Ataxia in dogs is a loss of normal coordination that makes a dog appear wobbly, unsteady, or drunk when walking. The word comes from Greek, literally meaning “without order.” It’s not a disease itself but a neurological sign that something is disrupting the brain, spinal cord, or inner ear’s ability to coordinate movement. Importantly, ataxia affects coordination without affecting strength, which distinguishes it from weakness or paralysis.
Three Types of Ataxia
Ataxia looks different depending on which part of the nervous system is involved. Veterinarians generally classify it into three types, each pointing to a different location of the problem.
Vestibular Ataxia
This is the most recognizable form and the one pet owners most commonly encounter. The vestibular system, located in the inner ear and brainstem, controls balance and spatial orientation. When it’s disrupted, dogs develop a characteristic set of signs: a head tilt (one ear held lower than the other), swaying or leaning to one side, walking in tight circles, and sometimes falling or rolling over entirely. Many dogs also develop nystagmus, an involuntary rhythmic movement of the eyes that can be horizontal, vertical, or rotary.
With a one-sided vestibular problem, the head tilt, circling, and eye movement all occur toward the affected side. When both sides are involved, the dog sways its head back and forth, loses balance in both directions, and stands with legs wide apart for stability. Bilateral vestibular disease typically does not produce a head tilt or nystagmus, which can actually make it harder to recognize at first.
Cerebellar Ataxia
The cerebellum fine-tunes the rate, range, and force of every movement. When it’s damaged, dogs lose that calibration. Their movements become exaggerated and jerky. They overshoot when stepping (lifting their paws too high), a sign called hypermetria. They stand with a wide base, and their trunk sways as they try to stay upright. One of the hallmark signs is an intention tremor of the head: the head shakes or bobs when the dog is actively trying to focus on something, like food or a toy, but stays relatively still at rest. Despite all this clumsiness, the dog retains full muscle strength.
Proprioceptive (Sensory) Ataxia
Proprioception is a dog’s ability to sense where its limbs are in space without looking at them. When the spinal cord or the nerves carrying this positional information are damaged, dogs lose that awareness. They may drag their paws, cross their legs while walking, or stumble on uneven ground. This type of ataxia is commonly seen with spinal cord compression from disc disease, particularly in breeds prone to back problems like Dachshunds.
What Causes Ataxia
Because ataxia is a symptom rather than a diagnosis, the list of possible causes is long. They range from mild and self-resolving to serious and progressive.
Inner ear problems are among the most common triggers of vestibular ataxia. Ear infections that spread to the middle or inner ear can disrupt balance signaling. Older dogs frequently develop what’s called “old dog vestibular disease” or idiopathic vestibular syndrome, where symptoms appear suddenly with no identifiable cause and typically improve over days to weeks.
Toxin ingestion is a frequent cause of sudden-onset ataxia, especially in younger dogs. Marijuana is one of the more common culprits. Signs typically appear within 60 minutes of ingestion, and most dogs recover within 24 to 36 hours. Dogs that eat highly concentrated edible products may take several days to fully return to normal. Other toxins that can cause ataxia include xylitol (a sugar substitute), certain medications, antifreeze, and some plants.
Spinal cord disease is a major cause of proprioceptive ataxia. Intervertebral disc disease, degenerative myelopathy, spinal tumors, and spinal infections can all compress or damage the cord and disrupt the signals that tell the brain where the limbs are positioned.
Brain tumors, strokes, and inflammatory brain diseases can affect the cerebellum or brainstem and produce cerebellar or vestibular ataxia. These causes tend to be more common in middle-aged to older dogs and often come with additional neurological signs beyond just poor coordination.
Breeds With Genetic Risk
Some breeds carry genetic mutations that cause the cerebellum to degenerate over time, a condition called cerebellar abiotrophy. In these dogs, the brain cells responsible for coordination are present at birth but gradually die off, leading to progressive ataxia that often begins in puppyhood or young adulthood.
Breeds with documented genetic predispositions include Jack Russell Terriers, Parson Russell Terriers, Gordon Setters, Old English Sheepdogs, Australian Shepherds, Kerry Blue Terriers, Belgian Shepherds, Italian Spinones, Hungarian Vizslas, and Australian Working Kelpies. Some breeds develop signs very early in life. Norwegian Buhunds and Finnish Hounds, for example, are predisposed to early-onset progressive cerebellar ataxia, while Coton de Tulears can develop a neonatal form that appears shortly after birth. Genetic testing is available for several of these conditions and can identify carriers before breeding.
How Ataxia Is Diagnosed
A veterinarian’s first job is to figure out where the problem is located within the nervous system. This starts with a thorough neurological exam, watching how the dog walks, testing its reflexes, checking how it positions its paws, and looking for nystagmus or head tilt. These observations narrow down whether the issue is in the inner ear, cerebellum, brainstem, or spinal cord.
From there, the specific tests depend on the suspected location and cause. Blood work and toxin screening can identify metabolic issues or poisoning. Imaging of the ear canal may reveal infection. For suspected brain or spinal cord problems, MRI is the gold standard for visualizing soft tissue detail. In some cases, analysis of cerebrospinal fluid (collected under anesthesia) can detect infection or inflammation. For breeds with known genetic conditions, DNA testing may provide a definitive answer without invasive procedures.
Treatment and Recovery
There is no single treatment for ataxia because the approach depends entirely on the underlying cause. A dog with a severe ear infection needs targeted antimicrobial treatment. A dog with spinal cord compression from a herniated disc may need surgery. A dog that ate marijuana needs supportive care, possibly including IV fluids and anti-nausea medication, while the toxin clears its system.
Some causes of ataxia are benign and resolve with time and supportive care alone. Old dog vestibular disease, for instance, often improves dramatically within the first few days, though a mild head tilt can sometimes persist permanently. Other causes, like progressive cerebellar degeneration or certain brain tumors, are more difficult to manage and may worsen over time.
Good nursing care matters regardless of the cause. Dogs with significant balance problems are at risk of injuring themselves by falling down stairs or off furniture. Keeping them in a safe, confined space with good footing (not slippery floors), helping them eat and drink if they’re disoriented, and limiting activity during the acute phase all support recovery. Your veterinarian may recommend physical rehabilitation exercises once the dog is stable, particularly for spinal cord conditions where rebuilding proprioceptive awareness can make a meaningful difference in mobility.
What Ataxia Looks Like Day to Day
If your dog has suddenly started stumbling, walking as though drunk, tilting its head to one side, or falling over, those are the classic presentations of ataxia. The onset pattern itself is a useful clue. Sudden onset over minutes to hours suggests toxin exposure, a vascular event (stroke), or acute vestibular disease. Gradual worsening over weeks to months points more toward a tumor, degenerative condition, or slowly progressing infection.
Pay attention to whether the signs are symmetrical or one-sided. A dog that consistently leans, falls, or circles to the same side likely has a problem on that side of the vestibular system or brain. A dog that stumbles equally in all directions, or whose hind legs seem to give out without a directional pattern, may have a spinal cord or cerebellar issue. These observations are exactly what your veterinarian will ask about, so noting the details early helps move the diagnostic process forward.
