A blood oxygen level below 95% is generally considered low. For most people, normal oxygen saturation falls between 95% and 100% when measured with a pulse oximeter, the small clip-on device placed on your fingertip. A reading of 92% or lower warrants a call to your healthcare provider, and 88% or lower is a medical emergency.
How Blood Oxygen Is Measured
There are two main ways to check blood oxygen. The most common is pulse oximetry, where a small device clips onto your finger and shines light through your skin to estimate how much oxygen your red blood cells are carrying. It gives you a percentage called SpO2. This method is painless, takes seconds, and is the same technology used in hospitals, doctor’s offices, and home monitoring.
The more precise method is an arterial blood gas (ABG) test. Instead of drawing blood from a vein like most blood tests, a provider takes a sample from an artery, usually on the inside of your wrist. Arterial blood carries freshly oxygenated blood from your lungs, so it gives a more accurate picture. The test measures not just oxygen saturation but also the pressure of dissolved oxygen in your blood, carbon dioxide levels, and blood acidity. Because drawing from an artery is more uncomfortable than a standard blood draw, the provider may numb the area first and will apply pressure to the site for at least five minutes afterward to stop bleeding. ABG testing is typically reserved for hospital settings when providers need a detailed look at lung function.
What the Numbers Mean
For a healthy person at sea level, a pulse oximeter reading of 95% to 100% is normal. This means at least 95 out of every 100 red blood cells passing through your lungs are picking up oxygen. Below that range, your body starts to struggle.
- 91% to 94%: Mildly low. You may not feel obvious symptoms yet, but your body is getting less oxygen than it needs. Contact your provider if readings stay in this range.
- 88% to 91%: Moderately low. Symptoms like shortness of breath, confusion, and rapid heart rate become more likely. Medical attention is needed.
- Below 88%: Dangerously low. Get to an emergency room. Organ damage can occur without intervention.
These thresholds shift for people with chronic lung conditions. For someone with COPD, European and British guidelines recommend a target oxygen saturation of 88% to 92%. That range exists because giving too much supplemental oxygen to people with certain lung diseases can actually cause carbon dioxide to build up to harmful levels. If you have COPD, your provider will tell you what range is appropriate for you.
How Altitude Changes the Baseline
Oxygen saturation drops predictably as elevation increases, because the air itself contains less oxygen at higher altitudes. At sea level, a healthy person typically reads 98% to 99%. At around 2,500 meters (about 8,200 feet), the median drops to 96%. By 3,600 meters (roughly 11,800 feet), it falls to about 92%. At extreme elevations above 5,000 meters (16,400 feet), readings in the low 80s are common among acclimatized residents.
This means a reading of 92% that would prompt a call to your doctor at sea level could be perfectly normal for someone living in a high-altitude city. The variability in readings also increases at altitude, so two healthy people standing side by side on a mountain may show quite different numbers.
Symptoms of Low Blood Oxygen
The earliest signs are often a headache and shortness of breath. As oxygen drops further, you may notice a fast heartbeat, coughing, wheezing, and growing fatigue. Confusion is common at lower levels, which can make the situation dangerous because it impairs your ability to recognize that something is wrong.
One visible sign is cyanosis, a bluish or grayish tint to the skin, fingernails, or lips. On darker skin tones, this color change may be easier to spot in the nail beds, gums, or around the lips rather than on the skin surface. These symptoms can come on suddenly (in cases like a blood clot in the lung or a severe asthma attack) or develop gradually over weeks or months with progressive lung disease.
Common Causes
Most cases of low blood oxygen trace back to a problem with the lungs, the heart, or the blood itself. Lung-related causes are the most common and include pneumonia, COPD, asthma flare-ups, pulmonary fibrosis (scarring of lung tissue), and pulmonary embolism (a blood clot blocking blood flow in the lung). A collapsed lung and fluid buildup in the lungs can also drop oxygen levels sharply.
Heart conditions play a role too, particularly congenital heart defects that allow oxygen-poor and oxygen-rich blood to mix. Anemia, where you don’t have enough healthy red blood cells to carry oxygen, can reduce oxygen delivery to tissues even if your lungs are working fine.
Sleep apnea causes repeated drops in oxygen during the night as breathing stops and starts. Many people with sleep apnea have no idea their oxygen is dipping until they’re tested. Carbon monoxide poisoning is another cause, and it’s especially tricky because standard pulse oximeters can’t distinguish carbon monoxide bound to red blood cells from oxygen, so readings may appear falsely normal. Certain medications, particularly opioid pain relievers, can slow breathing enough to reduce oxygen intake.
When Pulse Oximeters Get It Wrong
Pulse oximeters are useful screening tools, but they have known limitations. Skin pigmentation and melanin affect how light passes through the fingertip, and it has been recognized for decades that this can lead to inaccurate readings. Specifically, oximeters tend to overestimate oxygen levels in people with darker skin, meaning the device may display a reassuring number while actual oxygen is lower than shown.
Poor circulation also throws off readings. Cold fingers, low blood pressure, or nail polish can reduce the signal the device relies on. If you’re monitoring at home and getting inconsistent numbers, try warming your hands first and removing any nail polish. Use the device on your index or middle finger, and keep your hand still during the reading. If symptoms don’t match a normal-looking number, trust your symptoms and seek care.
