A cath lab tech is a healthcare professional who works alongside cardiologists during minimally invasive heart procedures, helping diagnose and treat cardiovascular problems without open-heart surgery. Their official title is often cardiovascular invasive specialist, though you’ll also hear cardiac catheterization technologist. They work in a specialized operating-room-like suite called a cardiac catheterization laboratory, where thin, flexible tubes called catheters are threaded through blood vessels to reach the heart.

What a Cath Lab Tech Actually Does

The core of the job revolves around catheter-based heart procedures. Before a procedure begins, a cath lab tech prepares the patient by shaving and cleaning the insertion site (usually the wrist or groin), applying local anesthesia, and setting up all the sterile equipment the cardiologist will need. They also calibrate and test imaging systems, pressure monitors, and recording devices.

During the procedure itself, the tech takes on one of two roles: scrubbing in at the table to hand instruments and assist the physician directly, or circulating around the room to manage equipment, pull medications, and troubleshoot. Throughout, they monitor the patient’s blood pressure, heart rhythm, and oxygen levels on an electrocardiogram (EKG) display. They watch for dangerous rhythm changes or drops in pressure and alert the physician immediately. Because fluoroscopy runs throughout most procedures, cath lab techs spend their careers near X-ray equipment. Radiation exposure is the biggest occupational hazard in this role. Everyone in the room wears a lead apron, which blocks over 95% of scattered radiation. Pregnant staff members wear an additional dosimeter at waist level under the apron and use thicker shielding. Proper fit matters: gaps under the arms, for instance, can expose breast tissue to unnecessary radiation.

On-Call Demands

Heart attacks don’t follow a schedule, so cath lab techs carry a significant on-call burden. When a patient arrives at the emergency department with a blocked coronary artery, the cath lab team needs to mobilize fast, often in the middle of the night. This crossover pathway typically requires at least one year of full-time work in invasive cardiovascular technology plus a minimum of 600 completed cardiac procedures before sitting for the national certification exam.

Certification Options

The primary credential is the Registered Cardiovascular Invasive Specialist (RCIS), administered by Cardiovascular Credentialing International (CCI). The exam costs $365, lasts three hours, and consists of 170 multiple-choice questions covering hemodynamics, anatomy, pharmacology, equipment operation, and patient care. You need a high school diploma plus one of the qualifying pathways described above.

There are three main eligibility routes. Graduates of accredited invasive cardiovascular programs can sit for the exam immediately. Graduates of non-accredited programs need at least 800 clinical hours completed during their training. Healthcare professionals crossing over from other fields need a year of cath lab experience and 600 documented procedures.

Radiologic technologists have a separate option: the Cardiac Interventional Radiography credential through the American Registry of Radiologic Technologists (ARRT), which requires structured education and documented clinical procedures specific to cardiac intervention. Some employers accept either credential, though RCIS is more widely recognized in dedicated cath labs.
