What Is a Garlic Supplement Good For?

Garlic supplements have the strongest evidence for lowering blood pressure, with clinical trials showing reductions of about 9 points in systolic pressure among people with hypertension. Beyond that headline benefit, research supports more modest effects on blood sugar, cholesterol, and immune function. The type of supplement you choose matters more than you might expect, because garlic’s active compounds behave very differently depending on how they’re processed.

This is where garlic supplements have the most convincing track record. A meta-analysis published in the American Journal of Hypertension found that garlic preparations lowered systolic blood pressure by an average of 9.1 mmHg and diastolic by 3.8 mmHg in people with high blood pressure, compared with placebo. To put that in context, a 9-point systolic drop is roughly what you’d get from cutting sodium intake significantly or starting a structured exercise program. The effect was specific to people who already had elevated blood pressure; garlic doesn’t appear to push normal readings lower.

The blood-thinning properties of garlic contribute to these cardiovascular effects. A sulfur compound in garlic irreversibly blocks platelets from clumping together, which is the same basic mechanism as aspirin. This is useful for circulation but creates a real concern if you’re taking blood thinners like warfarin or clopidogrel, or if you have surgery coming up. Anesthesia guidelines recommend stopping garlic supplements seven days before any procedure. Surgical case reports have documented excessive bleeding that converted minimally invasive operations into open surgeries because of poor clotting in patients taking garlic supplements.

Blood Sugar and Insulin Sensitivity

A 2024 meta-analysis of randomized controlled trials found that garlic supplementation reduced fasting blood glucose by about 7 mg/dL on average and lowered HbA1c (the three-month blood sugar average) by 0.66 percentage points. That HbA1c reduction is clinically meaningful. For someone with prediabetes or early type 2 diabetes, a drop of that size could represent the difference between progressing and staying stable. Separate trials have also shown improvements in insulin resistance, particularly in people with fatty liver disease, after about three months of garlic powder supplementation.

These effects are supplementary, not a replacement for other interventions. But for someone already managing their blood sugar through diet and exercise, garlic may offer a modest additional benefit.

Immune Function and Cold Severity

Garlic’s reputation as an immune booster is partially supported by research, though the results are more nuanced than the marketing suggests. A 90-day randomized trial during cold and flu season gave 120 healthy adults either aged garlic extract (2.56 grams daily) or a placebo. The garlic group didn’t get sick less often, but when they did catch a cold or flu, they had fewer symptoms, fewer days of feeling run down, and missed fewer days of work or school.

So garlic supplements won’t prevent you from catching a cold, but they may reduce how miserable you feel and how long it disrupts your life. The mechanism likely involves garlic’s effects on certain immune cells that respond to infections, rather than preventing the infection itself.

Why the Type of Supplement Matters

Garlic’s health effects come from sulfur-containing compounds, but which compounds you actually absorb depends entirely on how the supplement is made. This is one of the most important and least understood aspects of garlic supplementation.

Fresh garlic contains a compound called alliin, which is inert on its own. When you crush or chop a clove, an enzyme converts alliin into allicin, the pungent compound responsible for garlic’s smell. Here’s the catch: allicin is extremely unstable. It has a half-life of about 2.5 days at room temperature and breaks down rapidly once you swallow it. No allicin has ever been detected in human blood or urine even after eating 25 grams of raw garlic (roughly seven cloves). The enzyme that creates allicin is also destroyed by stomach acid, so swallowing a whole clove produces essentially no allicin at all.

This means garlic powder tablets, which rely on that enzyme activating in your gut, are inconsistent. Enteric-coated tablets try to solve this by delaying release past the stomach, but results vary. Garlic oil supplements contain the fat-soluble breakdown products of allicin rather than allicin itself. Oil macerates contain the highest concentration of the compound responsible for garlic’s blood-thinning effects, making them the most potent form for cardiovascular purposes but also the riskiest for bleeding complications.

Aged garlic extract takes a different approach entirely. During months of aging in liquid, garlic’s compounds transform into water-soluble molecules like S-allylcysteine, which is stable, survives digestion intact, and has been detected in plasma, liver, and kidney tissue in studies. This form was used in the immune function trial and has the most consistent absorption profile. It also has less odor, which is why it’s popular despite typically requiring higher doses.

Dosages Used in Research

Clinical trials have used a range of doses depending on the form. Common daily amounts include 2 to 5 grams of fresh raw garlic (roughly one to two cloves), 0.4 to 1.2 grams of dried garlic powder, 2 to 5 milligrams of garlic oil, 300 to 1,000 milligrams of garlic extract in solid form, and 2,400 milligrams of aged garlic extract in liquid form. The immune function trial specifically used 2.56 grams per day of aged garlic extract over 90 days.

Most studies showing blood pressure and blood sugar benefits ran for at least 8 to 12 weeks, so garlic supplements aren’t a quick fix. Consistent daily use over months is what the evidence supports.

Who Should Be Cautious

Garlic supplements are generally well tolerated, but the blood-thinning effect is a genuine safety concern for specific groups. If you take anticoagulants or antiplatelet medications, garlic supplements can amplify their effects and increase bleeding risk. Oil-based garlic supplements carry the highest risk in this category. Anyone scheduled for surgery should stop garlic supplements at least a week beforehand. Gastrointestinal side effects like heartburn, nausea, and body odor are common but usually mild, especially with aged garlic extract, which tends to be gentler on the stomach than raw or powdered forms.
