What Is a Too Low Heart Rate and When to Worry?

A heart rate below 60 beats per minute is generally considered low, a condition called bradycardia. But “too low” depends less on the number itself and more on whether your body is getting enough blood flow. A fit athlete can walk around with a resting heart rate in the 40s and feel perfectly fine, while someone else at 55 beats per minute might feel dizzy and exhausted. The line between normal-low and dangerously-low is really about symptoms.

What Counts as a Low Heart Rate

The standard medical threshold is 60 beats per minute at rest. Anything below that qualifies as bradycardia. But this number is a starting point, not an alarm. During sleep, a healthy adult’s heart rate typically drops to 50 to 75 beats per minute, and rates as low as 40 during deep sleep can be perfectly normal. Well-trained endurance athletes often have resting rates near 40 beats per minute because their hearts pump more blood with each beat, so they simply need fewer beats to keep everything running.

Where the numbers start to matter more: a sleeping heart rate consistently in the 20s warrants a conversation with your doctor to verify the reading is accurate and rule out a conduction problem. And any heart rate that drops low enough to cause symptoms is, by definition, too low for your body, regardless of the specific number.

How It Feels When Your Heart Rate Is Too Low

The core problem with a dangerously slow heart rate is that your brain and organs stop getting enough oxygen-rich blood. When that happens, you may notice:

  • Dizziness or lightheadedness, especially when standing up
  • Unusual fatigue, particularly during physical activity that didn’t used to tire you out
  • Fainting or near-fainting
  • Shortness of breath
  • Chest pain
  • Confusion or memory problems

If you’re experiencing none of these, a low number on your fitness tracker is generally not a crisis. The distinction doctors care about most is whether your slow heart rate correlates with symptoms. A person who feels great at 48 beats per minute is in a fundamentally different situation than someone who keeps nearly passing out at 52.

What Causes a Heart Rate to Drop Too Low

Your heart’s rhythm starts with a tiny cluster of cells called the sinus node, which acts as a natural pacemaker. When something disrupts this node or the electrical pathways that carry its signal through the heart, the rate slows down. The causes fall into two broad categories.

Problems Inside the Heart

The most common internal cause is age-related scarring (fibrosis) of the sinus node. Over time, the tissue that generates your heartbeat can stiffen and lose its ability to fire electrical signals reliably. Heart disease plays a role too. Reduced blood flow to the sinus node from coronary artery disease can damage it, and a heart attack can temporarily or permanently disrupt the heart’s electrical system. Heart failure and structural remodeling of the heart after a major cardiac event can also slow conduction. Less commonly, diseases that deposit abnormal material in heart tissue, such as sarcoidosis or amyloidosis, interfere with electrical signaling. Some people carry genetic mutations affecting the ion channels that regulate heart rhythm, making them prone to a slow rate from birth.

Problems Outside the Heart

Many cases of bradycardia come from external factors that are often reversible. Medications are the most frequent culprit. Beta-blockers, commonly prescribed for high blood pressure and heart conditions, slow the heart rate by design. Calcium channel blockers like diltiazem and verapamil do the same. Other offenders include certain antiarrhythmic drugs, the heart medication digoxin, lithium (used for bipolar disorder), clonidine (for blood pressure), and even some antidepressants like citalopram. Beta-blocker eye drops for glaucoma can occasionally cause bradycardia because enough of the drug absorbs into the bloodstream to affect the heart.

Metabolic and hormonal imbalances also play a role. An underactive thyroid is a classic cause. High or low potassium levels, low calcium, low body temperature, and low oxygen levels can all slow the heart. Athletic training increases vagal tone, the activity of the nerve that naturally puts the brakes on heart rate, which is why endurance athletes have such low resting rates. This is a healthy adaptation, not a disease.

When a Low Heart Rate Becomes Dangerous

A persistently slow heart rate that causes symptoms can lead to real problems over time. Repeated fainting or near-fainting episodes carry the risk of falls and serious injuries, especially in older adults. When the heart can’t pump enough blood during exertion, exercise tolerance drops and daily activities become difficult. In severe cases where the heart rate is profoundly slow or pauses for several seconds at a time, organs can sustain damage from inadequate blood flow.

The combination of a slow heart rate with chest pain, fainting, or significant confusion is a situation that needs urgent medical attention. These symptoms suggest your heart isn’t meeting your body’s basic oxygen demands.

How a Low Heart Rate Is Treated

Treatment depends entirely on the cause. If a medication is slowing your heart rate too much, your doctor may adjust the dose or switch to an alternative. If an underactive thyroid or an electrolyte imbalance is responsible, correcting that underlying problem often restores a normal rate. These are reversible causes, and addressing them is always the first step.

When the slow rate comes from permanent damage to the heart’s electrical system, the standard treatment is a pacemaker. This small device monitors your heart rhythm and delivers a tiny electrical impulse whenever the rate drops below a programmed threshold, essentially taking over the job your sinus node can no longer do reliably. The main criteria for getting one are straightforward: your symptoms clearly match the slow rhythm, the problem persists after any reversible causes have been addressed, and the electrical dysfunction is significant enough to warrant it (such as complete heart block or documented pauses).

Modern pacemakers come in two forms. The traditional type uses a small battery-powered generator placed under the skin near the collarbone, connected to the heart by thin wires threaded through a vein. Newer leadless pacemakers are tiny capsules implanted directly inside the heart, eliminating the need for wires. The choice depends on the type of pacing you need. For most people, a pacemaker effectively eliminates symptoms and restores the ability to be physically active without dizziness or fatigue.

Low Heart Rate During Sleep

If you’re checking a smartwatch or sleep tracker and seeing numbers that look alarming, some context helps. A sleeping heart rate between 40 and 100 beats per minute falls within the expected range for adults. Your heart rate naturally dips during deep sleep as your nervous system shifts into rest-and-repair mode. Endurance athletes can see rates in the 30s during sleep without any cause for concern, as long as they feel well during the day.

The numbers that genuinely warrant attention are consistent readings in the 20s during sleep, or any sleeping heart rate paired with daytime symptoms like unexplained fatigue, dizziness, or episodes where you feel like you might black out. A single low reading on a consumer device can also just be a measurement error, so patterns matter more than any individual number.
