A healthy body temperature for most adults falls in the range of 97.0°F to 99.0°F (36.1°C to 37.2°C), not the classic 98.6°F you probably grew up hearing. That old number dates back to 1868, and average human body temperature has measurably dropped since then. Your own “normal” depends on your age, the time of day, where you measure, and even your menstrual cycle.
Where 98.6°F Came From
The 98.6°F standard traces back to a German physician named Carl Wunderlich, who published a landmark study in 1868 analyzing over one million armpit temperature readings from roughly 25,000 patients. He identified 37.0°C (98.6°F) as the mean temperature of healthy adults, and that number stuck for more than 150 years.
But Wunderlich’s number no longer reflects reality. A Stanford University study using 677,423 temperature measurements found that average body temperature in the United States has been declining by about 0.05°F per decade. Men born in the early 1990s run roughly 1.06°F cooler than men born in the early 1800s. Women born in the same era run about 0.58°F cooler than women born in the 1890s. The likely explanation: lower rates of chronic infection and inflammation across the population, combined with the fact that most people now live in climate-controlled environments. Both of those reduce your body’s baseline metabolic rate, which produces less heat.
How Temperature Changes Throughout the Day
Your body temperature isn’t a fixed number. It follows a predictable circadian rhythm, hitting its lowest point around 6 a.m. and peaking around 8 p.m. In healthy people, that daily swing ranges from 0.5°F to 1.9°F, which means a reading of 97.5°F in the early morning and 98.8°F in the evening could both be perfectly normal for the same person.
Several other factors can push your temperature up or down temporarily. Vigorous exercise is the most dramatic: without the body’s cooling mechanisms (sweating, increased blood flow to the skin), even moderate exercise would raise core temperature by about 1.8°F every five to six minutes. Well-trained endurance athletes can sustain core temperatures of 102°F to 106°F during prolonged effort and still be fine. Stress, dehydration, hot weather, and warm clothing all nudge your reading higher as well.
Temperature Differences by Age
Children tend to run warmer than adults, partly because their metabolisms are higher relative to their body size. For kids, a fever is defined as a rectal or ear temperature of 100.4°F (38°C) or higher, an oral temperature of 100°F (37.8°C) or higher, or an armpit reading of 99°F (37.2°C) or higher.
Older adults trend in the opposite direction. Core body temperature decreases with age, and adults over 65 typically have a baseline between 96.4°F and 98.5°F (35.8°C to 36.9°C). This matters because a reading of 99°F in a 75-year-old may represent a significant fever, even though the same number would be unremarkable in a younger adult.
How the Menstrual Cycle Affects Temperature
After ovulation, rising progesterone levels cause basal body temperature to increase by 0.4°F to 1.0°F (0.22°C to 0.56°C). This shift happens during the second half of the cycle (the luteal phase) and stays elevated until the next period begins. It’s why tracking morning temperature before getting out of bed is a common method for identifying ovulation. The exact size of the rise varies from person to person, but even at its smallest, it’s a consistent and measurable change.
Where You Measure Matters
Not all thermometer placements give you the same number. A rectal reading runs 0.5°F to 1.0°F higher than an oral reading, while an armpit (axillary) reading runs 0.5°F to 1.0°F lower. Forehead (temporal artery) and ear (tympanic) thermometers fall somewhere in between, though they tend to be less consistent. Rectal temperatures are considered the most accurate, which is why they’re the standard for infants and young children. For everyday use in adults, oral thermometers are reliable as long as you haven’t eaten or had a drink in the last 15 minutes.
If you’re comparing a reading to a fever threshold, make sure you’re using the right cutoff for your measurement site. An armpit temperature of 99°F is more concerning than an oral reading of the same number.
When Temperature Is Too High
The CDC defines a fever as a measured temperature of 100.4°F (38°C) or higher. This is the threshold used in most clinical settings and public health screening. A fever itself isn’t a disease. It’s your immune system deliberately raising your thermostat to create a less hospitable environment for viruses and bacteria.
Low-grade fevers (around 99°F to 100.3°F) often resolve on their own and may not need treatment. Temperatures above 103°F in adults warrant closer attention, and anything above 105°F can become dangerous as proteins in the body begin to function abnormally at sustained high heat.
When Temperature Is Too Low
Hypothermia begins when core body temperature drops below 95°F (35°C). It’s classified in three stages:
- Mild (90°F to 95°F): Shivering, poor coordination, difficulty with fine motor tasks.
- Moderate (82°F to 90°F): Shivering may stop, confusion sets in, drowsiness increases.
- Severe (below 82°F): Loss of consciousness, dangerously slow heart rate, risk of cardiac arrest.
Older adults are especially vulnerable because their lower baseline temperatures mean they have less margin before crossing into hypothermia. A cool indoor environment that feels comfortable to a younger person may gradually lower an older adult’s core temperature over hours.
Finding Your Own Baseline
Given how much healthy temperature varies, the most useful thing you can do is learn what’s normal for you. Take your temperature at the same time of day, using the same method, over a week or two when you’re feeling well. Most people will find their average lands somewhere between 97.0°F and 98.6°F. Once you know your personal baseline, you can spot a meaningful elevation earlier, even if the thermometer still reads below the official 100.4°F fever cutoff.
