What Is a High Temperature for an Adult?

For adults, a temperature of 100.4°F (38°C) or higher is considered a fever. That’s the widely accepted medical threshold, regardless of where you’ve seen other numbers floating around. But not all fevers carry the same level of concern. A reading of 100.5°F is very different from 104°F, and knowing where your temperature falls on that spectrum helps you decide what to do next.

The Fever Threshold and Why It’s Not 98.6°F

Normal body temperature isn’t a single fixed number. Your internal thermostat, a small region in the brain called the hypothalamus, keeps your core temperature in a range roughly between 97°F and 99°F throughout the day. It dips lower in the morning and rises in the late afternoon. So 99°F at 5 p.m. is perfectly normal, while the same reading at 6 a.m. might be slightly elevated for you.

The 100.4°F cutoff is the point where your body has clearly shifted above its normal operating range. Some sources use a slightly lower oral threshold of 100°F (37.8°C), which reflects the fact that oral readings tend to run a bit cooler than rectal ones. Either way, once you cross into that territory, something is driving your temperature up.

Fever Severity Levels

Not every fever needs the same response. Here’s how the numbers break down in practical terms:

  • Low-grade fever (100.4°F to 102°F / 38°C to 38.9°C): The most common range during routine infections like colds and flu. Rest and fluids are typically all you need. Medication isn’t necessary unless you’re uncomfortable.
  • Moderate fever (102°F to 104°F / 38.9°C to 40°C): Worth paying closer attention to. Over-the-counter pain relievers like acetaminophen or ibuprofen can help bring your temperature down and ease discomfort. Stay hydrated.
  • High fever (above 104°F / 40°C): Call your doctor. A temperature this high signals a more serious infection or inflammatory process and may need medical evaluation.
  • Hyperpyrexia (above 106.7°F / 41.5°C): A medical emergency. At this level, organs start to malfunction. The brain, heart, kidneys, and liver can all sustain damage, and the risk of brain swelling, permanent neurological injury, or coma is real. This requires immediate emergency care.

Why Your Body Raises Its Temperature

A fever isn’t a malfunction. It’s a deliberate defense mechanism. When your immune system detects an invading virus or bacteria, it releases chemical signals that travel to the hypothalamus and essentially turn up the thermostat. Your body then works to reach that new, higher set point: blood vessels near the skin constrict to trap heat, and sometimes your muscles start shivering to generate more warmth. That’s why you can feel freezing cold even though your temperature is climbing.

The elevated temperature makes your body a less hospitable environment for many pathogens while also speeding up certain immune responses. This is why mild fevers often don’t need to be treated aggressively. Your body is doing something useful. The concern starts when the temperature climbs high enough to cause more harm than the infection itself.

Thermometer Placement Matters

The number on your thermometer depends partly on where you take the reading. Rectal measurements are the most accurate reflection of core body temperature. Oral readings run slightly lower, and armpit (axillary) readings tend to be lower still. Forehead and ear thermometers fall somewhere in between and can vary based on technique.

There’s no reliable formula for converting between sites. Adding or subtracting a degree to “adjust” a forehead reading to match an oral one doesn’t hold up consistently. The better approach is to use the same method each time so you can track whether your temperature is trending up or down. If you’re using an oral thermometer, the 100.4°F threshold applies directly. If you’re using a different method, keep in mind your reading may be slightly off from that benchmark.

Managing a Fever at Home

For fevers up to about 102°F, you likely don’t need medication at all. Rest, drink plenty of fluids, and let your body do its work. Water, broth, and electrolyte drinks all help replace the fluid you lose through sweating.

Once your temperature pushes above 102°F or you’re feeling genuinely miserable, acetaminophen, ibuprofen, or aspirin can bring your temperature down and relieve aches. Follow the dosing instructions on the package carefully. One common mistake: taking a combination cold medicine that already contains acetaminophen and then taking a separate dose of acetaminophen on top of it. Check ingredient labels to avoid doubling up.

Light clothing and a comfortable room temperature help more than piling on blankets, even if you feel chilled. Avoid ice baths or alcohol rubs, which can cause shivering and actually raise your core temperature further.

Red Flags That Need Immediate Attention

A fever by itself, even one above 103°F, isn’t always an emergency. But certain symptoms alongside a fever point to something more serious. Seek immediate medical care if a fever comes with any of the following:

  • Stiff neck, especially with pain when bending your head forward
  • Confusion, unusual behavior, or difficulty speaking
  • Seizures or convulsions
  • Difficulty breathing or chest pain
  • Severe headache or unusual sensitivity to bright light
  • Persistent vomiting
  • A new or unexplained rash
  • Painful urination or foul-smelling urine
  • Severe pain anywhere in the body

These combinations can indicate meningitis, sepsis, or other conditions where hours matter. A fever lasting more than three days without improvement also warrants a call to your doctor, even if none of these red flags are present.
