Bayer low dose aspirin (81 mg) is primarily used to reduce the risk of heart attack and stroke in people with cardiovascular disease or a history of blood clots. It works by preventing platelets in your blood from clumping together, which makes dangerous clots less likely to form. For some people without existing heart disease, it may also be recommended as a preventive measure, though guidelines on this have narrowed significantly in recent years.
How It Prevents Heart Attacks and Strokes
Aspirin blocks an enzyme in your platelets that produces a chemical called thromboxane, which normally signals platelets to stick together. Without that signal, your blood is less likely to form the clots that can block arteries feeding the heart or brain. This is why doctors refer to low dose aspirin as an “antiplatelet” therapy rather than a blood thinner, even though the practical effect is similar.
The most common use is in people who have already had a cardiovascular event. If you’ve experienced a heart attack, stroke, or a “mini-stroke” (transient ischemic attack), daily low dose aspirin helps prevent a second one. It’s also prescribed for people with diagnosed cardiovascular disease who haven’t yet had an event but are at elevated risk.
Who Should Take It for Prevention
Guidelines from the U.S. Preventive Services Task Force draw a clear line based on age and risk level. For adults aged 40 to 59 with a 10% or greater chance of a cardiovascular event over the next 10 years, daily aspirin is a personal decision to make with a doctor. The task force describes the net benefit in this group as small, and it only tips in your favor if you’re not already at increased risk for bleeding.
For adults 60 and older who have never had a heart attack or stroke, the task force recommends against starting aspirin for prevention. The bleeding risks climb with age, and the protective benefit shrinks. For people already taking aspirin, modeling suggests it may be reasonable to consider stopping around age 75 for the same reason.
These guidelines apply to people without existing cardiovascular disease. If you already have heart disease or have had a clot-related event, your situation is different, and the benefit of staying on aspirin is generally much clearer.
The 81 mg Dose
The standard “low dose” is 81 mg per day, which is one quarter of a regular aspirin tablet. This amount is enough to suppress the clot-forming enzyme in platelets without delivering the higher anti-inflammatory dose used for pain relief. The task force specifically notes that 81 mg daily is a reasonable dose for people initiating aspirin therapy. Bayer also sells an extended-release capsule at 162.5 mg, but the 81 mg tablet remains the most widely used form for daily cardiovascular prevention.
Enteric Coating: Does It Help?
Bayer markets a “safety coated” version with an enteric coating designed to pass through the stomach and dissolve in the small intestine instead. The idea is to protect the stomach lining. In practice, research shows this coating provides minimal to no stomach protection. Rates of ulcers and bleeding are the same whether aspirin is coated or uncoated, because the bleeding risk comes from aspirin’s effects once it enters the bloodstream, not from where the tablet dissolves.
There’s actually a downside to the coating. Enteric-coated aspirin is absorbed more slowly and less completely than regular aspirin. If you’re taking it for daily prevention, this delayed absorption could reduce its effectiveness at keeping platelets in check. Chewing or crushing an enteric-coated tablet eliminates the coating’s effect and allows normal absorption.
Bleeding Risks
The main tradeoff with daily aspirin is bleeding. A systematic review of observational studies found that low dose aspirin roughly doubles the risk of upper gastrointestinal bleeding (stomach and upper intestine) and increases the risk of lower GI bleeding by about 80%. In absolute terms, GI bleeding occurred in roughly 0.5 to 3.6 cases per 1,000 people per year among aspirin users, so the overall risk for any individual remains low, but it’s not zero.
Aspirin also raises the risk of bleeding inside the skull (intracranial hemorrhage) by about 40%. People with a history of stomach ulcers, recent bleeding episodes, or conditions that impair clotting face higher risk. This is why the decision to start aspirin therapy involves weighing your cardiovascular risk against your bleeding risk.
Drug Interactions to Watch For
Several common medications interact with low dose aspirin in ways that matter. Blood thinners like warfarin and antiplatelet drugs like clopidogrel increase bleeding risk when combined with aspirin. Over-the-counter pain relievers like ibuprofen and naproxen belong to the same drug family as aspirin (NSAIDs), and taking them together raises the chance of stomach irritation and bleeding. Acetaminophen (Tylenol) is considered safe to take alongside low dose aspirin.
Other medications that can interact include certain blood pressure drugs, diabetes medications like insulin, lithium, methotrexate, and digoxin. Even some herbal supplements, such as turmeric, can increase bleeding risk when combined with aspirin. If you take any of these, your doctor needs to know before you start daily aspirin.
Aspirin During a Suspected Heart Attack
You may have heard that chewing an aspirin during a heart attack can be lifesaving. The American Heart Association no longer advises people to start aspirin on their own during a suspected heart attack. The priority is calling 911. Emergency personnel will decide whether to give you aspirin based on your symptoms, medical history, current medications, and test results. There’s no definitive evidence that taking aspirin 15 to 30 minutes earlier at home provides an added benefit over receiving it from the ambulance team or at the hospital.
