofen (up to 400 mg per dose) or naproxen can help, but reassess after five days of use.
Heat should also be avoided during the first 48 hours. It increases blood flow and can worsen swelling in freshly damaged tissue. Cold packs are fine for short-term pain relief during this window, applied for 15 to 20 minutes at a time with a cloth barrier to protect your skin.
After the First Few Days
Once the acute pain settles, the goal shifts from protection to controlled movement. This is the “LOVE” half of the framework: load, optimism, vascularization, and exercise.
Start adding gentle mechanical stress back to the muscle as soon as your pain allows. This doesn’t mean jumping back into your normal workout. It means light, pain-free movement that encourages the healing fibers to align properly and rebuild strength. Walking, gentle stretching, or easy range-of-motion exercises are good starting points depending on which muscle you injured.
Pain-free aerobic activity, like a short walk or easy cycling, is especially valuable in the days after a strain. It boosts blood flow to the injured area, delivers nutrients the tissue needs to rebuild, and helps with mood and motivation during recovery. The key qualifier is “pain-free.” If an activity causes sharp or worsening pain, scale it back.
This is also when heat becomes useful. After 48 hours, warm compresses or a heating pad can relax the surrounding muscles, reduce stiffness, and make it easier to move through your recovery exercises.
Massage and Hands-On Therapy
Massage can be helpful, but timing matters. Direct pressure on a freshly torn muscle can worsen internal bleeding and increase damage. Wait at least 48 hours, and only begin massage once the acute pain has tapered to mild discomfort. Start with light pressure around (not directly on) the most tender spot, and gradually increase as tolerance improves over the following days.
Passive treatments like ultrasound therapy, acupuncture, or electrical stimulation have minimal impact on pain and function compared to simply moving and exercising. An active approach to recovery consistently outperforms passive modalities, and relying too heavily on hands-off treatments can actually slow your progress.
Nutrition for Muscle Repair
Your body needs raw materials to rebuild torn fibers, and protein is the primary one. Most people eating a balanced diet get enough, but it’s worth paying attention during recovery. General recommendations suggest 15 to 30 grams of protein at each meal. If you exercise regularly, aim for 1.1 to 1.5 grams of protein per kilogram of body weight daily. For someone weighing 70 kg (about 154 pounds), that’s roughly 77 to 105 grams spread across the day.
Staying well-hydrated and eating enough overall calories also matters. Your body can’t repair tissue efficiently if it’s running a significant caloric deficit. You don’t need supplements or special recovery foods. Consistent meals with adequate protein from sources like eggs, chicken, fish, beans, or dairy will do the job.
How Long Recovery Takes
A mild Grade 1 strain often feels better within a week. You might still notice some tenderness or tightness, but normal activity is usually possible. Grade 2 strains are more variable and can take several weeks to a few months, depending on the muscle involved and how well you manage the early recovery. Hamstring strains, for example, are notoriously slow healers because the muscle is under load during so many daily movements.
Returning to activity too quickly is the most common mistake. A muscle that feels “fine” at rest can still be vulnerable under load. Gradually increase the intensity of your activities and use pain as a reliable signal. If a movement hurts, your tissue isn’t ready for that level of stress yet.
Your mindset during recovery also plays a measurable role. Catastrophic thinking, fear of re-injury, and depression are all associated with slower recovery and worse outcomes. Staying optimistic and trusting the healing process isn’t just feel-good advice. It correlates with genuinely better results.
Signs You Need Medical Attention
Most pulled muscles heal on their own with the approach described above. But some signs point to a more serious injury. If you heard or felt a pop when the injury happened, can’t move the muscle at all, or have severe bruising and swelling, get evaluated promptly. These suggest a possible complete tear that may need imaging or surgical repair.
Also watch for nerve-related symptoms: numbness, tingling, sudden weakness, or difficulty controlling the muscle. These can indicate nerve damage alongside the strain. A physical exam is usually enough to diagnose a pulled muscle, but an MRI can be used to determine the extent of the tear when the injury seems severe or isn’t improving on the expected timeline.
