An A1c of 6.5% or higher is considered high enough to diagnose diabetes. Below that, a result between 5.7% and 6.4% falls in the prediabetes range, and anything under 5.7% is normal. These thresholds matter because they mark the points where your risk of serious health complications begins to climb.
The A1c Ranges That Matter
The A1c test measures the percentage of your red blood cells that have glucose stuck to them. Because red blood cells live for about three months, the test captures a rolling average of your blood sugar rather than a single snapshot. About half of the reading reflects the most recent 30 days, with the remaining half weighted toward the 30 to 90 days before that. So recent weeks influence your result more than earlier ones.
The diagnostic cutoffs are straightforward:

- Prediabetes: 5.7% to 6.4%
- Diabetes: 6.5% or above
If you already have diabetes, the general target is an A1c under 7%. That number translates to an estimated average blood sugar of about 154 mg/dL. When a Higher Target Is Appropriate
A target under 7% works for most adults with diabetes, but it isn’t universal. The American Diabetes Association’s 2025 guidelines recognize that some people benefit from a less aggressive goal, up to 8%. This applies if you experience frequent or severe low blood sugar episodes, have a limited life expectancy where intensive control may not pay off in time, or live with significant cognitive or functional impairments that make tight management risky. In those cases, pushing for a lower number can cause more harm than good, particularly from dangerous blood sugar drops.
What High A1c Feels Like
A mildly elevated A1c, say in the 6.5% to 7% range, often produces no obvious symptoms. Many people discover it on routine bloodwork. As the number climbs higher, though, chronically elevated blood sugar starts to announce itself. Early signs include increased thirst, frequent urination, headaches, and blurred vision. These happen because excess glucose pulls water from your tissues and overwhelms your kidneys’ ability to reabsorb it.
When blood sugar stays high for months or years, the symptoms shift. Persistent fatigue, unexplained weight loss, slow-healing cuts, recurring skin or yeast infections, and tingling in your hands or feet all point to long-term damage that a single glucose reading would miss. This is exactly the window the A1c test is designed to catch.
Health Risks at Specific Levels
The connection between A1c and complications isn’t a vague “higher is worse.” Research in Diabetes Care found that the risk of microvascular complications (damage to small blood vessels in the eyes, kidneys, and nerves) increases significantly once A1c reaches 7.2% or above. Major cardiovascular events follow the same pattern, rising sharply at 7.2% and above compared to readings in the low-to-mid 6% range.
Interestingly, very low A1c levels in people being treated for diabetes also carry risk. In patients with severe kidney disease, A1c readings below 5.8% were linked to increased cardiovascular events, likely because aggressive medication use can trigger dangerous drops in blood sugar. This U-shaped pattern is one reason doctors personalize targets rather than pushing everyone toward the lowest possible number.
A1c vs. Your Daily Glucose Readings
If you check your blood sugar at home, you can roughly translate your A1c into a daily average. A 6% A1c equals about 126 mg/dL. At 7%, it’s 154 mg/dL. At 8%, roughly 183 mg/dL. At 10%, you’re looking at an average around 240 mg/dL. These conversions help bridge the gap between the percentage your doctor reports and the numbers you see on your meter every day.
Keep in mind that A1c is an average. Two people can have identical A1c results with very different daily patterns. One might have stable blood sugar that hovers near the average, while the other swings between high spikes and low crashes. If your A1c looks fine but you’re experiencing symptoms, the day-to-day variability may be the issue rather than the average itself.
Factors That Can Skew Your Result
Certain conditions make A1c readings unreliable. Iron deficiency anemia, which is especially common in women and during pregnancy, can push your A1c reading higher than your actual blood sugar warrants. Treating the iron deficiency with supplements typically brings the number back down, even if glucose levels haven’t changed. Late pregnancy in non-diabetic women can produce the same false elevation for this reason.
Hemoglobin variants like sickle cell trait or disease also interfere with the test. People with these conditions often have faster red blood cell turnover, which shortens the window the test measures. Depending on the specific lab method used, results can read falsely high or falsely low. If you have a known hemoglobin disorder, your doctor may rely on alternative markers like fructosamine or continuous glucose monitoring instead.
How Often to Get Tested
If your A1c is in your target range, testing every six months is typical. If your result came back high, your medication changed, or you’re actively working to bring the number down, every three months gives you enough time to see the effect of changes without waiting too long. Since the test heavily weights the most recent month, a full three-month interval captures a meaningful new picture of your blood sugar control.
