A research fellow is a researcher who holds an advanced degree and works in a time-limited position focused on conducting original research, typically under the guidance of a senior mentor. The role exists across universities, government agencies, hospitals, and pharmaceutical companies, but the specifics of what a research fellow actually does, earns, and works toward vary significantly depending on the setting.

Where Fellows Fit in the Academic Hierarchy

A research fellow sits above a graduate student but below a permanent faculty member. The position is almost always temporary, designed as a training period where someone with a doctoral degree (PhD, MD, or equivalent) deepens their expertise before moving into an independent role. Think of it as the bridge between finishing your degree and landing a permanent position.

At the National Institutes of Health, one of the largest research employers in the U.S., the hierarchy illustrates how granular these distinctions can get. Entry-level positions include Intramural Research Training Award (IRTA) Fellows and Visiting Fellows. Above those sit Research Fellows and Clinical Fellows, then Senior Research Fellows and Senior Clinical Fellows. A Senior Research Fellow at NIH is someone with more than 10 years of experience beyond their postdoctoral training, a completely different tier from a newly minted PhD starting their first fellowship.

The terms “research fellow” and “postdoctoral researcher” overlap heavily, and many institutions use them interchangeably. The key distinction is that “fellow” usually implies a specific funding source or program structure, while “postdoc” is a broader category describing anyone doing research after earning a doctorate. All research fellows are postdocs, but not all postdocs hold named fellowships.

Clinical Fellows vs. Basic Science Fellows

Research fellowships split into two broad tracks. Basic science fellows (usually PhDs) work in labs studying fundamental biological, chemical, or computational questions. Clinical fellows (usually MDs or MD-PhDs) conduct research involving patients, clinical trials, or translational science that bridges lab findings and medical practice.

Patient-facing research positions require a medical degree because of the direct implications for patient safety. How Fellowships Are Funded

Research fellows receive funding through several channels. These awards are competitive and look impressive on a CV because they signal that an external review panel judged your research plan worthy of funding.

Other fellows are paid through their mentor’s existing grants, through institutional training grants that support multiple trainees at once, or through the university’s own funds. The funding source matters because it affects your independence, your obligations (some grants require specific training activities), and how the position is perceived when you apply for jobs later. Holding your own fellowship carries more weight than being supported by your advisor’s grant.

Research fellow compensation varies widely, but NIH stipend levels serve as a national benchmark that many universities follow. As of October 2024, a postdoctoral fellow at NIH with zero to one year of prior experience earns $67,200 in their first year. That figure increases with both experience and time in the position. A fellow with four to five years of prior experience earns $84,450 at entry, and stipends can climb above $100,000 for those with six or more years of experience.

These numbers represent a significant increase from historical levels (NIH stipends were below $50,000 as recently as a few years ago), but they still lag behind what PhD holders earn in industry. Fellows at pharmaceutical companies or biotech firms often receive higher compensation, though direct comparisons are difficult because industry fellowships bundle salary with different benefits packages.

How Long Fellowships Last

Most research fellowships run two to three years, with some extending to five. NIH has a general policy that postdoctoral trainees should not remain for more than five years, with an overall cap of eight years regardless of appointment type. Extensions beyond five years require an exceptional justification and approval through a formal review process. Time spent at NIH before earning your doctorate doesn’t count toward these limits.

Industry Fellowships

Pharmaceutical and biotech companies offer structured fellowship programs that look quite different from academic ones. These programs train recent graduates in specific functional areas of drug development: clinical development, regulatory affairs, medical affairs, pharmacovigilance, market access, and health economics, among others. The primary goal is producing skilled industry professionals rather than independent academic researchers.

Industry fellowships typically last one to two years. One-year programs focus almost entirely on work at the sponsoring company, while two-year programs often integrate academic coursework or rotations at a partnering university. In programs with an academic affiliation, fellows may spend a larger share of their first year at the university before shifting to primarily company-based work. By the end, fellows are expected to function as scientific leads within their chosen area.

What Makes a Competitive Applicant

Publications are the single most important credential when applying for a research fellowship. Fellowship programs in subspecialties of internal medicine, for example, explicitly evaluate applicants based on their publication record. Data from medical training programs shows that applicants who go on to publish during their fellowship had significantly more manuscripts at the time of application compared to those who didn’t (roughly two publications versus one). First-author papers and publications in higher-impact journals carry additional weight, though these qualitative factors are harder to measure systematically.
