BOAS surgery is a set of procedures that open up the airways of flat-faced (brachycephalic) dog breeds like Bulldogs, Pugs, and French Bulldogs. These dogs are born with compressed skulls that crowd the soft tissues of the nose, mouth, and throat, making it harder for them to breathe. The surgery corrects the specific anatomical problems causing obstruction, and the full procedure typically costs between $1,500 and $5,000.
What BOAS Actually Is
BOAS stands for Brachycephalic Obstructive Airway Syndrome. It’s not a single problem but a combination of structural issues that develop because the dog’s skull was bred shorter while the soft tissue inside stayed roughly the same size. The result is a nose with pinched nostrils, a soft palate that’s too long and too thick for the space it occupies, and sometimes tissue inside the throat that gets sucked inward from the effort of breathing against resistance.
Not every flat-faced dog needs surgery. Veterinarians use a grading system from 0 to 3. Dogs graded 0 or 1 are considered clinically unaffected, though puppies under two or three may be monitored annually since the condition can worsen with age. Grade 2 dogs show moderate breathing difficulty and typically need weight management, surgery, or both. Grade 3 dogs have severe respiratory signs and should be evaluated for surgical correction.
The Three Main Procedures
BOAS surgery usually involves correcting up to three problems in a single session under general anesthesia. Which procedures your dog needs depends on what the surgeon finds during examination.
Widening the Nostrils
The most visible part of the surgery addresses stenotic nares, or pinched nostrils. The surgeon removes a small wedge of tissue from each nostril to widen the openings and let air flow in more easily. This is sometimes performed on its own in mild cases and costs $800 to $2,500 as a standalone procedure.
Shortening or Thinning the Soft Palate
The soft palate is the flap of tissue at the back of the roof of the mouth. In brachycephalic dogs, it’s often so long that it dangles into the airway, vibrating with each breath and causing the characteristic snoring and snorting sounds. The traditional approach simply trims the palate shorter. A newer technique called folded flap palatoplasty goes further: it addresses both the length and the thickness. In some flat-faced dogs, the soft palate can be four to six times thicker than normal, which contributes to blocked airflow through the back of the nose, exercise intolerance, lethargy, and sleep apnea. The folded flap technique removes excess tissue from inside the palate, making it both shorter and thinner.
Removing Everted Laryngeal Saccules
When a dog struggles to pull air through narrowed passages for months or years, the constant negative pressure can suck small pouches of tissue near the voice box (laryngeal saccules) inward, where they block even more of the airway. In severe cases, these saccules can occupy up to two-thirds of the windpipe opening. Removing them is a relatively simple procedure that opens the lower part of the airway, and complication rates are very low when done with specialized instruments.
Laser Surgery vs. Traditional Scalpel
Some veterinary surgeons use a CO2 laser instead of a scalpel, particularly for the soft palate and nostril work. The laser seals blood vessels as it cuts, so there’s minimal bleeding and better visibility during the procedure. For soft palate resection, most patients have no hemorrhage at all, and sutures often aren’t needed. For nostril repair, the laser allows more symmetrical tissue removal and a cleaner cosmetic result. Studies comparing the two methods show similar clinical outcomes, but laser surgery tends to be faster and simpler to perform.
What Recovery Looks Like
The first 24 hours after surgery are the most critical. Your dog will be kept intubated (breathing tube in place) until fully conscious and able to swallow on their own. The veterinary team monitors breathing rate and effort for at least 12 to 24 hours before sending a dog home. Water is withheld for the first 12 hours, then offered in small amounts. If that goes well, food is gradually reintroduced while the team watches for any signs of swallowing difficulty.
At home, expect some coughing and gagging in the days and weeks following surgery. This is normal and generally resolves over several weeks as the swelling goes down and the tissues heal. Your dog will likely wear an E-collar to prevent scratching or rubbing at the nose. Any sutures placed in the nostrils stay in for 10 to 14 days. During this time, keep exercise gentle and avoid anything that gets your dog panting hard or overheated.
How Much Improvement to Expect
Most dogs breathe noticeably better after BOAS surgery, and the improvements hold up over time. In a study published in Veterinary Surgery that compared short-term and long-term results using objective breathing measurements, post-operative scores remained stable rather than declining, meaning the benefits didn’t fade as months passed.
Owner satisfaction tells a more nuanced story. About 55% of owners reported that their dogs no longer had breathing problems after surgery. Another 39% felt their dogs still had some breathing difficulty, though objective measurements showed the two groups were actually quite close in their breathing scores. This gap likely reflects the reality that surgery significantly improves airflow but doesn’t give a flat-faced dog the respiratory system of a Labrador. Dogs with more severe disease before surgery may still snore or breathe noisily, even though they’re dramatically better than they were.
Cost Breakdown
The full corrective surgery, addressing nostrils, palate, and saccules, runs $1,500 to $5,000. That fee typically covers the physical exam, anesthesia, the surgery itself, and a follow-up visit. Antibiotics and pain medication are sometimes included, sometimes billed separately. Specialty veterinary hospitals charge more than general practices, but they also tend to have more experience with these procedures and better post-operative monitoring equipment.
Pre-surgical diagnostics add to the total. Why Timing Matters
BOAS is a progressive condition. The longer a dog struggles to breathe through narrowed airways, the more secondary damage accumulates. Constant effort to pull air through tight spaces creates negative pressure that can collapse structures in the throat, thicken tissues, and cause irreversible changes to the larynx. Early surgical correction, often recommended between one and two years of age, prevents these secondary changes and gives the dog the best possible outcome. Dogs who undergo surgery later in life still benefit, but they’re more likely to have permanent changes that surgery can’t fully reverse.
Weight also plays a significant role. Extra body fat deposits around the throat and narrows the airway further. For Grade 2 dogs, weight loss alone sometimes improves breathing enough to delay or avoid surgery. For dogs heading into surgery, being at a healthy weight reduces anesthetic risk and improves results.
