A PN nurse, short for practical nurse, is a healthcare professional trained to provide basic medical care under the supervision of registered nurses (RNs) and doctors. After completing about 12 months of education and passing a licensing exam, practical nurses earn the title Licensed Practical Nurse (LPN) in most states, or Licensed Vocational Nurse (LVN) in Texas and California. The roles are identical; only the name differs by state.

What a Practical Nurse Does

Practical nurses handle a wide range of hands-on patient care tasks. Their core responsibilities include monitoring vital signs like blood pressure and heart rate, changing bandages and wound dressings, inserting catheters, feeding patients, and helping with basic hygiene and comfort. In many states, PNs also administer medications, including oral and injectable drugs.

The exact scope of practice varies by state. Some states allow practical nurses to start IV fluid drips and give IV medications, while others restrict those tasks to RNs. This means a PN working in one state may have a noticeably different day-to-day job than one working across a state line. Your state’s Board of Nursing sets these boundaries and handles licensing, disciplinary actions, and continuing education requirements.

How PNs Differ From Registered Nurses

The biggest distinction is autonomy. Practical nurses work under the direction of an RN or physician and are not authorized to manage nursing care independently. RNs perform comprehensive patient assessments, formulate nursing diagnoses, and decide the extent and frequency of monitoring a patient needs. PNs participate in ongoing assessment by collecting data using structured guidelines and recognizing when a patient needs immediate attention, but they don’t design the overall care plan.

Delegation is another key difference. RNs assign tasks to other nursing staff and are accountable for the care their team delivers. That supervisory role falls outside the PN scope of practice in most states. In practical terms, this means a PN carries out the plan that an RN or doctor has put in place rather than creating or modifying it.

Education and Licensing

Becoming a practical nurse is one of the fastest paths into healthcare. PN training programs, typically offered through community colleges and technical schools, run about 12 months. The curriculum combines classroom instruction in subjects like anatomy, pharmacology, and nutrition with supervised clinical rotations where students practice skills on real patients.

After graduating, you must pass the NCLEX-PN, a national licensing exam, before you can practice. Every state requires this exam, and your license is issued by your state’s Board of Nursing. Most states also require continuing education credits to keep your license active.

Where Practical Nurses Work

Practical nurses are employed across nearly every healthcare setting. Nursing homes and long-term care facilities are among the largest employers, where PNs monitor residents’ health, administer daily medications, and coordinate with physicians on changes in condition. Hospitals, outpatient clinics, home health agencies, and physicians’ offices also employ large numbers of PNs. In home health settings, a PN may be the primary caregiver a patient sees each day, handling wound care, vital sign checks, and medication management during scheduled visits.

Salary and Job Outlook

The median annual wage for licensed practical and vocational nurses was $54,620 as of May 2022, according to the Bureau of Labor Statistics. The mean (average) salary was slightly higher at $55,860. Pay varies significantly by state, employer type, and experience level. PNs working in hospitals or metropolitan areas generally earn more than those in rural long-term care settings.

Demand for practical nurses remains steady. An aging population and the ongoing need for long-term care keep this occupation among the more reliable entry points in healthcare, with tens of thousands of job openings projected each year from retirements and workforce growth.

Advancing From PN to RN

Many practical nurses use the role as a stepping stone toward becoming a registered nurse. LPN-to-RN bridge programs allow you to build on your existing training rather than starting from scratch. These programs, which award an Associate Degree in Nursing (ADN), typically require proof of an active PN license, official transcripts, and an entrance assessment such as the TEAS exam.

Bridge programs are shorter than traditional RN programs because they give credit for the clinical knowledge you already have. However, most nursing schools will not transfer credits for undergraduate nursing courses from other institutions, so the bulk of your advanced coursework will be completed within the bridge program itself. Completing the ADN qualifies you to sit for the NCLEX-RN exam, which opens the door to a significantly broader scope of practice, higher pay, and supervisory responsibilities.
