What Is Bacitracin Zinc Ointment Used For?

Bacitracin zinc ointment is a topical antibiotic used to prevent and treat minor skin infections from cuts, scrapes, and small burns. It’s one of the most common over-the-counter wound care products, first approved by the FDA in 1948, and you’ll find it in most household first aid kits. The “zinc” in the name refers to the specific formulation: bacitracin combined with zinc, which extends the product’s shelf life from about 2 years to 5 years and boosts its germ-killing effectiveness.

How It Works Against Bacteria

Bacitracin targets gram-positive bacteria, the category responsible for most common skin infections. It works by interfering with a molecule that bacteria need to build and maintain their cell walls. Specifically, it blocks a lipid that shuttles building materials from inside the bacterial cell to the outer wall. Without this recycling process, the cell wall weakens and the bacterium dies.

Zinc plays a direct role in this process. Bacitracin needs a metal ion to bind to its bacterial target, and zinc provides the highest potency of any option. That’s why the zinc salt formulation is the standard for therapeutic use rather than bacitracin alone.

Common Uses

The primary use is preventing infection in minor skin injuries. This includes small cuts from kitchen knives or paper edges, scraped knees, shallow abrasions, and minor first-degree burns like those from briefly touching a hot pan. The ointment creates a protective barrier over the wound while delivering the antibiotic directly to the skin’s surface, where bacteria are most likely to enter.

The FDA has also approved an ophthalmic (eye) formulation of bacitracin for treating superficial infections of the cornea and conjunctiva. This is a separate prescription product, not the same tube you’d pick up at a drugstore for wound care. The standard skin ointment should never be applied to the eyes.

Where Not to Use It

Bacitracin zinc ointment is designed for minor, surface-level injuries only. It should not be used on deep cuts, puncture wounds, animal bites, serious burns, or injuries covering a large area of skin. These types of wounds carry a higher risk of deep tissue infection and typically need professional treatment.

The ointment is for external skin use only. Keep it away from your eyes, nose, and mouth. For children, avoid applying it to the diaper area if the skin is broken or raw unless a doctor specifically directs you to. If you do apply it there, avoid tight-fitting diapers or plastic pants, which can increase absorption.

How to Apply It

Clean the wound gently with mild soap and water before applying a thin layer of ointment. You can cover the area with a sterile bandage to keep the ointment in place and protect the wound from dirt and friction. Reapply one to three times daily, or whenever you change the bandage.

Most minor wounds heal within a week. If you don’t see improvement after a few days, or if redness, swelling, or warmth around the wound increases, stop using the ointment and have the wound evaluated. Those signs can indicate a deeper infection that bacitracin alone won’t resolve.

Bacitracin vs. Neosporin and Polysporin

Bacitracin zinc ointment contains a single antibiotic. Polysporin combines bacitracin with a second antibiotic (polymyxin B), and Neosporin adds a third (neomycin) on top of those two. More antibiotics might sound better, but the practical differences are smaller than you’d expect.

Research comparing antibiotic ointments to plain petroleum jelly for post-procedure wound care found that antibiotics may not be necessary for satisfactory healing of minor wounds. The combination products did not consistently outperform simpler options. In one study, the bacitracin-polymyxin combination caused significantly more burning at the one-week mark, and one participant developed allergic contact dermatitis from it.

Neomycin, the extra ingredient in Neosporin, is a well-known contact allergen. If you’ve ever had a rash or itching from Neosporin, switching to plain bacitracin zinc eliminates that ingredient. However, bacitracin itself is not allergen-free.

Allergy Risk

Bacitracin allergy has become increasingly common. Epidemiologic studies show the rate of allergic contact dermatitis to bacitracin rose from 1.5% during 1985 to 1989 up to between 7.8% and 11.8% during 2011 to 2015. That’s a substantial increase, likely driven by decades of widespread over-the-counter use.

An allergic reaction to bacitracin typically looks like redness, itching, or a rash at the application site that worsens rather than improves with continued use. It’s easy to mistake this for a worsening wound infection, which can lead people to apply even more ointment and make the reaction worse. If a wound seems to be getting more irritated despite treatment, bacitracin allergy is worth considering. For people with a known allergy to bacitracin or zinc, plain petroleum jelly is a reasonable alternative for keeping minor wounds moist and protected during healing.

Does It Speed Up Healing?

The main benefit of any wound ointment, including bacitracin zinc, is keeping the wound moist. Moist wounds heal faster than dry ones because new skin cells can migrate across the wound bed more easily. Bacitracin zinc adds a layer of infection prevention on top of that moisture, which matters most for wounds that are in high-contact areas (hands, knees) or in environments where contamination is likely.

For very clean, shallow wounds in healthy people, the antibiotic component may offer little advantage over plain petroleum jelly. The real value of bacitracin zinc is in situations where the risk of bacterial contamination is higher: outdoor injuries, wounds that are hard to keep clean, or scrapes in children who aren’t great at keeping bandages on.
