A stiff neck is a tightness or restricted movement in the neck, usually caused by muscle strain or spasm in the muscles that support your cervical spine. It affects roughly 10% to 21% of adults in any given year, making neck pain one of the top five chronic pain conditions worldwide in terms of prevalence and years lost to disability. Most episodes resolve on their own within a few days to six weeks, but the experience can range from mildly annoying to severe enough to interfere with driving, sleeping, and working.
Why Your Neck Feels Stiff
The neck relies on a complex stack of seven vertebrae, cushioned by discs and held in place by layers of muscles. When one of those muscles gets strained, inflamed, or locked in a protective spasm, it restricts your ability to turn or tilt your head. The muscle most commonly involved is the levator scapulae, which runs from the upper part of your shoulder blade to the side of your upper neck. It helps you rotate and tilt your head, so when it tightens or develops trigger points, you feel stiffness on one side that can radiate toward the shoulder blade.
Other muscles in the same region, including the splenius and the scalenes along the side of the neck, contribute as well. When any of these muscles sense strain or injury, they tighten reflexively. This “muscle guarding” is your body’s attempt to protect the area from further damage. It’s useful in the short term but creates the locked-up feeling that makes turning your head painful.
Common Causes
The most frequent trigger is simply sleeping in an awkward position. Spending the night with your neck angled too far to one side or propped up too high can overstretch the muscles on one side and compress them on the other. You wake up with that familiar one-sided stiffness that makes it painful to check your blind spot while driving.
Prolonged posture is another major contributor. Hours spent looking down at a phone or hunching over a laptop push the head forward of the shoulders, forcing the neck muscles to work harder to support its weight. Over time, this forward head posture leads to chronic tightness and recurring episodes of stiffness.
Sudden movements, like whipping your head during sports or jerking awake, can strain neck muscles the same way you’d pull a hamstring. Stress and anxiety also play a role: many people unconsciously hold tension in their shoulders and neck, which gradually builds into stiffness by the end of the day. Cold drafts or sleeping with a fan blowing directly on your neck can cause muscles to contract and stiffen overnight as well.
Age-Related Wear and Tear
If your stiff neck keeps coming back or feels like a gradual loss of mobility rather than a sudden kink, normal aging of the spine may be involved. Cervical spondylosis, the medical term for age-related disc and joint changes in the neck, is present in about 25% of people under 40, roughly half of those over 40, and up to 85% of people over 60. By age 65, it shows up on imaging in 95% of people.
What happens is straightforward: the discs between your vertebrae lose water content and height over the years. This changes the natural curve of your neck and shifts how weight is distributed across the joints. The body responds by growing small bone spurs along the edges of the vertebrae to stabilize things. Those spurs, combined with thickening of the joints, gradually reduce how far you can turn and tilt your head. This kind of stiffness tends to develop slowly, feels worse in the morning, and improves somewhat with gentle movement throughout the day.
What Helps at Home
Most stiff necks from muscle strain start improving within a few days and fully resolve within about six weeks. During the first day or two, both heat and ice applied for around 30 minutes provide similar, modest pain relief. Ice can feel better when there’s acute inflammation, while heat tends to help more with muscle tightness and spasm. Choose whichever feels more soothing, or alternate between the two.
Gentle movement matters more than rest. Staying completely still allows the muscles to stiffen further. Slowly turning your head side to side, tilting your ear toward each shoulder, and looking up and down (only within a pain-free range) helps restore mobility. Avoid forcing through sharp pain.
Once the acute pain settles, isometric exercises can strengthen the neck without requiring you to move through a painful range. Press your palm against your forehead and resist with your neck muscles, holding for 10 seconds. Repeat five times, then do the same thing pressing against each side of your head and finally against the back of your head. These exercises build stability in the muscles that support your cervical spine without putting stress on irritated joints.
Preventing Stiffness During Sleep
Your pillow setup has a direct effect on whether you wake up with neck pain. The key principle is that your pillow should keep your spine in a neutral line, not kinked up or sagging down. Research on pillow ergonomics consistently finds that the ideal height differs depending on your sleeping position: back sleepers need a lower pillow (around 10 cm, or roughly 4 inches), while side sleepers need more height to fill the gap between the shoulder and head.
A contoured pillow with a lower center and higher sides accommodates both positions if you switch during the night. One study recommended center heights of about 2 to 4 cm (roughly 1 to 1.5 inches) with side heights of 12 to 14 cm (about 5 inches) depending on body size. Stomach sleeping is the hardest position on your neck because it forces sustained rotation to one side. If you can train yourself out of it, your neck will likely thank you.
When a Stiff Neck Signals Something Serious
A stiff neck from muscle strain is uncomfortable but not dangerous. Rarely, neck stiffness is a sign of meningitis, an infection of the membranes surrounding the brain and spinal cord. The critical difference is context: meningitis stiffness comes with a fever and gets worse when you try to tuck your chin to your chest, not when you turn your head side to side.
Research on early warning signs of meningococcal disease found that the combination of symptoms matters far more than any single one. Neck stiffness paired with confusion, sensitivity to light, a spreading rash (often red or purple spots that don’t fade when you press on them), or unusual leg pain significantly raises the probability of a serious infection. Confusion alone was the strongest individual red flag. A stiff neck without fever, without a rash, and without any change in mental clarity is almost certainly muscular.
Other reasons to seek prompt evaluation include stiffness after a car accident or fall (to rule out a fracture or ligament injury), stiffness that progressively worsens over weeks rather than improving, or stiffness accompanied by numbness, tingling, or weakness radiating into your arms or hands. Those arm symptoms suggest a nerve is being compressed, either by a herniated disc or by bone spurs narrowing the space where nerves exit the spine.
