A blood alcohol concentration (BAC) of 0.08% is the legal limit for driving in 49 U.S. states, and any level at or above that is broadly considered “high” in a legal sense. But medically, the picture is more layered: impairment begins well below 0.08%, and BAC levels above 0.15% enter dangerous territory where vomiting, blackouts, and loss of consciousness become likely. Above 0.30%, alcohol poisoning can be fatal.
BAC Levels and What They Feel Like
Blood alcohol concentration measures the grams of alcohol per deciliter of blood, expressed as a percentage. A BAC of 0.10% means there is 0.10 grams of alcohol in every 100 milliliters of your blood. Here’s what happens at each level:
- 0.02%: Slight mood changes, mild relaxation, and a small dip in judgment. Most people feel almost normal.
- 0.05%: Lowered alertness, reduced inhibition, and noticeably impaired judgment. Utah treats this as legally impaired for driving.
- 0.08%: The legal limit in every other U.S. state and Washington, D.C. Muscle coordination drops, detecting danger becomes harder, and reasoning suffers.
- 0.10%: Slurred speech, slowed thinking, and clearly delayed reaction times.
- 0.15%: Nausea, vomiting, loss of balance, and significant loss of muscle control. This is roughly the threshold where many states impose enhanced penalties for drunk driving.
- 0.15% to 0.30%: Confusion, drowsiness, and disorientation. Blackouts (gaps in memory where you’re conscious but not forming memories) become common.
- 0.30% to 0.40%: Alcohol poisoning is likely. Loss of consciousness occurs, and areas of the brain controlling breathing, heart rate, and body temperature begin shutting down. Death is a real possibility.
These ranges assume an average adult without significant tolerance. The numbers don’t shift for people who drink regularly, but the way they feel at each level can shift dramatically, which creates its own set of risks.
How Tolerance Distorts the Picture
Someone who drinks heavily over time may seem relatively functional at a BAC that would incapacitate a lighter drinker. They might walk steadily, speak clearly, and appear sober at 0.15% or higher. But tolerance only changes how drunk a person feels and looks. It does not improve reaction time or peripheral vision, the two abilities most critical for driving and avoiding danger.
This gap between feeling and function is what makes tolerance so dangerous. A person with high tolerance is less able to judge their own impairment because the internal cues they rely on (feeling dizzy, slurring words) no longer match their actual BAC. Their liver still clears alcohol at the same rate, and their risk of organ damage at high levels is unchanged.
What Counts as a Standard Drink
In the United States, one standard drink contains 0.6 ounces (14 grams) of pure alcohol. That works out to 12 ounces of regular beer at 5% alcohol, 5 ounces of wine at 12%, or 1.5 ounces of 80-proof liquor at 40%. Many real-world servings exceed these amounts. A craft IPA at 8% in a pint glass, for instance, is closer to two standard drinks. A generous restaurant wine pour can easily be 7 or 8 ounces.
This matters because official definitions of “high” drinking are all based on standard drink counts. If your drinks are larger or stronger than the standard, you’ll reach dangerous BAC levels faster than the numbers suggest.
Binge, Heavy, and High-Intensity Drinking
The NIAAA defines binge drinking as a pattern that brings BAC to 0.08% or above. For a typical adult, that means about five drinks in two hours for men and four for women. Binge drinking is the most common form of excessive drinking in the U.S., and it’s the pattern most directly tied to dangerously high BAC levels in a single session.
Heavy drinking looks at patterns over time: five or more drinks on any single day, or 15 or more per week for men. For women, it’s four or more on any day, or eight or more per week. You don’t need to get visibly drunk to meet this threshold; steady daily drinking at moderate-seeming levels can qualify.
High-intensity drinking is the most extreme category: consuming at least double the binge threshold in one session, meaning 10 or more drinks for men or 8 or more for women. The consequences scale sharply. People who drink at twice the binge threshold are 70 times more likely to end up in an emergency room for an alcohol-related reason compared to non-binge drinkers. At three times the threshold, that risk jumps to 93 times higher.
How Fast Your Body Clears Alcohol
Your liver processes roughly one standard drink per hour, and nothing speeds that up. Coffee, cold showers, and food won’t accelerate elimination once alcohol is in your bloodstream. Time is the only factor that brings BAC down.
Several things do affect how quickly BAC rises in the first place. Drinking on an empty stomach lets alcohol pass into the small intestine faster, where it absorbs rapidly. Fatty and high-protein foods slow this process by keeping the valve at the bottom of your stomach closed longer during digestion. Drinking speed matters too: gulping raises BAC faster than sipping the same total amount over a longer period.
Women generally reach higher BAC levels than men after the same number of drinks, even when body weight is similar. This is partly because women tend to have higher body fat and lower body water percentages, which concentrates alcohol in the bloodstream. Men also typically produce more of the enzyme that begins breaking down alcohol in the stomach before it ever reaches the blood.
Fatigue and stress amplify impairment at any BAC level. If you’re exhausted, the same number of drinks will affect you more severely than on a day when you’re rested.
When Alcohol Levels Become Life-Threatening
Alcohol overdose occurs when BAC climbs high enough to suppress the brain’s ability to maintain basic functions: breathing, heart rate, and temperature regulation. This can happen at BAC levels as low as 0.30%, though individual variation exists. The classic signs are loss of consciousness, slow or irregular breathing, vomiting while unconscious (which creates a choking risk), cold or bluish skin, and seizures.
A person doesn’t need to pass out for the situation to be serious. Someone who is conscious but extremely confused, cannot stand, or is vomiting repeatedly at an estimated BAC of 0.15% or above is already in a medically concerning range. BAC can also continue rising after a person stops drinking, because alcohol in the stomach and intestines is still being absorbed. A person who seems “just very drunk” can deteriorate quickly.
How Doctors Detect High Alcohol Use
Beyond a breathalyzer or blood test that captures BAC at a single moment, doctors can check for signs of sustained heavy drinking through several blood markers. A liver enzyme called GGT rises even in moderate drinkers, particularly men, and stays elevated for two to three weeks after heavy use. Red blood cell size (measured as MCV) increases with prolonged high alcohol intake, and this change takes two to four months to normalize, making it a useful window into longer-term patterns.
More specific markers exist as well. One test measures a protein in the blood that changes shape after roughly two weeks of consuming 40 or more grams of alcohol per day (about three standard drinks daily). Another detects a direct byproduct of alcohol metabolism that correlates closely with how much someone has been drinking over recent weeks. These tests are commonly used in treatment programs and legal monitoring to verify abstinence or detect relapse.
The Official Guidelines for “Moderate”
The current U.S. Dietary Guidelines define moderate drinking as two drinks or fewer per day for men and one drink or fewer per day for women. Anything above that is, by federal health standards, excessive. These limits exist because risk for liver disease, several cancers, heart problems, and alcohol use disorder rises meaningfully above these thresholds. Choosing not to drink at all is explicitly listed as an equally valid option in the guidelines.
