A fasting blood sugar level of 100 mg/dL or above is considered higher than normal, and 126 mg/dL or above on two separate tests meets the threshold for diabetes. But “high” means different things depending on when you last ate, whether you’re pregnant, your age, and whether you already have a diabetes diagnosis. Here’s how to make sense of the numbers.
Fasting Blood Sugar Ranges
A fasting blood sugar test measures your glucose after at least eight hours without food, typically first thing in the morning. The standard cutoffs break down into three categories:
- Normal: below 100 mg/dL (5.6 mmol/L)
- Prediabetes: 100 to 125 mg/dL (5.6 to 6.9 mmol/L)
- Diabetes: 126 mg/dL (7.0 mmol/L) or higher on two separate tests
A single reading of 126 mg/dL isn’t enough for a diagnosis on its own. Stress, illness, a poor night of sleep, and certain medications can all push a fasting reading temporarily above normal. That’s why the diabetes threshold requires confirmation with a second test.
Prediabetes is worth paying attention to even though the numbers don’t look dramatically different from normal. At this stage, your cells are already starting to resist insulin, the hormone that moves sugar out of your blood and into your muscles, fat, and liver for energy. Over time, your pancreas struggles to keep up with the extra demand, and glucose stays in the bloodstream longer than it should. Prediabetes is reversible with changes to diet and activity, which is why catching it matters.
Blood Sugar After Eating
Your blood sugar naturally rises after a meal, peaks around one to two hours later, and then drops back down. For people managing diabetes, the CDC recommends a reading below 180 mg/dL two hours after the start of a meal. In someone without diabetes, that post-meal peak is typically lower, often staying under 140 mg/dL.
If you’re checking your blood sugar at home, timing matters. A reading taken 30 minutes after eating will almost always look higher than one taken at the two-hour mark. Comparing numbers that were taken at different times after meals will give you a misleading picture of your control.
What A1c Tells You About Average Levels
While a finger-stick test captures a single moment, the A1c test reflects your average blood sugar over roughly three months. It measures the percentage of your red blood cells that have glucose attached to them. A normal A1c is below 5.7%, prediabetes falls between 5.7% and 6.4%, and diabetes is 6.5% or higher.
To put those percentages into daily numbers: an A1c of 7% translates to an estimated average glucose of about 154 mg/dL. At 8%, the average jumps to 183 mg/dL, and at 9% it reaches 212 mg/dL. These averages help explain why someone can have a “normal” fasting reading on a given morning but still have an elevated A1c. Post-meal spikes, overnight fluctuations, and day-to-day variability all feed into the three-month picture.
Targets During Pregnancy
Pregnancy tightens the definition of “high” considerably. Gestational diabetes targets are stricter than standard diabetes targets because even moderately elevated blood sugar can affect fetal development. The recommended ranges for pregnant women with gestational diabetes are:
- Fasting: below 95 mg/dL
- One hour after a meal: below 140 mg/dL
- Two hours after a meal: below 120 mg/dL
These post-meal readings are timed from the start of the meal, not when you finish eating. That distinction can make a real difference when you’re tracking numbers closely.
Adjusted Targets for Older Adults
For adults over 65, guidelines from the Joslin Diabetes Center loosen blood sugar targets based on overall health status. The logic is straightforward: aggressively lowering blood sugar in an older person with multiple health conditions increases the risk of dangerous lows (hypoglycemia), falls, and other complications that can be more harmful than running slightly higher.
A healthy older adult with few other medical conditions might aim for a fasting or post-meal glucose of 80 to 130 mg/dL and an A1c below 7.5%. Someone managing several chronic illnesses or mild cognitive decline has a wider target of 90 to 150 mg/dL and an A1c below 8%. For those in poor health or long-term care, the range extends to 100 to 180 mg/dL with an A1c goal under 8.5%.
These aren’t loose standards. They reflect the reality that blood sugar management always involves a tradeoff between preventing long-term complications and avoiding short-term danger from lows.
How High Blood Sugar Feels
Mildly elevated blood sugar often produces no symptoms at all, which is one reason diabetes goes undiagnosed for years in many people. As levels climb higher, the most common early signs include increased thirst, frequent urination, fatigue, and blurred vision. Your kidneys work harder to filter the extra glucose, pulling more water out of your body in the process, which explains the thirst and urination cycle.
More persistent or severe highs can cause slow-healing cuts, recurring infections, unexplained weight loss, and tingling or numbness in the hands and feet. These symptoms point to blood sugar that has been elevated for weeks or months, not just a single spike after a large meal.
When High Blood Sugar Becomes Dangerous
There’s a difference between “high” and “dangerously high.” A reading of 200 mg/dL after a big pasta dinner is concerning if it happens regularly, but it’s not an immediate emergency. The situation changes at higher thresholds.
Blood sugar that stays above 240 mg/dL, combined with signs of ketones in the urine, warrants urgent medical attention. At that level, the body may be breaking down fat for fuel because it can’t access glucose properly, producing acidic byproducts called ketones. This is the beginning of diabetic ketoacidosis (DKA), which is diagnosed when blood sugar exceeds 250 mg/dL along with changes in blood acidity. DKA develops most often in people with type 1 diabetes but can occur in type 2 as well. Symptoms include nausea, vomiting, abdominal pain, fruity-smelling breath, and confusion.
An even more extreme condition, hyperosmolar hyperglycemic state, typically involves blood sugar above 600 mg/dL. This occurs almost exclusively in type 2 diabetes, often in older adults, and can cause severe dehydration, confusion, and loss of consciousness. Both conditions are medical emergencies that require hospital treatment.
Common Causes of Elevated Readings
If you’re seeing higher-than-expected numbers, the explanation isn’t always food. Illness and infection raise blood sugar because your body releases stress hormones that increase glucose production in the liver. Physical inactivity reduces your cells’ ability to take in glucose efficiently. Poor sleep, even a single bad night, temporarily increases insulin resistance the next day. Certain medications, particularly steroids, can push readings up dramatically.
For people already managing diabetes, missed doses of medication, changes in routine, and emotional stress are frequent culprits. Dehydration can also concentrate glucose in the blood, making readings appear higher. Identifying the pattern behind your spikes, whether it’s a specific meal, a time of day, or a life circumstance, is often more useful than reacting to any single number.
