Aspirin 81 mg, often called “baby aspirin” or low-dose aspirin, is primarily used to reduce the risk of heart attacks and strokes in people with cardiovascular disease. It works by preventing blood clots from forming in your arteries. Beyond heart health, it also has a specific role in pregnancy for reducing the risk of preeclampsia.
How 81 mg Aspirin Protects Your Heart
At this low dose, aspirin’s main job is to keep your blood from clotting too easily. It does this by permanently disabling platelets, the tiny cell fragments responsible for clumping together to form clots. Specifically, it blocks platelets from producing a chemical that triggers clotting and narrows blood vessels. Because platelets can’t repair themselves (they lack the internal machinery to make new proteins), a single dose of aspirin keeps each affected platelet from clotting for its entire lifespan of about 7 to 10 days.
This is why 81 mg is enough. You don’t need a full-strength pain-relief dose to shut down platelet clotting. The low dose also has a practical advantage: it preferentially targets platelets while leaving the blood vessel lining more able to recover its own protective functions.
Secondary Prevention: After a Heart Attack or Stroke
The strongest case for daily 81 mg aspirin is in people who already have cardiovascular disease. If you’ve had a heart attack, a stroke, or been diagnosed with coronary artery disease, daily low-dose aspirin is a standard part of treatment. Current guidelines recommend antiplatelet therapy (aspirin being the most common choice) for people with these conditions, typically at a dose of 75 to 100 mg daily.
You’ll also likely be prescribed 81 mg aspirin after procedures that open blocked arteries, such as stent placement. It helps prevent new clots from forming at the treatment site. People with peripheral artery disease (reduced blood flow to the legs) benefit as well, since aspirin improves circulation. It’s also used for people with certain abnormal heart rhythms that increase clot risk.
Primary Prevention: Before Heart Disease Develops
The guidelines for taking aspirin when you haven’t yet had a cardiovascular event have shifted significantly. For decades, many healthy adults took a daily aspirin “just in case.” That advice has narrowed considerably.
The U.S. Preventive Services Task Force now recommends against starting aspirin for heart disease prevention if you’re 60 or older. The bleeding risks at that age generally outweigh the benefits. For adults aged 40 to 59 with a 10% or greater chance of a cardiovascular event in the next 10 years, the decision is individual. The net benefit in this group is small, and it only tips in your favor if you’re not at increased risk for bleeding and you’re willing to take it consistently.
For people who do start aspirin for primary prevention, modeling data suggest it may be reasonable to consider stopping around age 75, when bleeding risk climbs steeply. The benefits that have built up over years of use don’t disappear, but continuing becomes less favorable as you age.
Preeclampsia Prevention in Pregnancy
Low-dose aspirin has a well-established role in reducing the risk of preeclampsia, a dangerous pregnancy complication involving high blood pressure and organ damage. The recommended dose in the U.S. is 81 mg daily, started after 12 weeks of gestation.
This applies to pregnant people considered at high risk, which includes those with a history of preeclampsia, carrying more than one baby, chronic high blood pressure, type 1 or type 2 diabetes, kidney disease, or autoimmune conditions like lupus. Combinations of moderate risk factors can also qualify someone: being pregnant for the first time, a BMI over 30, a family history of preeclampsia, age 35 or older, or conception through IVF.
Why 81 mg Instead of 325 mg
A large trial of 15,000 patients with a history of cardiovascular disease compared these two doses head to head. Over a median follow-up of about two years, there was no difference in effectiveness. Rates of heart attack, stroke, and death were the same in both groups. There was also no measurable difference in major bleeding events between the two doses.
Given that both doses work equally well, 81 mg has become the standard recommendation simply because there’s no reason to take more. Lower doses also tend to be easier on the stomach day to day, even if the statistical bleeding risk is similar in clinical trials.
Bleeding Risks and Side Effects
The main concern with daily aspirin is bleeding. Across observational studies, low-dose aspirin increases the relative risk of gastrointestinal bleeding by about 40%, with upper GI bleeding risk more than doubling. The absolute numbers are relatively small, ranging from roughly 0.5 to 3.6 cases per 1,000 people per year, but they add up over years of daily use. The risk of bleeding inside the brain also rises by about 40%.
These risks are why the calculus has changed for primary prevention. If you already have heart disease, the benefit of preventing a second heart attack or stroke clearly outweighs the bleeding risk for most people. If you don’t have heart disease, you’re taking on that bleeding risk for a smaller potential payoff.
Certain conditions make bleeding more likely and may rule out daily aspirin entirely. These include a history of stomach ulcers, current use of blood-thinning medications, liver or kidney problems, and uncontrolled high blood pressure. People with asthma may also have aspirin sensitivity. Aspirin should never be given to children under 16 unless specifically prescribed, due to the risk of a rare but serious condition called Reye’s syndrome.
Enteric-Coated vs. Regular Tablets
Most 81 mg aspirin sold in the U.S. is enteric-coated, meaning it has a smooth shell designed to survive stomach acid and dissolve in the intestine instead. The idea is that bypassing the stomach reduces irritation and bleeding. In practice, studies have found no advantage. Coated and uncoated aspirin are equally effective at preventing heart attacks and strokes, and coated versions don’t reduce the rate of major bleeding episodes. The coating does slow absorption, which is why uncoated aspirin is preferred if you’re chewing one during a suspected heart attack, when speed matters.
