What Is Azithromycin 250 mg Used For and How It Works

Azithromycin 250 mg is an antibiotic used to treat mild to moderate bacterial infections, most commonly in the lungs, throat, sinuses, and skin. It belongs to a class of antibiotics called macrolides, and you’ve probably seen it packaged as a “Z-Pack,” which is the most common way the 250 mg tablet is dispensed. It works by stopping bacteria from building the proteins they need to grow and multiply.

Infections Treated With Azithromycin 250 mg

The FDA-approved uses for this dosage center on four categories of infection:

  • Community-acquired pneumonia: bacterial pneumonia picked up outside a hospital setting, caused by common respiratory bacteria.
  • Acute flare-ups of chronic bronchitis: worsening bacterial infection in people who already have chronic obstructive pulmonary disease (COPD) or long-term bronchitis.
  • Strep throat and tonsillitis: azithromycin is not the first-choice antibiotic for strep throat, but it’s used when a person can’t take penicillin or amoxicillin.
  • Uncomplicated skin infections: bacterial infections of the skin and surrounding tissue caused by staph or strep bacteria.

All of these are considered mild to moderate infections. Azithromycin is an oral medication, so it’s prescribed when the infection doesn’t require IV antibiotics or hospitalization. It is not effective against viral infections like the common cold or flu.

The Z-Pack: How the 5-Day Course Works

The standard Z-Pack contains six 250 mg tablets taken over five days. On day one, you take two tablets (500 mg total) as a loading dose. On days two through five, you take one 250 mg tablet per day. That’s it: six pills, five days, and you’re done.

This short course is possible because of how azithromycin behaves in the body. After you swallow a dose, the drug moves rapidly out of your bloodstream and concentrates in your tissues, particularly at the site of infection. Tissue concentrations end up significantly higher than what’s circulating in your blood. The drug also has an unusually long half-life of about 68 hours, meaning it takes nearly three days for even half of it to leave your system. In practical terms, azithromycin keeps working in your tissues for several days after you take the last pill. So even though the course feels short, the actual duration of antibiotic activity extends well beyond day five.

How Azithromycin Fights Bacteria

Azithromycin blocks bacteria from making proteins, which they need to survive and reproduce. Without new proteins, the bacteria can’t grow, and your immune system can clear the infection more effectively. Beyond this core mechanism, azithromycin also disrupts something called quorum sensing, a chemical signaling system bacteria use to coordinate behavior in large groups. It also reduces biofilm formation, which is the protective slime layer some bacteria build around themselves to resist treatment.

One reason azithromycin works well for respiratory and tissue infections is that it accumulates inside immune cells called phagocytes. These are the same cells your body sends to the site of an infection, so the drug essentially hitchhikes to exactly where it’s needed and gets released in high concentrations right at the battleground.

Common Side Effects

Most people tolerate azithromycin without major problems. The most frequent side effects are gastrointestinal: nausea, stomach pain, diarrhea, and occasionally vomiting. These are typically mild and resolve on their own once you finish the course.

The more serious concern is a cardiac one. The FDA has issued a warning that azithromycin can cause changes in the heart’s electrical activity, specifically prolonging what’s known as the QT interval. In rare cases, this can trigger a dangerous irregular heart rhythm. People at highest risk include those who already have a heart rhythm disorder, low potassium or magnesium levels, a naturally slow heart rate, or who take other medications that affect heart rhythm. If you experience an irregular heartbeat, dizziness, shortness of breath, or fainting while on azithromycin, that warrants immediate medical attention.

Antibiotic Resistance

Azithromycin remains effective for most common respiratory infections, but resistance is a growing concern. Macrolide antibiotics like azithromycin are the first-choice treatment for Mycoplasma pneumoniae, one of the most common causes of “walking pneumonia.” A CDC-linked study of nearly 1,000 Mycoplasma samples found that about 2.4% carried genetic mutations making them resistant to macrolides. That’s still a low number in the U.S., but resistance rates are much higher in parts of Asia, where they can exceed 80% in some regions.

This is one reason azithromycin should only be used for confirmed or strongly suspected bacterial infections. Taking it for viral illnesses or “just in case” contributes to the development of resistant bacteria, making the drug less effective when it’s truly needed.

What to Know While Taking It

Azithromycin 250 mg tablets can be taken with or without food, though taking them with a meal can help if you’re prone to stomach upset. Take each dose at roughly the same time each day. Even if you start feeling better after a day or two, finishing all six tablets is important. Stopping early gives surviving bacteria a chance to regrow and potentially develop resistance.

Because the drug stays active in your body for days after the last dose, side effects or drug interactions can persist beyond the five-day course. If you’re taking antacids containing aluminum or magnesium, spacing them apart from your azithromycin dose helps ensure the antibiotic is absorbed properly.
