An ER technician (also called an ED tech) is a frontline healthcare worker in a hospital emergency department who assists nurses and physicians with patient care. They handle a wide range of hands-on clinical tasks, from drawing blood and running EKGs to splinting fractures and cleaning wounds, all in a fast-paced, high-acuity setting. It’s an entry-level allied health position, but the work is far from basic.

What ER Technicians Do Every Day

The core of the job is keeping the emergency department moving. ER techs take and record vital signs, monitor oxygen saturation levels, and perform reassessments on patients as their conditions change. They draw blood, collect specimens, and transport those samples to the lab. When a physician orders an EKG, the tech is typically the one hooking up the leads and running the strip.

Beyond diagnostics, ER techs do a significant amount of direct patient care. They irrigate and dress wounds, apply splints and posterior molds for orthopedic injuries, insert peripheral IV lines, and prep IV setups. They also handle infection control procedures, assist with bedside procedures, enter computer orders, and manage the constant flow of paperwork and requisitions that keep the department organized. In many hospitals, techs also provide post-mortem care.

The variety is one of the role’s biggest draws. Unlike a certified nursing assistant on a hospital floor, where the work centers on long-term patient needs like bathing and toileting, an ER tech deals with acute situations. You might help stabilize a car accident victim in one bay and then walk into the next to draw blood on someone with chest pain. The patient population rotates constantly, and no two shifts look the same.

How It Differs From EMT and CNA Roles

These three roles overlap but serve different purposes. A CNA (certified nursing assistant) typically works on hospital floors or in long-term care facilities, focusing on daily living tasks like vital signs, hygiene, and mobility. An EMT works in the field, responding to 911 calls and transporting patients to the hospital. An ER technician works inside the emergency department, bridging the gap between the two.

ER techs generally have a broader clinical skill set than CNAs. They regularly perform phlebotomy, EKGs, splinting, wound irrigation, and procedure preparation, tasks that CNAs on a medical floor rarely touch. Compared to EMTs working in the field, ER techs lose some of the autonomy that comes with being the first responder on scene, but they gain close, daily interaction with physicians and a wider exposure to medical decision-making. Most hospitals prefer to hire ER techs who hold an EMT certification rather than a CNA credential alone, because EMT training covers a higher level of clinical assessment. CNAs with extensive experience can still land ER tech positions, but the path is more competitive.

Training and Certification

There is no single national “ER technician” license. Instead, most hospitals require candidates to hold an EMT-Basic (EMT-B) certification as a baseline. To earn that, you need to complete a state-approved EMT course that meets the National Emergency Medical Services Education Standards, then pass the National Registry of Emergency Medical Technicians certification exam along with a state-approved skills competency evaluation. EMT programs typically run a few months and are offered at community colleges, technical schools, and some fire departments.

Some hospitals also require or prefer additional credentials like a phlebotomy certificate or BLS (Basic Life Support) certification. Once hired, most new ER techs go through an on-the-job orientation period where they learn the specific protocols and equipment used in that department.

Work Environment and Schedule

Emergency departments operate around the clock, and ER techs work accordingly. Most work full-time schedules of 40 or more hours per week, commonly in 12-hour shifts. Rotating shifts are the norm. In one study of emergency medical workers, 72% worked rotating schedules rather than fixed day shifts, and 86% worked weekends. Some facilities use 24-hour shifts, which are associated with higher stress and fatigue.

The work is physically demanding. You’ll be on your feet for the entire shift, lifting and repositioning patients, and occasionally dealing with combative or intoxicated individuals. Exposure to blood, bodily fluids, and infectious diseases is routine. The emotional toll is real, too. Repeated exposure to traumatic situations, combined with the pressure of life-or-death circumstances, contributes to occupational stress and burnout in emergency settings. That said, people who thrive in this role tend to be drawn to exactly that intensity.

Pay and Job Outlook

ER technician salaries reflect the entry-level nature of the position. The national average sits around $33,000 to $38,000 per year, roughly $16 to $18 per hour, with variation depending on location, hospital size, and experience. The 25th percentile earns closer to $14 per hour, while techs at the 75th percentile bring in about $17. Urban trauma centers and teaching hospitals tend to pay on the higher end.

Job prospects are steady. The Bureau of Labor Statistics projects 5% employment growth for EMTs and paramedics from 2024 to 2034, which is faster than the average for all occupations. The broader category of health technologists and technicians is projected to grow at 6%. Emergency departments continue to face high patient volumes, and the need for support staff who can handle clinical tasks under pressure isn’t going away.

Career Advancement From ER Tech

Most people don’t stay ER techs forever, and the role is widely recognized as one of the best stepping stones in healthcare. The clinical exposure you get, working alongside emergency physicians, learning to assess acutely ill patients, functioning on an interprofessional team, translates directly into stronger applications for nursing school, physician assistant programs, or medical school. Admissions committees value the hands-on patient care hours, and the experience itself helps you figure out what specialty or career path fits.

The more traditional advancement track runs from EMT to paramedic, which significantly expands your scope of practice and pay. Paramedics can then specialize further into community paramedicine, where they deliver care protocols outside the hospital setting. Others use the ER tech role as a launchpad into registered nursing, where the emergency department experience gives them a head start on clinical confidence and multitasking under pressure.
