A certified surgical technologist (CST) is a healthcare professional who has earned a national credential verifying their competence to work in operating rooms, preparing instruments, maintaining sterile conditions, and assisting surgeons during procedures. The “certified” distinction means the person has graduated from an accredited program and passed a national certification exam administered by the National Board of Surgical Technology and Surgical Assisting (NBSTSA). As of May 2024, surgical technologists earn a median annual wage of $62,830.
What a Surgical Technologist Actually Does
Surgical technologists are the people who keep the operating room running smoothly before, during, and after every procedure. Before surgery begins, they prepare the operating room by sterilizing equipment, arranging surgical instruments on trays in the exact order the surgeon will need them, and ensuring all supplies are accounted for. They also help drape the patient and set up the sterile field, which is the carefully controlled area around the surgical site designed to prevent infection.
During the operation, the surgical technologist stands near the surgeon and passes instruments, holds retractors, cuts sutures, and anticipates what the surgeon will need next. This requires deep familiarity with surgical procedures, anatomy, and the specific preferences of individual surgeons. They also monitor the sterile field throughout the case, watching for any breaks in technique that could introduce contamination. After surgery, they count instruments and sponges to confirm nothing was left inside the patient, help apply dressings, and prepare the room for the next case.
Most surgical technologists work in hospitals, though outpatient surgery centers, dental offices, and physicians’ offices also employ them. The job is physically demanding. You’re on your feet for long stretches, sometimes for hours during complex procedures, and the work requires sustained focus in high-pressure situations.
What “Certified” Means and Why It Matters
The CST credential is awarded by the NBSTSA after a candidate passes a standardized exam. To sit for the exam, you must graduate from a surgical technology program accredited by either CAAHEP (the Commission on Accreditation of Allied Health Education Programs) or ABHES (the Accrediting Bureau of Health Education Schools). You cannot simply study on your own and take the test.
Certification matters because it serves as a standardized proof of competency that employers recognize nationwide. While not every state legally requires certification, holding the CST credential makes you significantly more competitive in the job market. Many hospitals and surgery centers list it as a hiring requirement or strong preference regardless of state law.
Once certified, you don’t hold the credential forever. The CST operates on a two-year renewal cycle that requires completing continuing education credits. This ensures certified technologists stay current with evolving surgical techniques, new equipment, and updated infection prevention standards.
Education and Training Path
Most people enter this career through an associate degree program in surgical technology, which typically takes about two years. Some community colleges and vocational schools offer certificate or diploma programs that can be completed faster, though associate degree holders may have a slight edge in hiring. Either way, the program must be accredited by CAAHEP or ABHES for you to qualify for the CST exam.
These programs combine classroom instruction in anatomy, microbiology, pharmacology, and surgical procedures with hands-on clinical rotations in actual operating rooms. Clinical hours are a critical part of the training. You’ll scrub into live surgeries under supervision, gaining experience across multiple specialties like orthopedics, general surgery, and obstetrics. Military veterans who completed surgical technology training through the armed forces, National Guard, or U.S. Public Health Service also qualify to work in the field and may be eligible for certification.
State Requirements Vary Widely
Only about thirteen states have laws specifically addressing the education and certification of surgical technologists, and just five states require surgical techs to register with the state. In all other states, the hiring facility decides its own credentialing requirements for who can work in the operating room.
Tennessee’s law offers a good example of how states that do regulate the profession typically structure their requirements. There, surgical technologists must meet at least one of several criteria: completing an accredited program and holding national certification, completing an accredited program and obtaining certification within eighteen months of finishing, having performed surgical technology services before a specific date with documented competency, or completing military surgical technology training. Other states with regulations follow similar frameworks, usually including a grandfather clause for experienced technologists who were already working before the law took effect.
Even in states without legal mandates, the trend is moving toward certification. Hospitals seeking accreditation from organizations like The Joint Commission often implement their own policies requiring or strongly preferring CST-credentialed staff.
Career Growth and Advancement
Employment for surgical technologists is projected to grow 4 percent from 2024 to 2034, which is faster than the average for all occupations. This growth is driven by an aging population that needs more surgical procedures and the expansion of outpatient surgical centers.
For technologists who want to advance, the most common next step is becoming a certified surgical first assistant (CSFA). While a surgical technologist works primarily within the sterile field passing instruments and maintaining the setup, a surgical first assistant takes on a more hands-on clinical role. First assistants may help with tissue retraction, hemostasis (controlling bleeding), and wound closure under the surgeon’s direct supervision. This role requires additional education and a separate certification.
Other surgical technologists move into education, becoming instructors in accredited programs. Some transition into sales roles for surgical equipment and device companies, where their operating room experience gives them credibility with the surgeons and hospital systems they’re selling to. Management positions like surgical services coordinators or sterile processing directors are also options with additional experience.
What the Day-to-Day Looks Like
Surgical technologists typically work full time, and schedules can include early mornings, evenings, weekends, and on-call shifts for emergency surgeries. A typical day might involve scrubbing into three to five cases depending on length and complexity, with time between each to break down the room and set up again.
The pace varies by setting. Trauma centers and large teaching hospitals tend to have longer, more complex cases and unpredictable schedules. Ambulatory surgery centers that focus on procedures like cataract removal, knee arthroscopy, or hernia repair often have more predictable hours and a faster turnover between cases. Some technologists prefer to specialize in a particular surgical area like cardiovascular, neurosurgery, or orthopedics, which can lead to higher pay and more specialized expertise over time.
