What Is an IR Tech? Role, Duties, and Salary

An IR tech, short for interventional radiology technologist, is a specialized radiologic technologist who assists physicians during minimally invasive, image-guided medical procedures. Rather than taking standard X-rays or CT scans for diagnostic purposes, IR techs work in procedure rooms that function much like operating rooms, helping guide catheters, needles, and other instruments through the body using real-time imaging. It’s a hands-on clinical role that blends technical imaging skills with direct patient care.

What an IR Tech Does

The core of the job is operating and maintaining the imaging equipment that lets physicians see inside the body during a procedure. The most common piece of equipment is a C-arm fluoroscopy system, a mobile X-ray unit shaped like the letter C, with an X-ray source on one end and a digital detector on the other. The IR tech positions this equipment, adjusts settings for each procedure, and captures images in real time so the physician can navigate instruments precisely. They also work with ultrasound equipment and image-processing software to produce, label, format, and review diagnostic images throughout a case.

But the role extends well beyond pushing buttons. Before a procedure, the IR tech screens the patient, interprets the physician’s orders, prepares the procedure room, and sets up a sterile environment similar to what you’d find in surgery. They explain what’s about to happen to the patient, position them on a specialized carbon fiber table (which doesn’t interfere with imaging), and perform safety checks on all equipment.

During the procedure, the IR tech maintains the sterile field, monitors the patient, and alerts the physician to any abnormal indicators. They select and modify imaging techniques on the fly to get the clearest possible pictures, and they may administer contrast dye or medications under supervision. Afterward, they help with cleanup, equipment maintenance, and ensuring patient records are accurately completed.

Types of Procedures

IR techs assist with a wide range of minimally invasive procedures. These fall into two broad categories: vascular and nonvascular. Vascular procedures involve blood vessels and include diagnostic angiography (imaging arteries and veins), angioplasty and stenting to open blocked vessels, and placing ports or central lines for long-term medication delivery. Nonvascular procedures include image-guided biopsies, draining fluid collections or abscesses, tumor treatments delivered through catheters, and placing drainage tubes in organs like the kidneys or liver.

What ties all of these together is that the physician works through tiny incisions or needle punctures rather than open surgery, relying on imaging to see what they’re doing. The IR tech is the person responsible for making that imaging work correctly in real time.

Radiation Safety

Because IR techs spend their days working near live X-ray equipment, radiation protection is a major part of the job. The standard protective setup includes a lead apron (often a vest-and-skirt combination to reduce back strain), a thyroid shield, and leaded eyewear with side shields. Many techs prefer the wrap-around apron style, which provides double-thickness protection across the front of the body.

Procedure rooms also use ceiling-suspended transparent lead shields, which dramatically reduce radiation exposure to the eyes, and specialized patient drapes containing metallic elements like bismuth or tungsten-antimony. These drapes alone have been shown to reduce operator exposure by 12-fold for the eyes, 26-fold for the thyroid, and 29-fold for the hands.

IR techs typically wear two dosimeters, small devices that measure cumulative radiation exposure. One goes under the apron and one at collar level above it. In the United States, the occupational limit is 50 millisieverts of effective dose in any single year, with a lifetime cap of 10 millisieverts multiplied by your age. Staying well within these limits is a daily concern, and IR techs are trained to use positioning techniques and protective equipment to minimize exposure for themselves, the physician, and the patient.

Education and Certification

Becoming an IR tech is a two-stage process. The first step is earning credentials as a general radiologic technologist, which typically requires completing an accredited associate’s or bachelor’s degree program in radiologic technology and passing a certification exam. Once you hold that foundational credential, you can pursue a postprimary certification in Vascular Interventional Radiography through the American Registry of Radiologic Technologists (ARRT). This advanced credential requires completing structured education in interventional procedures, logging a specified number and variety of clinical cases, and passing a specialized exam covering the techniques and knowledge unique to the IR setting.

Some IR techs enter the field by working in a general radiology role first and then transitioning into an interventional department where they gain hands-on experience before pursuing the advanced certification.

Work Environment and Schedule

Most IR techs work in hospital-based interventional radiology departments, which often function on a schedule similar to surgical teams. Daytime shifts during the week are standard, but hospitals typically require on-call availability for emergencies like internal bleeding or stroke interventions that need immediate attention outside regular hours.

A growing number of IR techs work in outpatient surgical centers, where the pace and culture differ from hospital settings. Outpatient centers generally handle scheduled, lower-risk procedures and don’t have the same emergency call demands. However, outpatient teams are smaller, so techs in these settings often take on broader responsibilities. Staff may cross-train to handle scheduling, equipment setup, and recovery duties that would be split among separate team members in a larger hospital.

Salary and Job Outlook

The Bureau of Labor Statistics groups IR techs with the broader category of radiologic technologists and technicians, which had a median annual salary of $77,660 as of May 2024. IR techs with vascular interventional certification generally earn above that median because of their specialized training and the higher-acuity procedures they support, though exact figures vary by employer and region.

Employment for radiologic technologists overall is projected to grow 4 percent from 2024 to 2034, faster than the average for all occupations. The growth is driven partly by the expanding range of conditions that can be treated with minimally invasive, image-guided techniques rather than open surgery, which continues to increase demand for skilled IR teams.
