A parasitic infection happens when an organism lives on or inside your body, feeding off you to survive. These infections are far more common than most people realize. An estimated 3.5 billion people worldwide are affected by intestinal parasites alone, and roughly 450 million become sick from them. Parasites range from single-celled organisms invisible to the naked eye to worms that can grow several feet long.
Three Types of Parasites That Infect Humans
Parasites that cause disease in humans fall into three broad categories: protozoa, helminths, and ectoparasites. Each behaves differently in the body, and understanding the distinction helps explain why symptoms and treatments vary so much from one infection to another.
Protozoa are microscopic, single-celled organisms. What makes them particularly dangerous is their ability to multiply inside your body. A single organism can reproduce enough to cause a serious infection. Malaria, which causes an estimated 249 million cases and over 600,000 deaths each year, is caused by a protozoan. So are giardia (a common waterborne gut infection) and toxoplasma, which infects up to one-third of the world’s population, though most carriers never develop symptoms.
Helminths are worms, and unlike protozoa, their adult forms are often visible to the naked eye. They cannot multiply inside the human body, so the severity of infection depends on how many worms you’re exposed to. The three main groups are flatworms (including tapeworms and flukes), roundworms, and thorny-headed worms. Roundworms are the most widespread: about 1.5 billion people, nearly a quarter of the global population, are infected with soil-transmitted helminths like roundworms, whipworms, and hookworms.
Ectoparasites live on the outside of your body rather than inside it. Ticks, fleas, lice, and mites all fall into this category. They attach to or burrow into the skin and can remain there for weeks or months. Beyond the direct irritation they cause, ectoparasites can also transmit other infections into your bloodstream.
How Parasites Get Into Your Body
The most common route for intestinal parasites is the fecal-oral pathway. This sounds alarming, but in practice it means swallowing contaminated water, eating unwashed produce, or touching contaminated surfaces and then your mouth. In areas with poor sanitation, this cycle is difficult to break. Parasites shed in feces contaminate water sources, soil, and food, restarting the chain of infection.
Insect bites are the second major transmission route. Mosquitoes spread malaria and other protozoan infections. Sand flies transmit leishmaniasis, which causes 700,000 to 1 million new cases annually. Ticks can deliver parasites directly into the bloodstream while feeding.
Animals are a significant source as well. You can pick up hookworms, tapeworms, or roundworms from pets and livestock through contact with their feces or contaminated environments. Cats, for example, shed the parasite that causes toxoplasmosis in their litter. Raccoons carry a roundworm that can infect humans who accidentally ingest eggs from contaminated soil. Eating undercooked pork or beef is another well-known route for tapeworm and trichinella infections.
Symptoms to Recognize
Parasitic infections most commonly affect the gut, so the classic symptoms are diarrhea, cramping, gas, nausea, and vomiting. These can start anywhere from a few days to several weeks after exposure. Giardia, for instance, typically takes one to four weeks from exposure to the onset of symptoms.
But parasites don’t always stay in the digestive tract. Depending on the type, they can migrate to other organs and cause a wider range of problems. Fever and fatigue are common across many parasitic infections. Some cause itchy skin rashes or visible lumps where a worm has burrowed. Others can reach the lungs, causing coughing or breathing difficulty, or the brain, leading to neurological symptoms like confusion or seizures. Malaria is known for its cyclical fevers and chills, while hookworms can cause iron-deficiency anemia by feeding on blood in the intestinal wall.
Many infections produce mild or no symptoms at all, especially in otherwise healthy adults. This is part of what makes parasites so successful: they can persist in a host for months or years without triggering an obvious alarm.
Long-Term Risks of Untreated Infections
Even infections that seem mild at first can cause lasting damage if left untreated. Chronic parasitic infections in children are linked to stunted growth, malnutrition, and impaired cognitive development. Because helminths and protozoa can interfere with nutrient absorption in the gut, children in heavily affected regions often fall behind in both physical and intellectual milestones.
Research on giardia infections illustrates how far-reaching the consequences can be. Even after the parasite is cleared, a subset of people develop chronic conditions including irritable bowel syndrome, chronic fatigue, joint pain, and fibromyalgia that can persist for months or years. The infection disrupts the intestinal lining, increasing its permeability and depleting its protective mucus layer. This barrier damage is believed to trigger some of these long-term complications. Notably, even people who had no symptoms during the active infection can develop these problems later.
How Parasitic Infections Are Diagnosed
The most common starting point is a stool test, formally called an ova and parasite exam. A lab technician examines stool samples under a microscope looking for eggs or actual organisms. Because parasites shed eggs intermittently, the CDC recommends collecting three or more samples on separate days to improve accuracy. A single negative test doesn’t necessarily mean you’re clear.
When stool tests come back negative but symptoms persist, a doctor may use endoscopy or colonoscopy to visually examine the intestinal lining. This involves inserting a thin, flexible tube through the mouth or rectum to look for parasites or tissue damage directly.
Blood tests play a role too, particularly for parasites that don’t live in the gut. Serology tests detect antibodies your immune system produces in response to a parasite, or antigens from the parasite itself. These are especially useful for diagnosing infections like malaria, toxoplasmosis, or leishmaniasis where the organisms circulate in the blood or hide in tissues.
How Parasitic Infections Are Treated
Treatment depends entirely on what type of parasite is involved. Antiprotozoal medications target single-celled parasites by disrupting their ability to reproduce or survive inside your cells. Anthelmintic drugs work against worms through different mechanisms: some paralyze the worm so your body can expel it naturally, while others interfere with the worm’s ability to absorb nutrients or maintain its cell structure, essentially starving it.
For many common infections like giardia or pinworms, treatment is straightforward and involves a short course of oral medication. More invasive infections, particularly those that have spread beyond the gut to organs like the liver or brain, can require longer and more intensive treatment. Ectoparasites like lice and scabies mites are typically treated with topical creams or lotions applied directly to the skin.
Practical Steps to Prevent Infection
Most parasitic infections are preventable with basic hygiene and food safety habits. Wash your hands with soap and water before eating, after using the bathroom, after changing diapers, and after handling animals. This single habit breaks the fecal-oral transmission cycle that spreads the majority of intestinal parasites.
Water safety matters significantly. Drink only from treated municipal water supplies. When camping, hiking, or traveling in areas where the water quality is uncertain, boil water for at least one minute before drinking it. Bottled beverages are a safe alternative. Avoid swallowing water while swimming in lakes, rivers, or even community pools.
With food, cook meat to safe internal temperatures: 145°F for whole cuts of beef, pork, and lamb (with a three-minute rest before eating), 160°F for ground meat, and 165°F for poultry. Wash, peel, or cook raw fruits and vegetables before eating them. Drink only pasteurized milk, juice, and cider.
If you have cats, clean litter boxes daily. Toxoplasma eggs in cat feces need more than a day to become infectious, so prompt cleaning reduces the risk substantially. Wear gloves when gardening or handling sandboxes, since outdoor cats often use these as litter areas. Keeping cats indoors and feeding them commercial food rather than letting them hunt also reduces the chance they’ll pick up the parasite in the first place.
