A certified registered nurse is a licensed RN who has earned an additional specialty credential by passing a national certification exam in a specific area of nursing practice. This goes beyond the standard RN license that every practicing nurse holds. While there are roughly 5.9 million active RN licenses in the United States, only a fraction of those nurses carry a board certification. The American Nurses Credentialing Center alone has certified more than 250,000 nurses, and several other organizations offer their own credentials.
How Certification Differs From an RN License
An RN license is a legal requirement to practice nursing. You earn it by graduating from an approved nursing program and passing the NCLEX-RN exam. It tells employers and patients that you’ve met the minimum standard to provide nursing care. Every nurse who works at the bedside, in a clinic, or in any other clinical setting must have one.
A certification, by contrast, is voluntary. It signals that a nurse has developed deep expertise in a particular clinical area, such as critical care, pediatrics, or cardiac nursing, and has proven that knowledge on a standardized exam. You’ll often see it reflected in the letters after a nurse’s name. For example, “RN, CCRN” indicates an RN certified in critical care, while “RN, MEDSURG-BC” indicates board certification in medical-surgical nursing.
Common Specialty Certifications
Dozens of certifications exist across nearly every branch of nursing. Some of the most recognized categories include:
- Nurse practitioner certifications: Family nurse practitioner (FNP-BC), psychiatric-mental health nurse practitioner (PMHNP-BC), and adult-gerontology acute or primary care nurse practitioner.
- Clinical nurse specialist: Adult-gerontology clinical nurse specialist (AGCNS-BC).
- Bedside and unit-based specialties: Medical-surgical nursing, cardiac-vascular nursing, pediatric nursing, pain management nursing, gerontological nursing, and ambulatory care nursing.
- Leadership and administrative roles: Nurse executive certification and nursing case management certification.
- Niche practice areas: Informatics nursing, nursing professional development, wound and ostomy care, and healthcare disaster response.
Several national organizations grant these credentials. The American Nurses Credentialing Center (ANCC) is the largest, but the American Association of Critical-Care Nurses (AACN), the Oncology Nursing Certification Corporation (ONCC), and other specialty bodies each administer their own exams tailored to their clinical focus.
Eligibility Requirements
You can’t sit for a certification exam right out of nursing school. Most certifying bodies require a current, unrestricted RN license plus a minimum number of hours spent providing direct patient care in the relevant specialty. The exact requirements vary by certification, but a common structure looks like this:
For the AACN’s progressive care certification (PCCN), for instance, nurses choose between two pathways. The two-year option requires 1,750 hours of direct care for acutely ill adult patients within the past two years, with at least 875 of those hours in the most recent year. The five-year option requires 2,000 hours over five years, with at least 144 in the most recent year. Hours must come from hands-on patient care or from supervising nurses at the bedside in roles like educator, manager, or preceptor.
Other certifications follow a similar pattern: a combination of time in practice and documented clinical hours, ensuring that candidates have real-world experience before they test.
The Certification Exam
Once eligible, nurses apply through the certifying organization and schedule a proctored exam, typically at a designated testing center. These exams are multiple-choice and focus on clinical judgment, evidence-based practice, and specialty-specific knowledge. Most contain between 100 and 200 questions and take several hours to complete. Pass rates vary by specialty, and many nurses spend weeks or months studying with review courses, practice exams, and specialty textbooks before sitting for the test.
Keeping the Certification Current
Certification isn’t permanent. Most credentials must be renewed on a set cycle, commonly every two to five years depending on the issuing organization. Renewal typically requires completing continuing education hours and maintaining a minimum amount of clinical practice in the specialty. Some boards also offer a renewal-by-exam option. The continuing education requirement ensures certified nurses stay current with evolving standards of care, new treatments, and updated clinical guidelines.
Impact on Patient Care
Certification isn’t just a professional badge. Research links higher rates of certified nurses on staff to measurable improvements in patient outcomes. Hospitals with board-certified infection control directors have significantly lower rates of MRSA bloodstream infections. Facilities that employ certified wound, ostomy, and continence nurses see better outcomes in preventing hospital-acquired pressure injuries. Studies have also found a statistically significant relationship between higher specialty certification rates among RNs and lower patient fall rates in acute care settings.
These findings make sense intuitively: a nurse who has invested the time to master a clinical area and prove it on a national exam tends to catch problems earlier, follow evidence-based protocols more consistently, and contribute to a stronger safety culture on the unit.
Salary and Career Benefits
Certified nurses generally earn more than their non-certified peers. Across the top 20 nursing specialties, the average salary for a certified professional is about 23% higher. In a broad survey, roughly 30% of nurse respondents held a specialty certification, and their salaries were 17% higher on average than those without one.
The premium varies dramatically by specialty. Certified dermatology nurses earn an estimated 52% more than non-certified nurses in the same field. Neurology and neuroscience certification carries a 49% salary premium, as does certification in ear, nose, and throat nursing. Clinical nurse specialists with board certification see a roughly 48% increase, and urgent care nurses with credentials earn about 47% more.
Beyond salary, certification often opens doors to leadership positions, clinical educator roles, and advanced practice opportunities. Many hospitals factor certification into hiring decisions, promotions, and clinical ladder programs that reward professional development with increased pay and responsibility.
