One unit of Botox is a measure of biological potency, not weight or volume. It’s defined as the amount of botulinum toxin needed to produce a specific effect in a standardized laboratory test. In practical terms, a single unit is an extraordinarily tiny quantity of protein, and the number of units injected determines how much muscle relaxation you get and how long it lasts.
How a “Unit” Is Measured
Unlike most medications measured in milligrams or milliliters, Botox is measured by what it does rather than how much it weighs. One unit is determined through a biological assay, a lab test that gauges the toxin’s strength in living tissue. The standard method has historically been the mouse LD50 assay, which measures the dose needed to produce a median lethal effect in mice. That result becomes the reference point for defining a single unit of potency.
This matters because botulinum toxin is incredibly potent. The actual mass of protein in one unit is so small it can’t be meaningfully weighed on a standard scale. A full 100-unit vial of Botox contains just 0.5 mg of human albumin (a stabilizing protein) and 0.9 mg of sodium chloride alongside the toxin itself. The toxin portion is a fraction of that total.
What One Unit Looks Like Physically
Botox arrives as a freeze-dried powder that gets mixed with saline before injection. The physical volume of one unit depends entirely on how much saline the practitioner adds. When a 100-unit vial is diluted with 2 ml of saline, one unit equals about 0.02 ml, roughly one-fiftieth of a milliliter. When diluted with 10 ml (as used for bladder injections), one unit equals 0.1 ml. Either way, the potency is the same. Only the concentration of the liquid changes.
How It Works in Your Body
Each unit of Botox blocks nerve signals that tell muscles to contract. The toxin binds to nerve endings, gets absorbed into the cell, and then cuts a specific protein that nerves need to release their chemical messenger (acetylcholine) at the junction where nerve meets muscle. Without that messenger, the muscle can’t receive the signal to tighten. The result is temporary relaxation of the targeted muscle.
This effect isn’t instant. Initial weakening takes several days, with peak results arriving a few weeks after injection. The relaxation fades around the two-month mark, and normal muscle strength typically returns by three months. This is why most cosmetic Botox patients schedule treatments every three to four months.
How Many Units Go Into Common Treatments
Knowing what one unit is becomes more useful once you see how units add up across different treatment areas:
- Horizontal forehead lines: 15 to 30 units
- Frown lines between the eyebrows (glabellar lines): up to 40 units, with men often needing higher doses due to stronger muscles
- Crow’s feet: 6 to 10 units per side, up to 20 units total
A typical cosmetic visit might involve 30 to 60 units spread across multiple areas. Medical uses like chronic migraine or muscle spasticity can require significantly more, sometimes 100 to 200 units per session.
What One Unit Costs
For cosmetic treatments, most clinics in the United States charge somewhere between $12 and $25 per unit, with the range depending on your city and the injector’s experience. That means a 30-unit forehead treatment could run $360 to $750, while a full-face session using 60 units might cost $720 to $1,500. Some clinics offer flat-rate pricing per area instead of per-unit pricing, so it’s worth asking how the fee is structured before your appointment.
Units Are Not Interchangeable Between Brands
This is one of the most important things to understand about Botox units. A “unit” of Botox (onabotulinumtoxinA) is not the same as a “unit” of Dysport (abobotulinumtoxinA) or Xeomin (incobotulinumtoxinA). Each brand uses its own proprietary assay to define its units, and there is no universal standard.
Xeomin and Botox are generally considered equivalent at a 1:1 ratio, meaning 20 units of one produces a similar effect to 20 units of the other. Dysport, however, requires roughly 2.5 to 3 times as many units to match the same effect. So 20 units of Botox would translate to approximately 50 to 60 units of Dysport. Using the wrong conversion ratio can lead to under-treatment or, more concerning, overdosing. If you’re switching between brands, your injector should be adjusting the unit count accordingly rather than simply matching numbers.
