For adults, a high fever is generally considered 103°F (39.4°C) or above when measured orally. A temperature of 104°F (40°C) or higher warrants a call to your doctor, and anything above 106.7°F (41.5°C) is a medical emergency called hyperpyrexia that can damage organs.
Fever Thresholds for Adults
Normal body temperature isn’t a single number. It typically ranges from 97°F (36.1°C) to 99°F (37.2°C), varying by person, time of day, and activity level. The old benchmark of 98.6°F is just an average.
A fever officially starts at an oral temperature of 100.4°F (38°C). From there, the severity breaks down roughly like this:

Why Your Thermometer Choice Matters
The number on your thermometer depends on where you take the reading. Rectal and ear temperatures run about 0.5°F to 1°F higher than oral readings. Armpit and forehead scanner readings run about 0.5°F to 1°F lower than oral. So if your forehead thermometer shows 102°F, your actual core temperature could be closer to 103°F.
Electronic forehead thermometers are roughly as accurate as oral probe thermometers. Ear thermometers are convenient but can give less consistent results. Plastic strip thermometers that you press against the forehead measure skin temperature, not core body temperature, and are the least accurate option.
Fever Works Differently in Older Adults
Adults over 65 tend to run lower baseline temperatures, which means a reading that looks unremarkable could actually signal a serious infection. Research from the University of Maryland found that lowering the fever cutoff from 100.4°F to 99°F (37.2°C) for older adults more than doubled the detection rate for bacterial infections, catching 83% compared to just 40% with the standard threshold. If you’re caring for an older adult, a temperature of 99°F with other symptoms like confusion or fatigue deserves attention, even if it doesn’t look like a “real” fever on paper.
What Happens in Your Body During a Fever
Fever isn’t a malfunction. It’s your immune system deliberately raising your internal thermostat. When your body detects an infection, immune cells release signaling molecules that travel to the brain’s temperature control center. These signals trigger a chemical chain reaction that resets your body’s target temperature higher, much like turning up a thermostat in your house.
Your body then works to reach that new set point. Blood vessels near the skin constrict to trap heat, sweating slows down, and your metabolic rate increases to generate warmth. That’s why you shiver and feel cold at the start of a fever, even though your temperature is actually rising. The higher temperature helps your immune cells work more effectively and makes the environment less hospitable to many viruses and bacteria.
Managing a Fever at Home
Most fevers under 103°F in otherwise healthy adults don’t need aggressive treatment. The fever itself is doing useful work. But if you’re miserable, over-the-counter options like acetaminophen or ibuprofen can bring your temperature down and ease aches. If you’re using acetaminophen, stay under 4,000 milligrams (4 grams) in a 24-hour period to protect your liver. Staying hydrated matters more than most people realize, since fever increases fluid loss through sweating and faster breathing.
Don’t bundle up in heavy blankets if you’re already hot. Lukewarm (not cold) compresses can help. Ice baths and alcohol rubs are outdated approaches that can cause shivering, which actually raises your core temperature further.
Symptoms That Matter More Than the Number
Temperature alone doesn’t tell the whole story. A fever of 101°F with a stiff neck and confusion is far more concerning than a fever of 103°F with typical flu symptoms. Regardless of how high your temperature reads, certain warning signs point to something potentially serious:
- Confusion or altered consciousness: Could indicate the infection is affecting the brain.
- Stiff neck with headache: A classic sign of meningitis.
- Trouble breathing: May signal pneumonia or another respiratory complication.
- Seizure: Requires immediate emergency care.
- Severe pain anywhere in the body: Especially if sudden or localized.
- Painful urination or foul-smelling urine: Suggests a urinary tract infection that may be spreading.
- Swelling or inflammation: Could point to an abscess or deep tissue infection.
A fever that persists for more than three days, even at a low grade, also warrants medical attention. And any fever in someone with a weakened immune system, whether from chemotherapy, HIV, or immunosuppressive medications, should be evaluated promptly since their body may not mount the same obvious response to dangerous infections.
