A blood alcohol concentration (BAC) of 0.15% or higher is generally considered high, and anything above 0.30% enters life-threatening territory. BAC measures the percentage of alcohol in your bloodstream: 0.08% is the legal driving limit in all 50 U.S. states, so “high” depends on whether you’re talking about legal risk, noticeable impairment, or medical danger.
How BAC Levels Map to Impairment
Alcohol’s effects follow a surprisingly predictable curve. At 0.02%, most people feel slightly relaxed with a mild mood shift, but their ability to track moving objects and divide attention between tasks already starts to decline. At 0.05%, judgment is noticeably impaired, alertness drops, and fine motor control (like focusing your eyes) becomes harder. These are levels many people would describe as “buzzed” rather than drunk, but the cognitive effects are already measurable.
At 0.08%, the legal limit for driving, muscle coordination deteriorates across the board: balance, speech, vision, reaction time, and hearing are all affected. Short-term memory weakens, and the ability to detect danger drops significantly. This is the level at which most people would recognize they’re impaired, though many overestimate how well they’re functioning.
At 0.10%, reaction time and motor control clearly deteriorate. Speech slurs, thinking slows, and drivers lose the ability to maintain lane position or brake properly. By 0.15%, the body starts showing signs of distress. Vomiting is common, balance is severely compromised, and muscle control is far below normal. The National Highway Traffic Safety Administration notes substantial impairment in vehicle control and the ability to process visual and auditory information at this level.
Where “High” Becomes Dangerous
Between 0.15% and 0.30%, you enter a range marked by confusion, drowsiness, and vomiting. Blackouts, where the brain stops forming new memories while you remain conscious, commonly occur in this range. A person at 0.20% or above may struggle to walk, respond to their surroundings, or stay awake.
Between 0.30% and 0.40%, alcohol poisoning is likely. Loss of consciousness occurs, and the body’s basic protective reflexes start shutting down. This means a person could choke on vomit without waking up, or their breathing could slow dangerously. Above 0.40%, the risk of coma and death from respiratory arrest is severe. The body simply cannot maintain breathing at these concentrations.
To put these numbers in perspective: a 160-pound man who drinks five beers in an hour might reach roughly 0.10%. Reaching 0.30% or higher typically requires consuming a very large amount of alcohol in a short period, which is why binge drinking and drinking games carry such outsized risk.
Why the Same Drinks Hit People Differently
Two people can drink the same amount and end up with very different BAC levels. Body weight plays a role, since alcohol distributes through body water, and larger bodies dilute it more. But biological sex matters in a way most people don’t realize. Women’s stomachs don’t produce an enzyme (active in men’s stomachs) that breaks down about 30% of alcohol before it ever reaches the bloodstream. This means a higher proportion of each drink enters a woman’s blood intact, producing a higher BAC from the same amount of alcohol.
Genetic variation also plays a major role. Roughly half of people of East Asian descent carry a gene variant that produces a nonfunctional version of a key enzyme involved in processing alcohol’s toxic byproduct, acetaldehyde. In people with this variant, acetaldehyde levels can spike to 20 times higher than normal, causing intense flushing, nausea, and rapid heartbeat even at modest BAC levels. This doesn’t mean their BAC is higher, but it means the same BAC produces far more severe effects.
Food in the stomach, hydration status, and how quickly you’re drinking all shift the curve as well. Drinking on an empty stomach allows alcohol to absorb much faster, producing a sharper BAC spike from the same number of drinks.
The Tolerance Trap
People who drink heavily and regularly often appear less impaired at a given BAC than occasional drinkers, which creates a dangerous false sense of security. Research from the University of Chicago found that people with alcohol use disorder showed less impairment than light drinkers on motor and cognitive tests at a BAC of 0.08% to 0.09%. But when those same experienced drinkers consumed amounts closer to their usual habits, reaching about 0.13%, their impairment more than doubled compared to the lower dose. Their physical impairment actually exceeded that of light drinkers at the standard intoxicating dose.
Critically, the heavy drinkers reported feeling less impaired than they actually were. Tolerance creates a gap between how drunk you feel and how drunk you are. The physical effects of alcohol accumulate regardless of experience, which is why high-tolerance drinkers are not protected from alcohol poisoning, organ damage, or impaired driving, even if they feel fine.
How Fast Your Body Clears Alcohol
The liver breaks down alcohol at a fixed rate of about 0.015% BAC per hour. This rate doesn’t change with coffee, food, cold showers, or exercise. It’s essentially one standard drink per hour for most people. That means if your BAC reaches 0.15%, it will take roughly 10 hours to return to zero. At 0.30%, you’re looking at about 20 hours.
This fixed rate is why a night of heavy drinking can leave you legally impaired well into the next morning. Someone who stops drinking at 2 a.m. with a BAC of 0.20% would still be above the legal driving limit at 10 a.m.
Legal Thresholds Beyond 0.08%
While 0.08% is the standard DUI threshold, many states impose escalating penalties at higher BAC levels. Several states classify a BAC of 0.15% or above as an “aggravated” or “extreme” DUI, carrying steeper fines, longer license suspensions, and mandatory jail time. Commercial drivers face a lower limit of 0.04%. Drivers under 21 can be charged with a BAC as low as 0.02% in most states, effectively a zero-tolerance policy.
It’s also worth noting that drivers can be convicted of DUI with a BAC below 0.08% if they show visible signs of impairment. The 0.08% threshold is a per se limit, meaning you’re considered impaired by law at that level regardless of how you appear, but it’s not a floor below which you’re legally safe.
