What Is a Normal Blood Pressure Range for Adults?

Normal blood pressure is a reading below 120/80 mm Hg. That means the top number (systolic) is under 120 and the bottom number (diastolic) is under 80. Once either number crosses those thresholds, your blood pressure falls into a higher category, each carrying progressively more risk to your heart, brain, kidneys, and blood vessels.

What the Two Numbers Mean

A blood pressure reading has two parts. The top number, systolic pressure, measures the force your blood exerts against artery walls each time your heart beats and pushes blood out. The bottom number, diastolic pressure, measures that same force between beats, when the heart is resting and refilling. Both numbers matter, and if they fall into different categories, the higher category is the one that applies to you.

Blood Pressure Categories for Adults

The 2025 guidelines from the American Heart Association and American College of Cardiology define four categories:

  • Normal: below 120 systolic and below 80 diastolic
  • Elevated: 120 to 129 systolic and below 80 diastolic
  • Hypertension Stage 1: 130 to 139 systolic or 80 to 89 diastolic
  • Hypertension Stage 2: 140 or higher systolic or 90 or higher diastolic

These thresholds apply to all adults regardless of age. Older guidelines used to set a more lenient cutoff of 150/80 for people over 65, but that distinction was dropped in 2017 after large clinical trials showed that tighter control benefits older and younger adults alike.

European guidelines draw the lines slightly differently. The 2024 European Society of Cardiology guidelines classify anything below 120/70 as “nonelevated,” 120 to 139 over 70 to 89 as “elevated,” and 140/90 or above as hypertension. The ESC intentionally avoids calling any blood pressure level “normal” or “optimal” because cardiovascular risk still rises with each incremental increase, even well below 120/70. The practical takeaway: lower is generally better, as long as you feel fine.

Why Blood Pressure Fluctuates Throughout the Day

Your blood pressure is not a fixed number. It follows a natural 24-hour rhythm, running higher during the day when you’re active and dropping at night while you sleep. This nighttime dip is a healthy pattern. Morning readings tend to be somewhat higher as your body ramps up for the day. Physical activity, stress, caffeine, a full bladder, and even a conversation can all push your numbers up temporarily.

Because of these fluctuations, a single reading in a doctor’s office is a snapshot, not the full picture. That’s one reason clinicians typically want multiple readings taken on separate occasions before making any diagnosis.

How to Get an Accurate Reading

Small details during measurement can swing your reading by 10 to 20 points, enough to shift you from one category to another. To get a reliable number at home or in a clinic, follow a few key steps. Sit in a chair with your back supported for at least five minutes before the reading. Keep both feet flat on the floor with your legs uncrossed. Rest your arm on a table so the cuff sits at chest height.

Cuff size also matters more than most people realize. A cuff that’s too small will give a falsely high reading, while one that’s too large reads falsely low. Standard adult cuffs fit arm circumferences between about 27 and 34 centimeters. If your upper arm is larger than that, you need a large or extra-large cuff. Most home monitors come with a standard cuff, so check the fit before trusting your numbers.

When Your Reading Doesn’t Match Reality

About 14% of people have what’s called white coat hypertension: their blood pressure reads high in a medical setting but is normal at home. The stress of being in a clinic is enough to push their numbers up. Among people who show elevated readings at the doctor’s office, this phenomenon accounts for as many as 43% of cases, making it surprisingly common.

The opposite problem, masked hypertension, is arguably more dangerous. About 10% of people show normal readings at the office but have elevated pressure in daily life. This is especially common among people already taking blood pressure medication. Roughly 41% of treated patients whose numbers look good at the clinic actually have higher readings at home. Masked hypertension carries real cardiovascular risk because it flies under the radar. Home monitoring with a validated device is the simplest way to catch it.

What Happens When Blood Pressure Stays High

Chronically elevated blood pressure damages organs silently, often for years before symptoms appear. The organs most affected are the heart, kidneys, brain, eyes, and blood vessels themselves. The heart muscle thickens as it works harder to pump against stiffer arteries. The kidneys gradually lose filtering ability. Small blood vessels in the brain can develop blockages or tiny bleeds that impair cognition over time. Arteries stiffen and narrow, which in turn pushes blood pressure even higher in a self-reinforcing cycle.

The risk isn’t hypothetical. Research tracking thousands of people found that having even one sign of organ damage from high blood pressure doubles or triples the risk of cardiovascular disease compared to people with the same blood pressure but no organ changes. And organ damage tends to cluster: among those with elevated blood pressure, between half and three-quarters already had at least one form of damage, and a third to a half had two or more types. This is why the emphasis is on catching high blood pressure early, before these changes become entrenched.

When Blood Pressure Becomes an Emergency

A reading of 180/120 or higher is considered a hypertensive crisis. If that number comes with symptoms like chest pain, shortness of breath, severe headache, blurred vision, confusion, or nausea, it requires emergency medical attention. Without symptoms, a reading that high still warrants contacting a healthcare provider promptly, but the immediate danger is lower. The distinction matters because organ damage during a true crisis can progress within minutes.
