Clindamycin 150 mg is an antibiotic capsule used to treat serious bacterial infections, particularly skin and soft tissue infections, bone and joint infections, respiratory tract infections, and certain infections caused by anaerobic bacteria (bacteria that thrive without oxygen). It is not a first-line antibiotic for mild infections. Because of a specific risk involving the gut, clindamycin carries a boxed warning and is generally reserved for situations where safer alternatives aren’t appropriate.

Infections Clindamycin Treats

The 150 mg capsule is one of the most commonly prescribed strengths and is typically taken every 6 hours. For moderate infections, the usual adult dose is 150 to 300 mg per dose. More severe infections call for 300 to 450 mg per dose, still every 6 hours.

Clindamycin is effective against a range of gram-positive bacteria, including many strains of staph and strep. It’s a go-to option in communities where community-acquired MRSA is common, making it especially useful for skin infections like abscesses, cellulitis, and wound infections that don’t respond to standard antibiotics. It also works against many anaerobic bacteria, which makes it valuable for deep infections in the abdomen, pelvis, and lungs where oxygen-poor conditions allow these organisms to flourish.

One notable use is in combination with penicillin for infections caused by toxin-producing strep bacteria. Clindamycin helps shut down the bacteria’s ability to produce toxins, which penicillin alone doesn’t do as effectively. Dental infections, bone infections (osteomyelitis), and joint infections are other common reasons you might be prescribed this drug.

Clindamycin should not be used for most upper respiratory infections, such as the common cold or typical sinus infections. These are usually caused by viruses, and using clindamycin unnecessarily increases the risk of side effects without any benefit.

How Clindamycin Works

Clindamycin stops bacteria from growing by blocking their ability to build proteins. It latches onto a critical part of the bacterial machinery responsible for assembling proteins, physically getting in the way of the process. Without new proteins, bacteria can’t repair themselves, reproduce, or produce the toxins that cause symptoms. This is why clindamycin is particularly useful in toxin-driven infections: it doesn’t just kill the bacteria, it also reduces the harmful substances they release.

The Boxed Warning About Gut Infection

Clindamycin carries the FDA’s most serious warning label because of its association with a potentially dangerous gut infection caused by a bacterium called Clostridioides difficile (C. diff). Like all antibiotics, clindamycin disrupts the normal balance of bacteria in your colon. This can allow C. diff to multiply unchecked and release toxins that inflame the colon lining.

The severity ranges from mild diarrhea to life-threatening inflammation of the colon. Certain strains of C. diff produce higher levels of toxins, making the infection harder to treat and sometimes requiring surgery. Older adults and people with serious underlying illness are at higher risk and may tolerate the diarrhea less well.

The tricky part is timing. C. diff symptoms can appear while you’re taking clindamycin or up to two months after you’ve finished the course. If you develop significant diarrhea during or after treatment, especially if it’s watery, frequent, or contains blood, that warrants prompt medical attention. In most cases, the antibiotic will be stopped.

Common Side Effects

Digestive symptoms are the most frequent complaints. Nausea, vomiting, abdominal pain, and diarrhea are all reported. Some people notice a metallic or unpleasant taste in their mouth after taking the capsule. Esophageal irritation and ulcers have been reported, so it’s best to swallow the capsule with a full glass of water and stay upright for at least 30 minutes afterward.

Skin rashes are the next most common reaction, usually mild and appearing as a flat, slightly bumpy rash. Hives and itching also occur. Rarely, clindamycin can trigger severe skin reactions or a systemic allergic response, so any rapidly spreading rash, facial swelling, or difficulty breathing is an emergency.

Less common effects include abnormalities in liver function tests, temporary drops in white blood cell counts, and joint inflammation. These are uncommon enough that routine monitoring isn’t always required, but they’re worth knowing about if you develop unexpected symptoms during treatment.

Drug Interactions to Know About

Clindamycin has muscle-relaxing properties of its own, which means it can amplify the effects of neuromuscular blocking agents used during surgery. If you’re scheduled for any procedure involving anesthesia, let your surgical team know you’re taking it.

Clindamycin and erythromycin (another antibiotic) work against each other. They compete for the same binding site on bacteria, so taking them together reduces the effectiveness of both. These two should not be used at the same time.

Tips for Taking It

Take each dose with a full glass of water to protect your esophagus from irritation. Space your doses as evenly as possible throughout the day, since the every-6-hour schedule keeps the drug at a consistent level in your bloodstream. Finishing the full prescribed course matters, even if you start feeling better within a few days, because stopping early increases the chance that surviving bacteria will bounce back or develop resistance.

If you miss a dose, take it as soon as you remember unless it’s nearly time for the next one. Doubling up to compensate isn’t recommended. Store the capsules at room temperature, away from moisture and heat.
