A CNA, or certified nursing assistant, is a frontline healthcare worker who helps patients with basic daily needs like bathing, dressing, eating, and moving around. CNAs work under the supervision of registered nurses and licensed practical nurses, providing the hands-on, everyday care that keeps patients comfortable and safe. They are found in nearly every healthcare setting, from nursing homes to hospitals, and represent one of the fastest entry points into a nursing career.
What a CNA Does Every Day
The core of a CNA’s job revolves around what healthcare professionals call activities of daily living. These are the fundamental tasks that sick, elderly, or recovering patients struggle to do on their own: getting out of bed, bathing, dressing, using the toilet, walking, and eating. A CNA might spend the morning helping residents in a nursing home get dressed and groomed, then assist with meals, reposition patients who can’t move independently, and help them exercise or walk the hallways.
Beyond personal care, CNAs also handle basic clinical tasks. They take and record vital signs like blood pressure, temperature, pulse, and breathing rate. They track how much a patient eats and drinks, measure urine output, and report any changes to the nursing staff. This monitoring role is critical because CNAs spend more direct time with patients than almost anyone else on the care team, making them often the first to notice when something is off.
What CNAs cannot do is equally important. They are not allowed to perform nursing assessments, administer most medications, start IV lines, or carry out complex wound care. Those tasks fall to licensed practical nurses and registered nurses. A CNA’s scope is deliberately limited to basic care and observation, with everything flowing through the supervision of a licensed nurse.
Hospital CNAs vs. Long-Term Care CNAs
The setting shapes what a CNA’s day looks like. In a long-term care facility or nursing home, the work centers on building ongoing relationships with residents. You assist the same people day after day with their routines, help maintain their dignity, and become familiar with their baseline health. The pace is steady and predictable.
In a hospital, the scope can be somewhat broader and the pace faster. Hospital CNAs (sometimes called clinical partners or patient care technicians) may assist with tasks like applying dressings, collecting specimens, administering enemas, or helping set up feeding equipment, all under direct nurse supervision. Patients change frequently, conditions are more acute, and the environment demands quicker decision-making about when to alert the nursing team. Both settings require the same certification, but the daily experience can feel quite different.
Training and Certification Requirements
Becoming a CNA is one of the quickest paths into healthcare. Federal law requires a minimum of 75 hours of training, including at least 16 hours of supervised clinical practice with real patients. However, more than half of U.S. states set their minimums higher. Thirty states and the District of Columbia require more than 75 hours, with some going as high as 180. Thirteen states and D.C. require at least 120 hours, which is the standard the National Academy of Medicine recommends.
Training programs are offered through community colleges, vocational schools, hospitals, and even some nursing homes. The coursework covers infection control, patient safety, communication skills, basic anatomy, and the hands-on techniques for assisting with daily care. After completing an approved program, you must pass a state competency exam that typically includes both a written (or oral) knowledge test and a skills demonstration where you perform tasks like taking blood pressure or transferring a patient from a bed to a wheelchair.
Once you pass, your name goes on your state’s nurse aide registry. Certification generally lasts two years. To renew, you need to show that you’ve worked at least eight hours in a nursing or nursing-related role during that 24-month period. If your certification lapses because you haven’t worked, you may need to retake the competency exam.
How CNAs Compare to LPNs and RNs
The nursing world has a clear hierarchy of education, responsibility, and scope. A CNA sits at the entry level, providing basic care under supervision. A licensed practical nurse (LPN) has completed roughly one year of nursing education and can administer medications (including injections and IV drugs in some states), perform wound care, start IV lines, and collect more detailed patient data. LPNs may also supervise CNAs directly.
Registered nurses hold either a two-year associate degree or a four-year bachelor’s degree in nursing. They assess patients, develop care plans, administer all types of medications, and make clinical judgments about treatment. RNs oversee both LPNs and CNAs. The key distinction is autonomy: CNAs carry out delegated tasks, LPNs perform a broader set of clinical duties with some independence, and RNs have the full scope of nursing practice including assessment and care planning.
Salary and Job Outlook
The median annual wage for nursing assistants was $39,530 in May 2024, which works out to about $18.96 per hour. Pay varies significantly by setting and location. Hospital CNAs generally earn more than those in nursing homes, and states with higher costs of living tend to offer higher wages.
Job growth for nursing assistants is projected at 2 percent from 2024 to 2034, which is slower than average across all occupations. That said, the sheer size of the workforce means tens of thousands of openings will still appear each year as workers retire, move into other roles, or leave the field. Demand in long-term care facilities remains particularly steady as the population ages.
Career Advancement From CNA
Many people use the CNA role as a stepping stone. Working as a CNA gives you direct patient care experience, exposure to clinical environments, and a realistic sense of whether nursing is the right career before committing to years of additional schooling.
The most common next step is pursuing an LPN certificate or an associate degree in nursing (ADN) to become a registered nurse. Some accelerated programs allow CNAs to earn an ADN in as little as one year or a bachelor of science in nursing (BSN) in under two years. Major nursing organizations recommend that associate-degree nurses eventually complete an RN-to-BSN program, since a bachelor’s degree opens doors to leadership roles, specialty certifications, and graduate programs.
From a BSN, the path continues to nurse practitioner programs, clinical nurse specialist roles, nursing education, and executive positions, all of which require a master’s or doctoral degree. For someone who enters healthcare as a CNA, each educational step builds on the practical foundation of knowing what patient care actually looks and feels like at the bedside.
