It’s a professional credential earned by registered nurses who specialize in caring for patients undergoing imaging procedures, from routine X-rays and CT scans to complex interventional radiology. The certification is issued by the Radiologic Nursing Certification Board (RNCB) and signals a validated level of competency in this specialty.
What a Certified Radiology Nurse Does
Radiology nurses work at the intersection of patient care and diagnostic imaging. Their day-to-day responsibilities look quite different from a typical hospital floor nurse. They start and check IV lines, administer medications (including contrast dyes used to enhance imaging), monitor vital signs, and manage sedation for patients who need it during procedures. They also insert urinary catheters, suction airways, and attend to patients’ personal needs before, during, and after scans or interventional treatments.
A significant part of the role involves working with high-acuity patients. Radiology departments regularly receive patients transferred from intensive care units, emergency situations, and pediatric cases requiring sedation. The CRN is often the person monitoring a patient’s heart rhythm and oxygen levels on a cardiac or pulse oximetry monitor while a radiologist performs a procedure. They also serve as the communication link between the patient and the imaging team, alerting the technologist or radiologist to any unusual patient needs.
Radiation Safety Responsibilities
Because CRNs work in environments with ionizing radiation, they follow strict safety protocols built around a principle called ALARA, which stands for “as low as reasonably achievable.” The idea is simple: any radiation exposure carries some increased risk, so every effort should be made to minimize it for both patients and staff. There’s no known safe threshold below which radiation definitively won’t cause harm, which is why the radiologic community treats every dose as meaningful.
In practice, this means CRNs help ensure proper positioning of equipment (keeping the X-ray source as far from the patient as possible while placing the detector close), wearing protective shielding, and confirming that each imaging procedure is clinically justified before it happens. They also need to recognize and respond quickly to adverse reactions to contrast media, which can range from mild allergic responses to serious anaphylaxis.
How to Earn the CRN Credential
To sit for the CRN exam, you must hold an active registered nurse license (or its international equivalent) and have logged at least 2,000 hours of radiology nursing practice within the past three years. That works out to roughly a year of full-time work in a radiology setting, though many candidates spread it across two or three years of part-time or rotating assignments.
The exam itself is computer-based and consists of 150 multiple-choice questions covering topics like patient assessment, procedural care, sedation management, contrast administration, and radiation safety. Once you pass, your certification is valid for four years.
Keeping the Certification Active
To recertify after that four-year period, you need to complete 60 contact hours of continuing education. At least 30 of those hours must be specifically related to radiology nursing, while the remaining 30 can come from broader nursing education topics. Recertification is also valid for four years, creating a continuous cycle of learning and renewal throughout your career.
Where CRNs Work
Most certified radiology nurses work in hospital imaging departments, which remain the largest employer for this specialty. But the role extends to outpatient imaging centers, ambulatory surgery centers that perform image-guided procedures, and clinics affiliated with large health systems. Some CRNs also work in cardiac catheterization labs or interventional radiology suites, where the procedures are more invasive and the nursing demands more intense.
Salary and Job Outlook
The Bureau of Labor Statistics reports a median annual salary of $93,600 for registered nurses, which includes specialty roles like radiology nursing. That translates to roughly $45 per hour. Earning the CRN credential can position you for higher-paying positions within imaging departments, since the certification demonstrates specialized expertise that employers value.
