A high reliability organization (HRO) is one that operates in conditions where disasters could easily happen, yet consistently avoids them. Think nuclear power plants, aircraft carriers, and air traffic control systems. These aren’t organizations that never make mistakes. They’re organizations that have developed habits of thinking and working that catch small problems before they become catastrophic ones.
The concept grew out of research that began in the mid-1980s at the University of California, Berkeley, where researchers Todd La Porte, Karlene Roberts, and Gene Rochlin studied three complex operations: US Navy Nimitz-class aircraft carriers, the FAA’s air traffic control system, and PG&E’s Diablo Canyon nuclear power plant. Later joined by Karl Weick, the team wasn’t studying failures. They were studying organizations that had every reason to fail but didn’t, trying to figure out what made them different.
The Five Principles That Define HROs
Weick and Sutcliffe distilled their findings into five hallmark characteristics. These aren’t policies written in a handbook. They’re patterns of thinking that run through every level of the organization, sometimes described as “collective mindfulness.” Three of the five help the organization spot trouble early. The other two help it respond when trouble arrives anyway.
Preoccupation With Failure
HROs treat every small slip, near miss, or unexpected outcome as a signal worth investigating. Where a typical organization might shrug off a minor glitch because nothing bad actually happened, an HRO sees that glitch as a window into a deeper vulnerability. Reporting these weak signals is encouraged and rewarded rather than punished. Staff are trained to actively look for hazards, and they work proactively to prevent mistakes rather than reacting after harm occurs. Past success doesn’t breed complacency. If anything, a long streak without incidents makes people in an HRO more suspicious, not more relaxed.
Reluctance to Simplify
When something goes wrong, the instinct in most workplaces is to find a quick explanation and move on. HROs resist that instinct. They understand that their systems are deeply interconnected, and a simple explanation for a problem often masks a more complex, systemic issue. Instead of settling for “human error” as a root cause, they dig into the conditions, processes, and system design that allowed the error to happen in the first place.
Sensitivity to Operations
This principle is about maintaining a real-time, ground-level awareness of what’s actually happening on the front lines. Leaders in an HRO don’t rely solely on reports, dashboards, or scheduled reviews. They pay attention to the messy, moment-to-moment reality of daily work. When the picture on paper doesn’t match what people on the floor are experiencing, that gap itself becomes a concern.
Commitment to Resilience
No system is perfectly designed, and HROs accept that errors will eventually slip through. The difference is in what happens next. Resilient organizations build the capacity to detect problems quickly once they emerge, contain the damage, and bounce back to normal functioning. They rehearse responses, cross-train staff, and develop flexible protocols so that when the unexpected happens, people aren’t frozen by surprise.
Deference to Expertise
In a crisis, decision-making authority in an HRO flows to whoever has the most relevant expertise, regardless of their rank or title. A junior technician who understands the specific system that’s failing may take the lead over a senior executive. This is a deliberate cultural choice. One real-world example: when a large healthcare system needed to address medication safety problems, it assembled a task force that pulled experts from pharmacy, nursing, human factors engineering, quality and safety teams, and clinical informatics, rather than assigning the problem to whoever held the highest administrative title.
Why Healthcare Adopted the Model
For decades, HRO thinking lived primarily in aviation, nuclear energy, and the military. Healthcare began embracing it after the field recognized that hospitals, like aircraft carriers, operate in high-stakes environments where small errors can cascade into serious harm. The Joint Commission, which accredits hospitals in the United States, now promotes a framework organized around three pillars: leadership committed to a goal of zero patient harm, a safety culture where any staff member can speak up about risks without fear, and an empowered workforce equipped with practical improvement tools to fix the problems they find.
The appeal is straightforward. A hospital unit shares key features with a nuclear control room: complex technology, time pressure, high stakes, and work performed by teams of people who must coordinate precisely. HRO principles offer a way to think about safety that goes beyond checklists and blame.
What Makes Implementation Difficult
Adopting HRO principles is genuinely hard, and the research is honest about that. One study of healthcare executives found several persistent challenges: uncertainty about how to sequence safety initiatives, a lack of benchmarking data to measure progress, and a pressing need for standardized information technology. Most initiatives were new enough that leaders weren’t yet confident whether their efforts could be sustained over time.
A deeper issue is the gap between knowing what to do and knowing how to do it. Healthcare literature has historically focused on evidence-based practices (the “what”) with much less attention to implementation practices (the “how”). You can tell an organization to defer to expertise during a crisis, but actually building a culture where a nurse feels safe overriding a physician’s decision in real time requires years of trust-building, structural change, and reinforcement.
Hierarchy is one of the biggest obstacles. HRO principles, especially deference to expertise and preoccupation with failure, require flattening traditional power structures. In industries like the military, this works because it’s been drilled into the operational culture for decades. In a hospital or a corporation with rigid chains of command, shifting that dynamic takes sustained effort from leadership.
HRO as a Mindset, Not a Checklist
One common misconception is that becoming an HRO means following a specific roadmap. Researchers have pushed back on this directly: high reliability organizing is not a prescription or a step-by-step formula. It’s better understood as a collective mindset, a state where everyone in the organization, from the newest hire to the CEO, structures their actions around shared safety goals.
This mindset shows up in small, daily behaviors. It’s the maintenance worker who reports a valve reading that seems slightly off even though it’s technically within normal range. It’s the team leader who asks a quiet member of the group for their take before making a decision. It’s the organization that investigates a near miss with the same rigor it would bring to an actual disaster. None of these behaviors require special technology or massive budgets. They require a culture that values vigilance over efficiency and learning over blame, and leadership willing to protect that culture even when it’s inconvenient.
