What Is an RNA Nurse? Roles, Pay, and Training

RNA nurse can refer to two very different roles in healthcare: a Registered Nurse Anesthetist (commonly called a CRNA) or a Restorative Nurse Assistant. The first is one of the highest-paid nursing specializations in the country, responsible for administering anesthesia during surgery. The second is a certified nursing assistant with extra training in rehabilitation techniques. Which one you’re looking for depends on the context where you encountered the term.

Registered Nurse Anesthetist (CRNA)

A Registered Nurse Anesthetist, formally known as a Certified Registered Nurse Anesthetist or CRNA, provides anesthesia care to patients before, during, and after surgery. They administer medications to keep patients asleep or pain-free and continuously monitor every biological function throughout a procedure. CRNAs work in hospitals, surgical clinics, emergency rooms, outpatient care centers, military facilities, and doctors’ offices.

Their day-to-day responsibilities include physically assessing patients before surgery, explaining what to expect from anesthesia, choosing and administering the right type of anesthesia for each procedure, adjusting medications throughout an operation to keep the patient stable, and managing recovery as the patient wakes up. In many settings, CRNAs are the primary anesthesia providers, particularly in rural hospitals where physician anesthesiologists may not be available.

How CRNAs Differ From Anesthesiologists

Anesthesiologists are physicians who completed medical school and a residency. CRNAs are advanced practice nurses who followed a nursing pathway to reach a similar clinical role. Both can administer anesthesia, but the degree of independence a CRNA has varies by state. As of 2024, 25 states and Guam have opted out of the federal requirement that CRNAs be supervised by a physician, meaning nurse anesthetists in those states can practice independently.

In states that haven’t opted out, CRNAs typically work under the supervision of or in collaboration with a physician anesthesiologist or surgeon. The clinical work they perform at the bedside, however, is largely the same regardless of the supervision model.

Education and Certification Requirements

Becoming a CRNA is one of the longest educational paths in nursing. You’ll need a bachelor’s degree in nursing first, followed by at least one year of full-time work as a registered nurse in a critical care setting like an ICU. That hands-on experience with critically ill patients is a strict prerequisite for admission to a nurse anesthesia program.

From there, you’ll complete a doctoral degree. All accredited nurse anesthesia programs now award a doctorate for entry into practice, which typically takes three to four years of graduate study. Programs combine advanced coursework in pharmacology, anatomy, and physiology with extensive clinical rotations where students administer anesthesia under supervision.

After graduating, you must pass the National Certification Examination (NCE), administered by the National Board of Certification and Recertification for Nurse Anesthetists (NBCRNA). Passing this exam earns the CRNA credential and allows you to begin practicing. Ongoing recertification is required to maintain the credential throughout your career.

Salary and Job Growth

CRNAs are among the highest-earning nurses in the United States. The median annual wage was $223,210 as of May 2024, according to the Bureau of Labor Statistics. Job growth for nurse anesthetists is projected at 9% from 2024 to 2034, which is faster than the average for all occupations. Demand is driven partly by an aging population needing more surgical procedures and partly by healthcare systems looking for cost-effective anesthesia providers.

Restorative Nurse Assistant (RNA)

In long-term care and rehabilitation settings, RNA stands for something entirely different: Restorative Nurse Assistant. This is an expanded role for Certified Nursing Assistants (CNAs) who complete additional training in therapeutic rehabilitation techniques. Rather than the years of graduate education required for a CRNA, the RNA credential builds on an existing CNA certification with specialized skills training.

Restorative Nurse Assistants help residents in nursing homes and rehabilitation facilities maintain or regain physical abilities. Their training covers skills from three therapy disciplines: speech therapy, occupational therapy, and physical therapy. Practical competencies include range of motion exercises, transfer techniques (helping residents move safely between a bed and wheelchair, for example), ambulation activities to keep residents walking, and proper draping during treatment. All of this work is prescribed and supervised by licensed therapists or nurses.

The role exists because residents in long-term care often need daily repetition of rehabilitation exercises between their scheduled therapy sessions. Having trained RNAs on staff means residents get more consistent practice with mobility and self-care skills, which can slow physical decline and improve quality of life.

How to Tell Which Role Someone Means

Context usually makes the distinction clear. If someone mentions RNA in connection with anesthesia, surgery, or advanced practice nursing, they’re referring to a Registered Nurse Anesthetist (CRNA). If the conversation involves nursing homes, rehabilitation, or CNA training, they mean a Restorative Nurse Assistant. In job postings, the abbreviation CRNA is far more standard for nurse anesthetists, while RNA almost always refers to the restorative nursing role. If you’re exploring career options, the two paths couldn’t be more different in terms of education, pay, and daily responsibilities.
