What Is an LVN in Healthcare? Duties and Career Path

An LVN, or Licensed Vocational Nurse, is a healthcare professional who provides basic bedside nursing care under the direction of a physician or registered nurse. The title is used exclusively in California and Texas. Every other U.S. state calls the same role a Licensed Practical Nurse (LPN). The job responsibilities, licensing requirements, and scope of practice are identical regardless of which term is used.

What an LVN Actually Does

LVNs handle a wide range of hands-on patient care tasks. Their scope of practice includes measuring vital signs, performing basic patient assessments, administering prescribed medications, providing wound care and hygienic care, and documenting patient information. With separate board certification, they can also perform non-medicated IV therapy and blood draws.

The work is technical and physical. LVNs spend much of their shifts at the bedside: checking blood pressure, changing dressings, helping patients with daily activities, and reporting changes in a patient’s condition to the supervising RN or physician. They follow established care plans rather than creating them, which is one of the key distinctions between their role and that of a registered nurse.

How LVNs Differ From RNs

The biggest difference comes down to clinical authority. LVNs typically cannot perform initial patient assessments, create nursing care plans, or triage patients. Those responsibilities belong to registered nurses. RNs also handle tasks that require close monitoring and frequent reassessment, like initiating blood transfusions, administering the first round of antibiotics, and placing IVs. Only some states allow LVNs to provide IV therapy at all, and even then, it must be done under supervision.

This doesn’t make the LVN role less important. In practice, LVNs often spend more direct time with patients than RNs do, particularly in long-term care settings where ongoing bedside support is the core of daily operations. The roles are complementary: RNs assess and plan, LVNs execute much of that plan through consistent, hands-on care.

Where LVNs Work

Nursing and residential care facilities employ the largest share of LVNs, accounting for about 37% of the workforce. Hospitals make up 16%, and home healthcare services account for 12%. The remainder work in physician offices, outpatient clinics, government agencies, and other healthcare settings.

The concentration in long-term care makes sense given the LVN scope of practice. Skilled nursing facilities need consistent bedside care for residents with stable but ongoing health needs, which aligns well with what LVNs are trained and licensed to do. Hospital roles tend to involve faster-paced environments where LVNs work alongside RNs on medical-surgical floors or in outpatient departments.

Education and Licensing

Becoming an LVN requires completing a state-approved vocational nursing program, which typically takes about 12 months of full-time study. Programs are often structured as certificate programs (around 49 units at the college level) rather than degree programs. Coursework covers nursing care for physical and mental health conditions, human development, health promotion, and hands-on clinical skills practiced across the lifespan.

After graduating, candidates must pass the NCLEX-PN, a national licensing exam developed by the National Council of State Boards of Nursing. The exam tests knowledge and clinical judgment across four major areas: safe and effective care environments, health promotion, psychosocial integrity, and physiological integrity. That last category covers everything from basic comfort care to pharmacology and managing complications. A passing score is required before you can practice in any state.

Why It’s Called LVN in Some States

California and Texas adopted the term “Licensed Vocational Nurse” when they established their licensing frameworks, while the rest of the country settled on “Licensed Practical Nurse.” The distinction is purely geographic. If you hold an LVN license in California and want to work in, say, Florida, you would apply for an LPN license there, but your training and qualifications carry over. The profession is the same.

Career Growth and Bridge Programs

Many LVNs use the role as a stepping stone toward becoming a registered nurse. LVN-to-RN bridge programs allow you to build on your existing training and earn an associate degree in nursing (ADN) in roughly 18 months of full-time study, compared to the two or more years a traditional ADN program takes for someone starting from scratch. You will need an active, unencumbered LVN license to enroll.

The bridge path is popular because it lets you work as an LVN while gaining clinical experience, then transition to a higher scope of practice without starting over. An RN license opens the door to higher pay, greater clinical independence, and specialization opportunities that aren’t available at the LVN level. For those who want to stay in the LVN role long-term, experience in specialized settings like IV therapy certification or wound care can also increase earning potential and job options.
