What Is an LTC Nurse? Role, Skills, and Salary

An LTC nurse is a licensed nurse who works in a long-term care facility, providing ongoing medical and personal care to residents who need extended support with health conditions or daily living. Unlike hospital nurses who treat patients through acute episodes and discharge them, LTC nurses build sustained relationships with the same residents over weeks, months, or years. They work in settings like skilled nursing facilities, assisted living communities, memory care units, and continuing care retirement communities.

Where LTC Nurses Work

Long-term care spans several types of facilities, each with a different level of medical involvement. Skilled nursing facilities (often called nursing homes) are the most medically intensive. They offer 24-hour supervision, nursing care, three daily meals, and rehabilitation services like physical, occupational, and speech therapy. Nurses in these settings handle complex medical needs on a daily basis.

Assisted living facilities require less clinical intensity. Residents there need help with daily activities but not the round-the-clock medical oversight of a nursing home. Some assisted living communities run specialized programs for residents with Alzheimer’s disease or other forms of dementia, and nurses in those units focus heavily on cognitive and behavioral care. Continuing care retirement communities combine independent living, assisted living, and skilled nursing on a single campus, so nurses may work across different levels of acuity depending on the building.

What LTC Nurses Do Every Day

The daily work of an LTC nurse blends clinical tasks with ongoing relationship-based care. Core responsibilities include assessing each resident’s condition, administering medications, monitoring vital signs, performing wound care, and helping with mobility. But the distinguishing feature of this role is care coordination over time. LTC nurses develop and update individualized care plans, track how chronic conditions progress, and adjust their approach as a resident’s needs change.

The medical conditions they manage reflect the population they serve. CDC data from residential care communities shows the most common chronic conditions among residents: high blood pressure (55%), Alzheimer’s disease and other dementias (34%), depression (27%), arthritis (20%), diabetes (20%), heart disease (17%), osteoporosis (12%), COPD (11%), stroke (10%), and cancer (9%). On any given shift, an LTC nurse might be managing several of these conditions simultaneously across a group of residents.

LTC nurses also serve as the communication hub for an interdisciplinary team. They coordinate with physicians, pharmacists, physical and occupational therapists, social workers, and home care coordinators. Because nurses spend more direct time with residents than any other provider, they’re often the first to notice a change in condition and the ones responsible for alerting the rest of the team.

Staffing and Patient Ratios

One of the realities of long-term care nursing is higher patient loads compared to hospital settings. In skilled nursing facilities, a single nurse may be responsible for anywhere from 9 to 25 residents depending on the shift and the residents’ acuity levels. Daytime shifts tend to have the most staff, with ratios tightening on evenings and nights. States set their own minimum staffing requirements, and some, like California, mandate specific hours of nursing care per resident per day, typically ranging from about 3.8 to 6.0 total nursing hours depending on the facility type.

These ratios mean LTC nurses need strong time management and prioritization skills. You’re juggling medication rounds, assessments, documentation, and family communication across a larger group of residents than a hospital nurse typically manages.

Education and Licensing Paths

Two main types of nurses work in long-term care: licensed practical nurses (LPNs) and registered nurses (RNs). The paths differ significantly in time and scope.

  • LPNs complete a practical nursing program that lasts at least nine months. These programs require a high school diploma for entry. After graduating, LPN candidates must pass the NCLEX-PN licensing exam. LPNs handle much of the direct bedside care in LTC facilities, including medication administration, wound care, and vital sign monitoring.
  • RNs complete either an associate degree (two years) or a bachelor’s degree in nursing (four years), then pass the NCLEX-RN exam. In long-term care, RNs typically take on supervisory roles, develop care plans, and manage more complex clinical decisions. Many LTC facilities employ a mix of both, with RNs overseeing LPNs and certified nursing assistants.

LTC is one of the few healthcare settings where LPNs make up a large portion of the nursing workforce, making it an accessible entry point for people starting a nursing career.

Specialization and Certification

Nurses who want to formalize their expertise in long-term care can pursue board certification in gerontological nursing through the American Nurses Credentialing Center. The GERO-BC credential requires an active RN license, two years of full-time nursing experience, at least 2,000 hours of clinical practice in gerontological nursing within the past three years, and 30 hours of continuing education in the specialty. The certification is valid for five years.

While certification isn’t required to work in long-term care, it signals specialized knowledge in aging-related conditions and can open doors to leadership positions like director of nursing or clinical nurse specialist roles within LTC organizations.

Skills That Matter Most in LTC

The technical skills of long-term care nursing overlap with other specialties, but the soft skills carry extra weight. Patience is essential when working with residents who have dementia or cognitive impairments and may need the same instructions repeated many times. Communication goes beyond talking to residents: it means reading nonverbal cues from people who may not be able to articulate their pain or confusion, and translating clinical information for family members who are often anxious about a loved one’s care.

Advocacy is another critical skill. LTC residents are among the most vulnerable patient populations, and nurses often serve as their primary voice when it comes to care decisions. The American Nurses Association highlights integrity and patient advocacy as core nursing strengths, and these qualities carry particular importance in settings where residents may have limited ability to speak up for themselves. Flexibility and emotional steadiness round out the profile. LTC nurses deal with grief, behavioral challenges, and the emotional complexity of caring for people at the end of life, sometimes over years.

Salary and Job Outlook

Registered nurses working in nursing and residential care facilities earned a median annual salary of $81,820 in May 2024, according to the Bureau of Labor Statistics. That’s lower than the overall RN median of $93,600, which reflects the fact that hospitals and outpatient centers generally pay more. LPN salaries in long-term care are lower still, though they vary widely by state and facility type.

Demand for LTC nurses is steady and growing. RN employment overall is projected to grow 5% from 2024 to 2034, faster than average for all occupations. The aging U.S. population is the main driver: as more people live into their 80s and 90s, the need for skilled nursing in residential care settings will continue to increase. For nurses who value continuity with patients and find fulfillment in geriatric care, long-term care offers stable, meaningful work with consistent demand.
